Accident Investigation & Reporting
All injuries however trivial, any material damage, all near misses and environmental incidents MUST be
reported and recorded. All injuries must be recorded in the company first aid book as soon as practically
possible.
All incidents and injuries should be reported in the first instance to your Supervisor, the supervisor must;
Assess the nature of the incident to establish if it is of such severity as to require the provisions detailed
below to be implemented. If not ensure the area is safe for work to continue or otherwise.
The supervisor must report it verbally to the Site / Project Manager and or Safety Department – A.S.A.P.
At the appropriate time ensure that the details are recorded on Personal Injury Report CHSEP 4/F1 (accident
book)
All accidents which appear to be serious:
Serious incidents, dangerous occurrences, high potential near miss incidents or accidents which result in a
person reporting to hospital or which may cause him to be absent from work MUST be reported to the
Company HSEQ Director.
Where such incidents occur, the work area MUST be maintained and remain undisturbed for investigation
purposes. The area should be secured as far as reasonably practicable, access to the area restricted and the
scene preserved.
In such events DO NOT move, alter or interfere with anything in the area UNLESS this is required to give
assistance to injured persons or to prevent further injury potential. The area MUST be maintained until
instructed otherwise by the HSEQ Director or a company Director.
Any incident involving the collapse or failure of a crane or lifting appliance must be notified to the HSEQ
Director (or his nominated delegate) even if the collapse or failure does not result in damage or injury.
In any such incident again the work area MUST be maintained for investigation purposes. The area should be

be maintained until instructed otherwise by the HSEQ Director or a company Director.
All employees who are directly involved or who are key witnesses MUST remain near to the scene until they
are spoken to by the safety department and a statement taken (unless they are in need or urgent medical
treatment).
Make sure you know the emergency telephone number, the names of the first aiders and the emergency
arrangements for your current place of work, do it NOW before you may need that information when every
second is vital. IF IN DOUBT ASK
