China has experienced tremendous disease spectrum change during past decade. One example is
increasing chronic wound problems due to systemic diseases that increase the health care
burden. As a solution, a Wound Healing Department was set up at Ninth People’s Hospital
Affiliated to Shanghai Jiao Tong University School of Medicine in Shanghai in 2011. Patients with
chronic wounds can be offered with both diagnostic and therapeutic support based on new
technologies. We established a model termed “link between hospital-based department and
community health care center”. This strategy provided combined wound management offered by
the community health clinics with the participation of hospital based specialists. A 4G
telemedicine system and wound information management system have been used to ensure
quick communication and standardized wound care between general hospital and community
health care center. Recent audit indicates that the model was facilitates qualified wound care at
reduced costs both aims being consistent with health-care reforms in China. Some limitations
have also been identified and are being addressed.

The classification of healthcare-associated infections within the HAIKU (Hospital Acquired Infections – Knowl- edge in Use) framework enables hospitals to consistently follow the standards along with their routine clinical practice and diagnosis coding to improve quality of care and patient safety. The HAI ontology (HAIO) groups over thousands of codes into a consistent hierarchy of concepts, along with relationships and axioms to capture knowledge on hospital-associated infections and complications with focus on the big four types, surgical site infections (SSIs), catheter-associated urinary tract infection (CAUTI); hospital-acquired pneumonia, and blood stream infection. By employing statistical inferencing in our study we use a set of heuristics to define the rule axioms to improve the SSI case detection. We also demonstrate how the occurrence of an SSI is identified using semantic e-triggers. The e-triggers will be used to improve our risk assessment of post-operative surgical site infections (SSIs) for patients un- dergoing certain type of surgeries (e.g., coronary artery bypass graft surgery (CABG)). According to the Centers for Disease Control (CDC) [2], hospital- acquired infections in the U.S., where the point preva- lence of HAIs among hospitalized patients is 4 %, result in an estimated 1.7 million infections, which lead to as many as 99,000 deaths and cost up to $45 billion annually [3, 4]. Similar or higher rates of HAI occur in other countries as well with an estimated 10.5 % of patients in Ca- nadian hospitals having an HAI [5]. Clinical assessment and laboratory testing are generally used to detect and confirm an infection, identify its origin, and determine appropriate infection control methods to stop the infection from spreading within a healthcare institution. AHRQ Impact Case Study Introduction After exploring the available case studies on the assigned websites Agency for Healthcare Research and Quality (AHRQ) Impact Case Studies site, I selected the case study titled Chicago Hospital Reduces Catheter-Related Infections to Zero with AHRQ’s CUSP Tools. (Agency for Healthcare Research and Quality, 2018). This seemed to be a great choice because, as a former ICU Registered Nurse, infection control was always at the top of our hospital’s quality initiatives. Summary of Case Study This case study was focused on reducing catheter-related infections in patients at Saint Anthony Hospital, which is a 150-bed hospital located in Chicago, IL. In order to achieve the goal of reducing catheter-related infections, the hospital adopted tools and strategies AHRQ’s Comprehensive Unit-Based Safety Programs (CUSP). The concept of team “huddles” was to have daily dialogue reporting of patients with special attention to those having catheters (urinary or venous). This awareness allowed for appropriate decision making regarding the need for the catheters to remain. The adoption of this method resulted in a 100% reduction rate of catheter-related infection between 2014 and 2017. If any barriers of catheter removal presented, such as provider preferences, they were addressed by hospital leadership. According to the case study, the medical director of infection control attributes the success of infection reduction to the use of huddles. They saw a reduction in readmissions, extended stays and a cost-savings of about $500,000 over a 2-year period. Conclusion Overall, the implementation of the huddles was a great achievement for the hospital. They were recognized by the Illinois Health and Hospital Association (IHA) and awarded the Innovation Challenge: Partners in Progress Award. This speaks volumes to the hospital's commitment to quality. Although I am no longer in direct patient care, I would say using the AHRQ CUSP tool and making it an all-inclusive practice was an excellent strategy by the hospital. Having all staff be accountable for seeing and reporting is vital to forward progress. The trickle-down effect is positive in that respect. Fewer catheters result in fewer infections which results in fewer readmissions resulting in cost-savings. That is a win-win for patients, providers and the hospital. This document describes the development of an evidence-based position statement on medical device-related hospital-acquired pressure ulcers (HAPUs) within a large healthcare system. A task force used the Iowa Model of Evidence-Based Practice to identify device-related HAPUs as an issue, review the literature, and define device-related HAPUs as injuries caused by external medical devices. They developed a position statement to standardize identification and reporting. Implementation involved disseminating the statement to various groups. Initial results showed improved identification and a 33% reduction in overall HAPU rates.
