Facility Abbreviations
• For the purpose of this manual, the following abbreviations are used: Fike Recreation Center (Fike),
Douthit Hills Fitness Center (Douthit), Upper and Lower Intramural Fields (Upper IM Fields and Lower IM
Fields), and Snow Family Outdoor Fitness and Wellness Center (Snow).
General Emergency Information
• Personnel must be aware of their role during an emergency. Position specific training should occur
in the area training.
• In the event of an emergency, the Recreation Supervisor should be notified immediately.
• Use best judgment and follow your level of training when taking action.
Building Security Coordinator
• Every major building on campus is assigned one or more building security coordinator(s) (BSC)
whose role is to facilitate the reporting of building, street, and grounds services needed by building
occupants. These services include maintenance, safety, security, renovations and construction, and
other miscellaneous needs.
• BSCs share the needs of building occupants with departments such as Facilities Management and
Clemson University Police Department (CUPD). Building security coordinators heighten the
awareness, knowledge, and issues affecting the quality of their work environment and the protection
of University resources.
• BSCs should be contacted prior to contacting CUPD for non-emergency facility related issues.
Media
• Student staff members are not designated nor authorized by Clemson University to interact with the
media or provide sensitive information to private inquires. This includes The Tiger student
newspaper.
o Always refer inquiries and media to Public Affairs (864-656-2061).
o Do not discuss an incident or injury with anyone other than Campus Recreation
Professional Staff, CUPD, and emergency personnel. Conversations should be
factual, without hearsay or opinion.

Emergency Action Plan for Injury
Life-Threatening or Serious Injuries (EMS Presence Requested)
*NOTE: You can always call EMS even if the victim refuses it. There is not cost for EMS to come, only if
they provide care or transport.*
• To Activate the Emergency Action Plan, alert the Recreation Supervisor over the Radio:
o “EMERGENCY INJURY IN (location), EMERGENCY INJURY IN (location). Call EMS.”
o If you do not have a radio, send an able participant to the nearest staffed area/desk.
o For Club Sports or Intramural Sports operating at outside fields, Managers should contact
EMS directly, then notify the Recreation Supervisor of the situation.
• Recreation Supervisor (or facility supervisor), wearing a first aid pack, will immediately respond to
the area via quickest route either acquiring Automatic External Defibrillator (AED) themselves or
requesting it brought to location by another able staff member. If blood borne pathogens are
present, also request a BBP Clean Up Kit brought to location.
• Recreation Supervisor will notify employees at Welcome Center to contact University Emergency
Dispatch Center at 864-656-2222 or 911. Welcome Center employees should be prepared to
provide the following information to the dispatcher.
o Nature of the Emergency
o Is the victim conscious, alert, and/or breathing
o Gender and age of victim
o Is there known previous history of this medical event
o Nearest access point to the emergency (please see Appendix 1 at the end of the EAP Manual)
• All Emergency Responders provide necessary care up to level of training until Emergency Medical
Services arrives and takes over.
• The primary responder must complete an incident report form and submit it to the Recreation
Supervisor. Staff Members and/or facility users witnessing incident should be listed on form with
contact information. *Keep witnesses separated whenever possible so they do not share their
accounts with others*
• At all locations, if EMS is contacted staff should notify the Recreation Supervisor.
• In Life-Threatening situations, Recreation Supervisor on duty will notify Building Security Coordinator
of the situation.
• In the event of death, employees involved with handling or witnessing the situation will be debriefed
individually immediately following incident by professional staff arriving at the scene.
Minor Injuries (EMS Presence Not Requested)
• Notify the Recreation Supervisor of the situation by radio and ask for assistance as needed.
o If you do not have a radio send an able participant to the nearest staffed area/desk.
• If the situation is questionable as to the severity, call and allow EMS to make determination.
• All responders provide necessary care up to your level of training.
• Recommend that the participant stop the activity. Always suggest that an injured participant seek
further evaluation by qualified medical personnel. Do not diagnose an injury, give an opinion, remark
on the severity of the injury, or give advice on treatment.
• Complete an incident report form as in major injuries above.

