Incident Summary- February 2023 - Manufacturing Plant, Khurda
 On 03/03/23 at 07.35 PM grinding activity was carried out by one of the
grinder on lower shell at Bay 2 Column No C-9.
 Just adjacent to the hot work area combustible materials like (Dye Penetrant
for NDT with developer, wastes clothes dipped in thinner, carton box) were kept.
 During grinding activity some spatter got contacted with the waste clothes
dipped in thinner and immediately the clothes caught fire.
 The heat generated from the fire caused formation of gases inside the developer
bottle and gradually lead to its explosion.
 One of the gas cutter acted promptly on the arising situation and rushed with
CO2 fire extinguisher (SMS/CO2/27) stored nearby and extinguished the fire.
 No personal injury or damage to property was reported.

Incident Summary- February 2023
 On 28/1/23 at 3.30 pm (approx.) One of the grinder was engaged in Grinding
activity of lower hood in front of the painting/blasting booth over the trolley .
 The grinder didn’t notice the empty paint drum which was kept nearby and
started grinding activity.
 Some of the spatters generated during the grinding activity came in contact with
paint drum with residual paint and immediately caught fire.
 One of the welder who was working near by acted promptly on the arising
situation and rushed towards DCP fire extinguisher stored nearby and extinguished the
fire.
 No personal injury or damage to property was reported.

Incident Summary- February 2023
 On 30/1/23 at 12.20 pm one welder was performing tig welding near the new
shop on LF Roof top section which was kept on a bench.
 As per the standard the activity was being performed holding filler metal in left
hand and torch in right hand with appropriate PPE’s.
 One the left side of the welder, an apprentice trainee was doing his work.
 The welder turned his left hand holding the hot filler metal to change his position
and at the same time the trainee also turned.
 Due to which the hot filler metal touched the trainee’s left eyelid.
 Immediately first aid was administered to the IP who sustained a burn injury
and was sent to hospital for further consultation and treatment.

Incident Summary- February 2023
Manufacturing Expansion Project, Khurda
 On 11/2/23 at 9.59 AM two riggers were engaged for fit up of side wall sag angle at
Grid H, Column No 17.
 During the fit up of side wall sag angle the top notching end of the sag angles were
fixed and the bottom notching end of the sag angles were left out without fit-up.
 While one of the rigger fitted this sag angle and tried to insert the next sag
angle by applying force on the purlin the pressed end of the previously fitted sag
angle got opened up.
 After loosening from the sag angle slot, the sag angle hit the column beam and
got deflected while falling down
 Due to this deflection the sag angle flew away from the line of fire and fell
on the ground nearby one personnel who just passing by.
 No personal injury was reported.
Top end of the sag angles is fixed and the bottom end of the sag angles is fitted later
During fit up of next sag angle by applying force on All activity on elevated platforms was stopped
the purlin the top notching end of the previously immediately and stand down was conducted
fitted sag angle got opened up and fell on the with site team for adequate communication
