This document outlines a program evaluation plan for a mental health services expansion project from 2009-2011. It presents the evaluator, Rob Rice, and his qualifications. The evaluation will use a basic systems model to measure inputs, processes, outputs, quality, outcomes, and feedback. Output and outcome measures are defined to track the percentage of school staff trained to make referrals, technical assistance provided, cultural competence, existence of linkage protocols between schools and mental health services, and student access to services. Schools are asked to identify staff to complete surveys measuring collaboration and identify point people to track student referrals and report data to the evaluator monthly.