Accident/Incident Reports
• An accident or incident report will be completed for all accidents, incidents, and injuries in all Campus
Recreation programs and facilities.
• Reports should be filled out on Connect2 electronically using objective factual statements.
• Data from reports will be collected by the appointed department liaison and kept for future reference.
Copies of reports will be provided as necessary through the liaison.
• All reports are considered confidential department documents. Anyone needing a copy of a report
will need to contact the department liaison.
Employee Injuries
• If an employee is injured and needs care, their supervisor will call CorVel (1-866-282-2674) to
complete a First Report of Injury Form prior to the employee seeking care, unless life-threatening.
o If it is life-threating, call 911 immediately, and then call employee’s supervisor.
o If it is non-life-threatening, but medical assistance is needed, call employee’s supervisor, and
then employee should go to Redfern Healthcare Center. A Campus Recreation Injury Report
should be completed as soon as possible.
▪ If Redfern is closed, CorVel will direct employee to an authorized off campus medical
facility.
o If it is non-serious injury where care may not be sought, complete a Campus Recreation Injury
Report and have the employee contact their supervisor prior to seeking care. Reminder, the
employee’s supervisor must be notified and CorVel must be contacted prior to seeking medical
care, unless life-threatening.
Non-Department Group Reservations on Upper or Lower IM Fields After Hours
• Occasionally non-campus recreation groups may be approved to host events on the Upper or Lower
IM Fields that occur outside of Fike Recreation Center operational hours.
• Event Hosts are instructed to call CUPD at 864-656-2222 or 911 in case of emergency.
• An incident report or account of the incident with be completed by the Event Host for all
incidents/accidents/injuries and submitted to the Assistant Director of Events and Scheduling.

Automatic External Defibrillator (AED)
There are 4 AED’s located in Fike Recreation Center they are located in a white box attached to the wall.
• On entry level, by the Welcome Center next to Club Gym sight window
• On entry level by entrance to racquetball courts
• On ground level by Fitness Atrium restrooms by Club Gym
• On Pool Deck in First Aid Station on glass wall
There is one AED located in the Snow Family Outdoor Fitness and Wellness Center Storage Warehouse in
main area in a white metal container.
There is one AED located at Douthit Hills Fitness Center Welcome Desk.
There are 5 portable AED’s within the department utilized for programming.

Fire Alarms & Fire Extinguishers
Facility maps identifying locations of all Fire Alarms and Extinguishers can be found in the specific location
appendixes.
Personal Protective Equipment (PPE)
Campus Recreation supplies PPE for staff. All staff that roam the fitness facilities, including recreation
supervisors, operations managers/assistants, and lifeguard staff wear a fanny pack with first aid and PPE
supplies (gloves and breathing barriers). Additional gloves and barriers can be found in first aid cabinet at
the Welcome Center desk, Climbing Wall desk, and Pool First Aid Station. Intramural and club sports
managers also wear a fanny pack while supervising program events. Climbing wall and CORE staffs have
PPE within a first aid container within their work area.
First Aid Supplies
All staff that roam the fitness facilities, including recreation supervisors, recreation specialist, operations
managers/assistants, lifeguards, climbing wall specialist, wear a fanny pack with a breathing barrier, gloves,
and gauze. Portable First Aid packs and additional supplies are located at the Welcome Center and Pool
First Aid station.
Intramural, club sports managers, and CORE trip leader are equipped with full First Aid packs.

Radio Usage

5. Staff on channels other than CH 1 FIKE REPEAT will need to switch to channel 1 to notify
Recreation Supervisors of accidents involving BBP Clean Up or EMS presence or any incidents
requiring attention from CUPD. Recreation Supervisors will switch to all other channels in the event
of inclement weather, etc. to communicate as necessary.
6. Press and hold Power/Info Button to Turn Off
a. Radios MUST BE TURNED OFF TO CHARGE. Ensure light on charger illuminates. Radio will
also announce battery level on the initial charge even when radio is switched off.
b. For general Fike & Douthit radios, check radio back in via Welcome Center Staff.

