This document outlines safety procedures for site management in the event of accidents or incidents at construction sites. It states that site managers must be notified immediately of any accidents or incidents and must take charge of the situation. This includes providing first aid, notifying emergency services if needed, and preventing further injuries. Site managers must also gather information about the accident and file reports as required by the company's safety policies and the Health and Safety Authority. Serious accidents require further investigation to identify causes and prevent future occurrences.

As per MJ Conroy Safety Statement;
• The site management must be notified of all accidents immediately.
• All persons on site are made aware of this requirement at site induction.
• The site management must take charge of the proceedings as follows:
• Notify a company trained first-aid person on site to attend the scene and to treat the casualty
• Observe the accident location and status of the injured person
• If there is a risk of further injury move the injured person to safety
• If necessary, call for medical assistance or the emergency services –doctor/ ambulance.
• If an ambulance is called, make sure the exact location is given and that the ambulance can access the site as near as
possible to the injured person
• Notify the company contracts manager and company safety officer of all serious accidents
• Establish the location of the hospital and appoint a suitable person to travel with the injury person

• In liaison with the Contracts Manager and Safety Officer; Site Management must:
• Gather immediately all information about the accident and what led up to it.
• Where necessary, obtain witness statements and write them down as they are given.
• Complete the site accident record book and furnish head office with a copy of the accident record
entry.
• Take sketches/photographs of the area where the accident happened.
• Assist in completion of company accident investigation form (MJC-S02)
Serious Accidents must be reported to Company Management immediately

Notification
Notification of a serious accident:
Must be made to the H.S.A. in the quickest means possible.
In the event of a death on site; the Gardai must also be informed.
If the H.S.A. are to inspect the site and location of the accident cordon off the accident area – do not disturb
it and do not permit anything to be moved unless necessary to avoid any further injury.
The Safety Officer is to notify the insurance company and company Loss Adjuster of all accidents.
If necessary, senior company management are to arrange to have the family of the injured person notified
and if required arrange to have them taken to the hospital.
All accidents are to be recorded in the accident record book held on site.
In addition, a company accident investigation form must also be completed (Form MJC/S02).

Accident Investigation
The investigation of the accident must identify;
1. The immediate actions taken as a result of the accident.
2. The immediate corrective actions to be taken to prevent recurrence.
3. The immediate causes of the accident.
4. The root causes of the accident.
5. The preventive action to be taken to prevent a future re-occurrence.
6. Close-out timescale for implementation of all actions.

Notification to the HSA
Where an employee is absent from work for more than three days following the accident (but excluding the day
of the accident) as a result of injury received in the accident, it must then be reported to the H.S.A. using the
Statutory IR1 form. This form is completed by the Safety Officer.
It is the responsibility of M. J. Conroy Construction to submit details (IR1 form) in relation any reportable
accidents to the Health and Safety Authority in respect of injury to any of their direct employees.
It is the responsibility of subcontractors to submit details (IR1 form) in relation any reportable accidents to the
Health and Safety Authority in respect of injury to any of their direct employees.
A copy of this completed form is to be issued to MJ Conroy Construction.

Incidents
For incidents involving MJ Conroy Construction, the company incident investigation
form (MJC-S13) will be completed.
The investigation of the incident must identify;
1. The immediate actions taken as a result of the incident.
2. The immediate corrective actions to be taken to prevent recurrence.
3. The immediate causes of the incident.
4. The root causes of the incident.
5. The preventive action to be taken to prevent a future re-occurrence.
6. Close-out timescale for implementation of all actions.

Dangerous Occurrence
In the event of a Dangerous Occurrence, which involves MJ Conroy Construction and which under the
regulations is notifiable to the Authority, then MJ Conroy Construction will complete and submit the
statutory form – IR3.
• Collapse/ overturn/ failure of lift/ hoist/ crane/ excavator
• Explosion
• Collapse of scaffold over 5 metres in height
• Unintended collapse of building/ structure under construction/ reconstruction/ demolition/ false
work involving fall over materials over 5 tonnes
• Contact with overhead power lines where voltage in excess of 220volts.
