During the Maneuvering of a manlift on the road located between Sphere tank and Condensate tank, the
manlift operator didn’t notice the short pipe
support structure (1.8 meter in height) while moving and accidently touch that pipe support result in minor
bent on the support.
To be highlighted that during this incident, no injuries were reported, only the pipe support sustained minor
damage.

While transporting a welding machine from AT12 to AT30 using a tractor for the routine activities, the tractor
trolley carrying welding machine was
passing through the road located nearby the 6" pipeline (newly installed gathering pipeline). Once the tractor
reached on a ramp, it lost its control and
move backwards, and the trolley connected with tractor hit the pipeline result in minor scratches on the pipe.
After detailed investigation the team believes that there was a failure in the brake system. Hence when the
operator applied break it fails and the tractor
along with trolley declined backwards from the ramp and hit on the pipeline.
To be highlighted that during this incident, no injuries were reported, however the pipeline sustained minor
scratches on its external body.

HYDRO DAMAS
On 27th November 2022, at around 14:30 hrs one of the Hydro Damas employees Mr. Boulahia Kamel with
his colleagues was trying to load a mini-roller compactor into the bucket of a backhoe loader. During this
course and after partially accommodating the compactor inside the bucket, the mini compactor got
disbalanced, as soon as Mr. Boulahia noticed that compactor is going to fall out from bucket he rushed into
support it, during this course of action, accidently the handle of the compactor came in contact with Mr.
Boulahia’s forehead resulting in a laceration wound.
The IP reported at PIUL clinic, where after receiving his medical treatment was discharged.
As per the witness statements, the incident occurred outside of CPF (near to the disposal pump area). No
evidence was found on the location.

On 27th January 2023 at around 11:00 AM, while SBT dump truck driver was reversing in between two 16”-
inch pipeline trenches to offload the sand, the driver miss judged to keep a safe distance from the trench’s
edge and drove/tilted his truck into the trench.
Earlier instructions were given by PIUL construction team to use a backhoe or bobcat for backfilling purpose
due to the road constriction, although the driver reversed for approximately 200 meters before ending up
tilted.
Investigation revealed that the foreman assigned for the task was inside the truck cabin along with the driver
as no one else was involved in this activity and no flag man was available to monitor the activity.
No injury was reported during the incident.

A crew from CSSI PIUL subcontractor were tasked to expose under ground cables manually at north side of
sales gas compressor at CPF, while
performing the task as per the permit to work and excavation certificate, the foreman decided to stop the
manual excavation and excavate mechanically
which has resulted in damaging of four telecom cables.