Facility Emergencies
Utility Emergencies
• If you discover a significant water leak, gas leak, or other utility failure, call CUPD at 864-656-2222
• To report a non-emergency outage or repair request contact University Facilities at 864-656-2186
• Do not attempt to correct any problems unless trained and qualified to do so.
• Turn off power to equipment such as computers and fitness machines to avoid damage once power
is restored.
• Assist patrons in dark areas cautiously to an area that has emergency lights.
• If in an elevator use emergency button or telephone to alert the authorities. Do not attempt to open
elevator car door or shake or jar the car to move. Follow all instructions from emergency personnel.
• In the event of a power outage, all patrons should be advised to stop activity in places that may be
too dark. The Recreation Supervisor should call the BSC to determine a plan of action.
Elevator Failure
• If you become trapped in an elevator, either call 911, use an "Emergency Call Box" to report incident,
or activate the elevator emergency alarm within the elevator car.
• Stay calm. Keep other occupants calm.
• If you hear an elevator alarm, Call 911 or CUPD (864) 656-2222 and give the University Emergency
Dispatcher the following information:
o Name of the building.
o Location within the building.
o Where the car is stopped, if known.
o If a medical emergency exists.
• Do not attempt to exit the car unless directed to and assisted by emergency personnel.
• Elevators have mechanical safety brakes that will operate in all situations, even during power failures.
DO NOT attempt to open the elevator car door or in any way "shake" or "jar" the car to move.
• The activation of an elevator lobby smoke detector will cause building elevators to return non-stop
to the main floor and lock with the doors open. Never use an elevator to evacuate a building.
Chemical Exposures & Spills
• If toxic chemicals come in contact with your skin, immediately and continuously flush the area with
clear water. Remove contaminated clothing, move to a safe area and contact University Emergency
Dispatch Center at 864-656-2222 or 911 if medical assistance is needed.
• For spills, releases or incidents requiring special training, procedures, equipment (PPE) that is beyond
abilities of present personnel, take the following steps:
o Immediately notify affected personnel and evacuate the spill area. Pull the building fire
evacuation alarm if evacuation is required.
o Call CUPD at 864-656-2222 or 911 to report incident.
o On duty staff should evacuate the affected area at once, and seal it off to prevent further
contamination of others until the arrival of emergency personnel.
o Anyone who is contaminated by the spill should avoid contact with others as much as possible,
remain in the vicinity.
o Washing off contamination and any required first aid should be started immediately with eye
wash stations or showers.
o No effort to contain and/or clean up the spill should be made unless you are qualified to
perform such work.
o If an evacuation is required, follow the Facility Evacuation Plan.
o Do not re-enter the area until directed by emergency personnel.
o Safety Data Sheets for chemicals are located at the facility’s Right to Know Center, at the Fike
Recreation Center Welcome Desk or at the CORE Office.

Fire Emergencies
Response to fire alarms
• When an alarm sounds, leave the building immediately. Never assume it is a false alarm.
• Do not use elevators.
• Do not return for personal belongings.
• Help people who are unfamiliar with the building or people with disabilities escape the building.
If you discover smoke or fire
• If it is safe to do so, get everyone from immediate danger and close the door to the area to confine
the smoke and fire.
• Pull the building fire alarm along your evacuation route.
• If the fire is small and you are properly trained, control the fire with available firefighting equipment.
• Before you go through a door to leave a room, feel the door with your hand. Do not open any door
that feels hot.
• If there is smoke, stay low.
• Use stairs to reach an exit floor. Do not use elevators.
• After you are out of danger, call 911.

