Subject: Newsflash- Pre Start Checks
As many of you may already be aware, there was a significant incident on our Kemsley Energy from Waste
project earlier this month when a 160t Ainscough Crane overturned on site whilst it was being repaired by
Ainscough's maintenance team.
The incident occurred because the Crane Operator did not check that the out-riggers were fully deployed
before slewing round. Two outriggers were not extended, so the crane became unstable and overturned. This
damaged the EFW plant and the crane itself. Thankfully nobody was injured.
The attached Newsflashes have been produced for you to display on site as a reminder to those carrying out
plant & equipment repairs that this work is still subject to the normal rigours of Health & Safety Management
[inductions, risk assessments, permits etc].
Therefore, please ensure that:
You review your site arrangements to ensure all plant & equipment repairs are authorised and
undertaken in a planned and controlled manner.
A Risk Assessment is in place for future maintenance personnel to sign onto.
A process is in place to ensure fitters etc receive a suitable induction to work on site.
The maintenance personnel have their own risk assessment / prestart checks in place before
they start work.
Remember:
• The Principal Contractor is responsible for all works on a construction site.
• Hired plant & equipment, on hire under CPA terms & conditions, is the responsibly of the Hirer
from when it leaves the depot until when it arrives back; this includes: setting it up; during
inspections; whilst in use; and when maintenance and repairs are been carried out.

A 160t crane overturned on a construction site while undergoing maintenance, damaging the site and crane. The incident occurred because the crane operator did not fully deploy the outriggers before maneuvering, making the crane unstable. To prevent future incidents, the memorandum reminds site managers to ensure all plant and equipment repairs are properly authorized, planned, risk assessed, and personnel receive necessary inductions and safety briefings.
