1) An accident occurred on August 2, 2022 at 9:30 am where an inspector, Bayu Aji Santoso, scratched his right hand on a part pillar while using a torque tool during inspection. 2) First aid was provided on site and the victim was sent to the hospital where they received 4 stitches. An investigation into the root cause was initiated. 3) The accident occurred when the torque tool strength did not match the standard and released unexpectedly, causing the victim's hand to swing into the sharp part pillar. Unsafe actions and conditions were identified as not wearing protective gloves and no protective rubber on sharp parts.

Accident Chronology :
The victim when checking the Pillar UPR RH /
Model KS part is in accordance with existing
standards, then a check will be made by how the
part is clamped on the holder then to measure the
strength of the part using a Torque Wrench
calibration tool, the position of the hand on the
part, then when doing the torsion process with
the Torque tool, the hardware strength did not
match the standard and was released causing the
hand swing to hit the part, which resulted in a torn
wound on the right hand apron to penetrate the
skin, first aid was given to the victim and the
victim was taken to Siloam Hospital immediately
treated by a doctor and the wound got 4 stitches
How did it happened
- Unsafe Action : The victim puts his hand on a sharp object,
when he pulls the torque tool, it releases causing a hand
swing to hit the part
- Unsafe Condition : No protective rubber on parts