Aquatic Emergency Procedures
* In the event of a rescue or other medical emergency in the pool, the following procedures will occur in
addition to the regular facility emergency action plan. *
Primary Lifeguard: The lifeguard who recognizes and responds first to an emergency.
Procedures:
• Recognize the emergency.
• Activate the EAP by blowing whistle three (3) LONG and LOUD blasts and announce “CLEAR THE
POOL!”
• Survey the scene and determine the safety of the area.
• Approach victim and perform the appropriate rescue.
• Remove victim from water with secondary lifeguard and backboard (if needed).
• Further assess the status of the victim and provide care as needed.
• Comfort and aid the needs of the victim until EMS arrives.
Secondary Lifeguard:
Procedures:
• Use the radio to notify the Recreation Supervisor that there is an emergency in the pool.
• Assist in further clearing both bodies of water and announce “CLEAR THE POOL!”
• Retrieve the AED, CPR equipment, and Backboard.
• Provide appropriate assistance with care or removing the victim from the water.
• Help with crowd control.
Tertiary Lifeguard (if available):
Procedures:
• Assist the primary and secondary rescuers as needed.
• Serve as communicator between Recreation Supervisor and on-scene staff.
• Meet EMS and escort them using quickest path to the area of the incident.
After the victim is rescued and/or extricated from the water:
• Use basic precautions to prevent disease transmission, provide necessary care, and monitor vital signs
until EMS arrives and takes over.
• Keep the victim comfortable and alert if they are responsive.
• Take a brief patient history using S.A.M.P.L.E. (Signs and symptoms of the incident, Allergies,
Medications, Pertinent past medical history, Last oral intake, Events leading up to the incident).
• Provide relevant information to EMS as they arrive and allow them access to victim.
• While EMS is working with the patient provide crowd control and room to work.
After Emergency Situation concluded:
• Provide all relevant information to Recreation Supervisor for them to fill out an Incident Report.
• Write own statement or sign statement of Recreation Supervisor.
Debrief the staff: make sure equipment is cleaned, returned to original position, and in good working order,
lifeguards are mentally, physically, and emotionally ready to return to work, and the Recreation Supervisor
is satisfied. At this point the pool can be reopened.

Severe Weather & Natural Emergencies
Emergency Siren Warning
Two warning siren systems are present on the campus of Clemson University. When a siren sounds, staff
and users should:
• Stop what you are doing
• Evaluate your surroundings and immediately seek information
• Be prepared to take immediate action for safety
Oconee Nuclear Warning Siren
These sirens serve the 10-mile planning zone surrounding the Oconee Nuclear Station north of Seneca.
These sirens, located on campus and throughout the Clemson and Seneca communities, are used to notify
citizens of a variety of events that may occur at the plant. When activated, do not attempt to immediately
evacuate the area. Tune in to local media or other information sources for details before taking action.
Clemson University Outdoor Sirens
These sirens are located at Memorial Stadium, Fike Recreation Center, Brynes Hall, and the band practice
field on Perimeter Road. They are intended to warn individuals outside campus buildings of impending
severe weather and other emergencies. They may not be heard throughout campus or inside buildings.
When activated, seek shelter first then gather information to determine the nature of the emergency.
Severe Weather Warnings
Clemson University receives targeted weather information from SkyGuard. SkyGuard, a customized weather
management service, provides detailed information and delivers it via text to university officials and via a
web browser for those with sign-on credentials.
In the event of a severe weather warning, the Fike Welcome Center staff will receive a web notification and
inform the Recreation Supervisor. Either the Recreation Supervisor or professional staff members will
communicate with student staff to take appropriate response.
In the event of a lightning warning within 8 miles of campus, staff is to suspend all formal outdoor
programming, inform drop-in users on all Campus Recreation supervised outdoor fields, and clear all users
of the bodies of water in the natatorium. Staff will encourage patrons to seek shelter immediately. The
Recreation Supervisor will inform drop-in users, while specific programs will be responsible for informing
their participants.
If severe weather results in the issuing of a tornado warning, patrons will be directed towards Tornado
Safer areas.
• Fike Recreation Center: Lower Fitness studio, hallway between the club gym and Main Atrium, and
the Locker rooms.
• Douthit: Locker rooms, hallway to the back emergency exit, Back left corner of the Functional
Training Space
• Snow: The Bath House
• Seeking shelter will be communicated from the Recreation Supervisor to all staff per Clemson
University communication through CU Alert Systems, www.clemson.edu/cusafety, or
www.twitter.com/cusafety. If a tornado is visually present without issuance of shelter seeking from
Clemson University shelter will be sought.

