Stop all work!
Act immediately! Sound the alarm and shout

Give priority to your own safety

If you are not trained to fight fires, leave immediately and shut the

5. Permit to Work System
- A formal written system to control certain type of work that is potentially
hazardous.
- A permit to work is a document that specifies the work to be done and the control
measures to be taken.
- It allows work to start only after safe procedure has been defined and it provides a
clear record that all foreseeable hazards have been considered.

where additional protection is needed.
• Workers must be trained to use and maintain equipment
properly.
• The employer and workers must understand the limitations of

97. Incident/ Accident Reporting Procedure
Action To Be Taken After An Accident/Incident
Personnel who witness the situation shall immediately
report to immediate Site Supervisor as follows:
Report to person in charge and Safety and
Health Officer or Safety and Health Supervisor
Give first-aid and CPR (if necessary)/Call
for first aider
Call ambulance (if necessary)
Cordon off the area
QHSE and HR Dept. shall be informed
within 24 hours
