This document provides emergency procedures and contact information for various emergency situations that may occur at Graves Construction sites, including fires, medical emergencies, severe weather, workplace violence, bomb threats, hazardous material spills, and property damage. It outlines steps for employees and supervisors to take to respond to and report each type of emergency.

4. Fire Emergency Procedures.
In the event a fire occurs, employees will
notify the site supervisor.
The site supervisor will immediately
verbally notify the appropriate safety
manager.
Minor fire- if it is safe to do so and the
employees have been properly trained, they
should assess the situation and extinguish the
fire by using one of the sites fire extinguishers.
5. Fire Emergency Procedures.
Major fire- if a major fire occurs or a
minor fire cannot be extinguished in less
than ten (10) seconds, employees
should evacuate the area. The
supervisor will summon off site
emergency response and should be
prepared to relay the following
information.
Type of emergency.
Severity of emergency.
Location and directions to the job site.
6. Fire Emergency Procedures.
The site supervisor will complete the
preliminary incident investigation and
fax or email it to the appropriate
Graves safety manager.
The site supervisor will submit the
final incident investigation in hard
copy form to the appropriate OCI
safety manager ASAP.
8. Injury Classifications
First Aid Injuries (Minor Injuries that
are treated onsite).
Non-Critical Injuries (Non-Life
Threatening Injuries Requiring Medical Care
Via Supervisor Transport in a Company
Vehicle).
Critical Injuries (Life Threatening Injuries
Requiring immediate Medical Transport &
Care).
9. What to Do for First Aid
Injuries.
Render Care to the employee.
Verbally notify your Office Safety Manager
Immediately. (If your unable to contact your Office
Safety Manager attempt to contact one of the other safety
managers).
Verbally notify your Supervisor.
Completed the First Aid Report and submit
it to you Office Safety Manager ASAP.
10. What to Do for First Aid
Injuries.
Complete the Incident Investigation
Report if requested by your safety
manager and submit it ASAP.
Continue to Monitor & Treat the Injury
until it has healed.
Verbally notify your office safety
manager if the condition of the injury
does not improve.
11. What to Do for Non-critical
Injuries.
Render First Aid Care
Verbally notify your Office Safety
Manager
Verbally notify your Supervisor
Transport the Injured to the
Clinic/Hospital
Request A Drug Screen be Performed
12. What to Do for Non-critical
Injuries.
Have the Treating Physician call your
Safety Manager immediately after they
have completed there assessment and
prior to providing care or medication to the
employee.
Complete the First Report of Injury and
Fax it to your Safety Manager.
Have the injured employee sign the
authorization to obtain information form.
13. What to Do for Non-critical
Injuries.
Complete the Preliminary Accident/Illness
Investigation and Fax or Email it to your
Safety Manager.
Complete the Incident Investigation Report
and submit it to your office Safety Manager
ASAP.
Continue to monitor & Treat the injury until
it has healed.
14. What to Do for Critical Injuries.
Call 911.
Render Care.
Verbally Notify you Office Safety
Manager Immediately. (If rendering
care direct someone else to notify).
Verbally Notify your Supervisor. (If
rendering care direct someone else to
notify).
Accompany or Follow the Injured
Employee to the Hospital.
15. What to Do for Critical Injuries.
Bring the Liberty Mutual Authorization for
Medical Information Release & the Drug
Test Authorization Form to the Hospital.
Verbally notify your safety manager of
arrival at the hospital and estimated time
of treatment.
Inform the Emergency Room that a drug
test needs to be performed.
Update your appropriate safety manager of
any information that you are given
regarding care.
16. What to Do for Critical Injuries.
Complete the appropriate state first report
of injury and fax it to you Safety Manger.
Have the injured employee sign the
authorization to obtain information form.
Complete the Preliminary Investigation
Report and Fax or email it to your safety
manager immediately.
Complete the Incident Investigation Report
and submit it to your safety manager.
Continue to Monitor and Treat the Injury.
18. Severe Weather Procedures
Graves Construction will monitor weather
conditions for impending severe weather.
When Severe weather is imminent, the
following procedures will be followed:
Thunderstorm/Lightening- In the event of a
thunderstorm, employees will be informed to
seek shelter through their Supervisors.
Employees will assemble in the shelter area
until given an “all clear” by the supervisor.
19. Severe Weather Procedures
Tornado- In the event of a tornado warning,
employees will assemble in an established
protected area preferably made of concrete or
block.
In all other cases, Supervisors will take employees to low
lying areas or ditches. Employees will remain in the
shelter area until the “all clear” notification is given.
21. Workplace Violence
Graves intend to provide working
environments for its professionals and
visitors that are free from violence.
Verbal or physical aggression and
threats of violence, whether directed at
a department head, supervisor,
coworker, client, vendor, or visitor, are
prohibited.
22. Workplace Violence
An employee who threatens, initiates,
or participates in any act of violence
while on the company’s or a clients
premises, including parking lots, will
be subject to disciplinary action up to
and including termination of
employment, arrest, and prosecution.
23. Workplace Violence
Any non-employee, who instigates or
performs an act of violence or
intimidation on company property,
including parking lots will be subject
to arrest and prosecution.
24. What to Do If an Act of Violence
Is in Progress.
Call 911 and report the situation.
Be prepared to direct the police to the
incident.
Do not attempt to intervene physically or
verbally.
If the situation merits it, advise those
nearby to move away form the area.
Follow Directions of law enforcement
officials when the arrive.
Do not evacuate the location unless
directed to do so.
25. What to Do for a Threat of
Violence.
If you hear, observe, or receive a threat,
contact your Safety Manager and Human
Resources Manager Immediately.
27. Bomb Threats
A majority of bomb threats to businesses are
hoaxes which result in nothing more that a
disrupted work routine. The threat must be
taken seriously, however, and appropriate
action taken

30. Bomb Threat Sequence of
Actions
If by phone, keep the caller on the line as long as
possible and get as much information as possible
(refer to checklist).
Signal others if possible to aid you in monitoring
the call.
If you were not the person receiving or adding in
monitoring the call leave the area as directed.
Immediately inform the your Safety Manager &
Supervisor, who will determine with the fire and
police departments whether the site should be
evacuated. Tell no one else.
32. Hazardous Material Spill
Emergency Plan
In the event a hazardous material spill
occurs, the employee (s) working in the
area will immediately notify the site
Supervisor.
The Supervisor will contact the appropriate
member of the Safety Department for
instructions on handling the spill.
If it safe to do so, the employees will stop the
spilled material from entering drains, creeks, or
other sources of water by using dirt berms,
plastic sheeting and absorbent material.
34. Property Damage Emergency
Response Plan
If an accident occurs which results in no injuries
but damage to property or equipment, the
employee (s) will immediately notify the site
Supervisor.
The Supervisor will contact a member of the
Graves Safety Department. Before moving any
equipment/materials involved in the accident,
photos will be taken by the site Supervisor to
document the positions of the
equipment/materials involved in the accident.