Earthquake
Unlike other emergencies, the procedures to deal with an earthquake are much less specific. Since
earthquake magnitude cannot be predetermined, everyone must initiate emergency precautions within a
few seconds after the initial tremor is felt, assuming the worst possible case.
• Take cover.
• Call 911 or use "Emergency Call Box" if emergency assistance is necessary.
• Evacuate if alarm sounds or if told to do so by emergency personnel.
The best earthquake instruction is to take precaution before the earthquake (e.g., secure or remove objects
above you that could fall during an earthquake).
During the Earthquake:
• Remain calm and act, don't react.
• If indoors, seek refuge under a desk or table or in a doorway and hold on. Stay away from windows,
shelves, and heavy equipment.
• If outdoors, move quickly away from buildings, utility poles, overhead wires, and other structures.
CAUTION: Avoid downed power or utility lines as they may be energized. Do not attempt to enter
buildings until you are advised to do so by the proper authorities.
• If in an automobile, stop in the safest place available, preferably an open area away from power lines
and trees. Stop as quickly as safety permits and stay in the vehicle for the shelter it provides.
After the Initial Shock:
• Be prepared for aftershocks. Aftershocks are usually less intense than the main quake, but can cause
further structural damage.
• Protect yourself at all times.
• Evaluate the situation and call 911 for emergency assistance, if necessary.
• Do not use lanterns, torches, lighted cigarettes or open flames since gas leaks could be present.
• Open windows, etc., to ventilate the building. Watch out for broken glass.
• If a fire is caused by the earthquake, implement the fire procedures.

Unique Situations
Active Shooter
An active shooter is an individual actively engaged in killing or attempting to kill people in a confined and
populated area; in most cases, active shooters use firearm(s) and there is no pattern or method to their
selection of victims.
Active shooter situations are unpredictable and evolve quickly. Typically, the immediate deployment of law
enforcement is required to stop the shooting and mitigate harm to victims.
Because active shooter situations are often over within 10 to 15 minutes, before law enforcement arrives
on the scene, individuals must be prepared both mentally and physically to deal with an active shooter
situation. Good practices for coping with an active shooter situation include:
• Be aware of our environment and possible dangers.
• Take note of the two nearest exits in any facility you visit.
• If there is an accessible path, attempt to evacuate the premises. (RUN)
• If you are in an office, stay there and secure the door. If you are in a hallway, get into a room and
secure the door. (HIDE)
• As a last resort, attempt to take the shooter down. When the shooter is at close range and you
cannot flee, your chance of survival is much greater if you try to incapacitate him/her. (FIGHT)
Facility Lockdown
A lockdown is used when there is an immediate threat of violence in or around the university. A lockdown
minimizes access and visibility and shelters students, faculty, staff, and visitors in secure locations.
Faculty/Staff members are responsible for students and ensuring that no one leaves the safe area.
Lockdown procedures would only be invoked in situations, which constitute life-threatening events, and
where a facility evacuation could be fatal. A lockdown will be called by the President or his designee, the
Clemson University Police Department or other emergency responders. The Building Security Coordinator,
Recreation Supervisors, Public Safety, and/or Facilities Services will secure building entrances.
Suspicious Mail or Packages
• To ensure the safety of the campus population, all suspicious packages and bomb threats will be
considered real until proven otherwise.
• If a suspicious object or potential bomb is discovered, remain calm. DO NOT HANDLE THE OBJECT.
CLEAR THE AREA.
• Call 911 immediately to notify the Campus Emergency Dispatch Center.
• In most cases, these incidents are meant to disrupt normal activities.
• Building evacuation is not a decision for anyone to make except for proper authorities.
• Do not use cell phones, radios or other wireless devices.
• Any employee who had direct contact with the package should wash their exposed skin with soap
and water after reaching a place of safety.

Bomb Threat
(See next page for detail bomb threat check list)
• Remain Calm.
• Keep the caller on the line as long as possible.
• Listen carefully. Note the time of the call and the telephone number displayed if you have Caller ID.
• Ask the following questions:
o Where is the bomb?
o When will it explode?
o What does the bomb look like?
o What kind of bomb is it?
o What will cause it to explode?
o Did you place the bomb?
o Why?
o What is your name?
o Are you an employee?
o Where are you calling from?
• Write down any pertinent information such as background noises, gender of caller, and voice pitches
and patterns.
• Call 911 immediately to notify CUPD.

Fike Facility Evacuation Procedures Summary
In the event of an evacuation from Fike, employees should abide by the following protocol for primary and
secondary egress points, as well as holding locations after evacuating the facility:
• Area 1: Entrance (Main Lobby, Welcome Center, CRC, all levels of Offices)
o Primary Egress Doors: Main Entrance Doors, Side Stairwell Entrance Doors
o Secondary Egress Doors: Main Gym Doors, Club Gym Doors
o Holding Location: Lower IM Fields
• Area 2: Upper Atrium (Track, MPR, Cardio Hallway)
o Primary Egress Doors: Main Entrance Doors, Side Stairwell Entrance Doors
o Secondary Egress Doors: Main Gym Doors, Club Gym Doors

• Area 3: Lower Atrium (Free Weights, Pin-Select, Lunge/Strength Hallway)

During an evacuation, all Fike Employees will take direction from the Recreation Supervisor on duty.
Additionally, the Welcome Center will make the following announcement:
• Fike Patrons, due to an emergency please evacuate the building at the nearest available exit
immediately and proceed to the Upper or Lower Intramural Fields. Please cooperate with staff during
this evacuation, and wait further direction once you reach the Upper or Lower Intramural Fields.
DO NOT REENTER THE FACILITY UNTIL DIRECTED BY EMERGENCY PERSONNEL.

  • 3. 3 3 General Information Emergency Telephone Numbers • CUPD & CUFD: 864-656-2222 • Building Security Coordinator (BSC): Jimmy Mullinax 804-586-3223 o Assistant Building Security Coordinators: ▪ (Fike/Douthit) Haley Cox 678-218-2103 ▪ (Snow) Robert Taylor 864-985-8508 • Facilities Management (FMO): 864-656-2186 • Redfern Health Services: 864-656-2233 Facility Abbreviations • For the purpose of this manual, the following abbreviations are used: Fike Recreation Center (Fike), Douthit Hills Fitness Center (Douthit), Upper and Lower Intramural Fields (Upper IM Fields and Lower IM Fields), and Snow Family Outdoor Fitness and Wellness Center (Snow). General Emergency Information • Personnel must be aware of their role during an emergency. Position specific training should occur in the area training. • In the event of an emergency, the Recreation Supervisor should be notified immediately. • Use best judgment and follow your level of training when taking action. Building Security Coordinator • Every major building on campus is assigned one or more building security coordinator(s) (BSC) whose role is to facilitate the reporting of building, street, and grounds services needed by building occupants. These services include maintenance, safety, security, renovations and construction, and other miscellaneous needs. • BSCs share the needs of building occupants with departments such as Facilities Management and Clemson University Police Department (CUPD). Building security coordinators heighten the awareness, knowledge, and issues affecting the quality of their work environment and the protection of University resources. • BSCs should be contacted prior to contacting CUPD for non-emergency facility related issues. Media • Student staff members are not designated nor authorized by Clemson University to interact with the media or provide sensitive information to private inquires. This includes The Tiger student newspaper. o Always refer inquiries and media to Public Affairs (864-656-2061). o Do not discuss an incident or injury with anyone other than Campus Recreation Professional Staff, CUPD, and emergency personnel. Conversations should be factual, without hearsay or opinion.
  • 4. 4 4 Emergency Action Plan for Injury Life-Threatening or Serious Injuries (EMS Presence Requested) *NOTE: You can always call EMS even if the victim refuses it. There is not cost for EMS to come, only if they provide care or transport.* • To Activate the Emergency Action Plan, alert the Recreation Supervisor over the Radio: o “EMERGENCY INJURY IN (location), EMERGENCY INJURY IN (location). Call EMS.” o If you do not have a radio, send an able participant to the nearest staffed area/desk. o For Club Sports or Intramural Sports operating at outside fields, Managers should contact EMS directly, then notify the Recreation Supervisor of the situation. • Recreation Supervisor (or facility supervisor), wearing a first aid pack, will immediately respond to the area via quickest route either acquiring Automatic External Defibrillator (AED) themselves or requesting it brought to location by another able staff member. If blood borne pathogens are present, also request a BBP Clean Up Kit brought to location. • Recreation Supervisor will notify employees at Welcome Center to contact University Emergency Dispatch Center at 864-656-2222 or 911. Welcome Center employees should be prepared to provide the following information to the dispatcher. o Nature of the Emergency o Is the victim conscious, alert, and/or breathing o Gender and age of victim o Is there known previous history of this medical event o Nearest access point to the emergency (please see Appendix 1 at the end of the EAP Manual) • All Emergency Responders provide necessary care up to level of training until Emergency Medical Services arrives and takes over. • The primary responder must complete an incident report form and submit it to the Recreation Supervisor. Staff Members and/or facility users witnessing incident should be listed on form with contact information. *Keep witnesses separated whenever possible so they do not share their accounts with others* • At all locations, if EMS is contacted staff should notify the Recreation Supervisor. • In Life-Threatening situations, Recreation Supervisor on duty will notify Building Security Coordinator of the situation. • In the event of death, employees involved with handling or witnessing the situation will be debriefed individually immediately following incident by professional staff arriving at the scene. Minor Injuries (EMS Presence Not Requested) • Notify the Recreation Supervisor of the situation by radio and ask for assistance as needed. o If you do not have a radio send an able participant to the nearest staffed area/desk. • If the situation is questionable as to the severity, call and allow EMS to make determination. • All responders provide necessary care up to your level of training. • Recommend that the participant stop the activity. Always suggest that an injured participant seek further evaluation by qualified medical personnel. Do not diagnose an injury, give an opinion, remark on the severity of the injury, or give advice on treatment. • Complete an incident report form as in major injuries above.
  • 5. 5 5 Accident/Incident Reports • An accident or incident report will be completed for all accidents, incidents, and injuries in all Campus Recreation programs and facilities. • Reports should be filled out on Connect2 electronically using objective factual statements. • Data from reports will be collected by the appointed department liaison and kept for future reference. Copies of reports will be provided as necessary through the liaison. • All reports are considered confidential department documents. Anyone needing a copy of a report will need to contact the department liaison. Employee Injuries • If an employee is injured and needs care, their supervisor will call CorVel (1-866-282-2674) to complete a First Report of Injury Form prior to the employee seeking care, unless life-threatening. o If it is life-threating, call 911 immediately, and then call employee’s supervisor. o If it is non-life-threatening, but medical assistance is needed, call employee’s supervisor, and then employee should go to Redfern Healthcare Center. A Campus Recreation Injury Report should be completed as soon as possible. ▪ If Redfern is closed, CorVel will direct employee to an authorized off campus medical facility. o If it is non-serious injury where care may not be sought, complete a Campus Recreation Injury Report and have the employee contact their supervisor prior to seeking care. Reminder, the employee’s supervisor must be notified and CorVel must be contacted prior to seeking medical care, unless life-threatening. Non-Department Group Reservations on Upper or Lower IM Fields After Hours • Occasionally non-campus recreation groups may be approved to host events on the Upper or Lower IM Fields that occur outside of Fike Recreation Center operational hours. • Event Hosts are instructed to call CUPD at 864-656-2222 or 911 in case of emergency. • An incident report or account of the incident with be completed by the Event Host for all incidents/accidents/injuries and submitted to the Assistant Director of Events and Scheduling.
  • 6. 6 6 Emergency Access & Equipment Access Points • EMS should access the facility through the door closest to the incident. • Fike Recreation Center (Fike) o Front (Main Entrance) o Main Gym Court 4/Two-Court Glass Doors o Sun Deck o Club Gym • Upper & Lower Intramural Fields (IM Fields) Give the nearest street and/or intersection point. The field is surrounded by: o Highway 93 on the North o Heisman St. on the South o Williamson on the East for Upper IM and on the West for Lower IM o Always use directions (i.e. Northeast corner near Heisman St.) • Lightsey Fields o Give specific location (i.e. Northeast corner of field) o Off of Perimeter Road between Newman Rd Extended and Common’s Ct • Snow Family Outdoor Fitness and Wellness Center (Snow Beach) o Highway 93 near Seneca Creek Road o Give specific location (i.e. Northeast corner of lot) • Barn o Front two doors next to Tractor Room roll-up door o Two doors next to CORE storage roll-up door o Side door into CORE storage room o Bicycle Shop and Classroom side doors • CORE Office o Front double glass doors o Rear solid door • Douthit Hills Fitness Center o The second floor of the Hub o Enter through glass double doors (opposite of the main entrance) Automatic External Defibrillator (AED) There are 4 AED’s located in Fike Recreation Center they are located in a white box attached to the wall. • On entry level, by the Welcome Center next to Club Gym sight window • On entry level by entrance to racquetball courts • On ground level by Fitness Atrium restrooms by Club Gym • On Pool Deck in First Aid Station on glass wall There is one AED located in the Snow Family Outdoor Fitness and Wellness Center Storage Warehouse in main area in a white metal container. There is one AED located at Douthit Hills Fitness Center Welcome Desk. There are 5 portable AED’s within the department utilized for programming. • Intramural Sports • Club Sports • CORE • LoConte Family Field • Lightsey Field
  • 7. 7 7 Fire Alarms & Fire Extinguishers Facility maps identifying locations of all Fire Alarms and Extinguishers can be found in the specific location appendixes. Personal Protective Equipment (PPE) Campus Recreation supplies PPE for staff. All staff that roam the fitness facilities, including recreation supervisors, operations managers/assistants, and lifeguard staff wear a fanny pack with first aid and PPE supplies (gloves and breathing barriers). Additional gloves and barriers can be found in first aid cabinet at the Welcome Center desk, Climbing Wall desk, and Pool First Aid Station. Intramural and club sports managers also wear a fanny pack while supervising program events. Climbing wall and CORE staffs have PPE within a first aid container within their work area. First Aid Supplies All staff that roam the fitness facilities, including recreation supervisors, recreation specialist, operations managers/assistants, lifeguards, climbing wall specialist, wear a fanny pack with a breathing barrier, gloves, and gauze. Portable First Aid packs and additional supplies are located at the Welcome Center and Pool First Aid station. Intramural, club sports managers, and CORE trip leader are equipped with full First Aid packs.
  • 8. 8 8 Radio Usage 1. Press and hold Power/Info Button to Turn On a. For general Fike & Douthit radios, check radio out via Welcome Center Staff. 2. Press Power/Info button for information on battery level 3. Double tap Power/Info button for information on radio identification a. FIKE 1-23, DOUTHIT 1-12, SNOW 1-12, SPORT 1-12 4. Use scroll button on top of radio adjust channel: CH+ CH- CH1: FIKE REPEAT All programs and services in Fike – uses a repeater to amplify signal and reach farther – can reach all locations on campus CH2: SNOW REPEAT All programs and services at Snow – uses a repeater to amplify signal and reach farther – can reach all locations on campus CH3: DOUTHIT TA All programs and services at Douthit – radios will communicate within a 1-mile radius of each other CH4: SPORT TA May be used for Intramural and Club Sports programs inside facilities or outside at fields – radios will communicate within a 1-mile radius of each other CH5: CORE TA May be used for CORE program at Snow or on trips – radios will communicate within a 1-mile radius of each other CH6: CUREC 1 TA Open ‘talk around’ channel for semi-private discussions to be utilized by any facility or program – radios will communicate within a 1-mile radius of each other CH7: CUREC 2 TA Open ‘talk around’ channel for semi-private discussions to be utilized by any facility or program – radios will communicate within a 1-mile radius of each other 5. Staff on channels other than CH 1 FIKE REPEAT will need to switch to channel 1 to notify Recreation Supervisors of accidents involving BBP Clean Up or EMS presence or any incidents requiring attention from CUPD. Recreation Supervisors will switch to all other channels in the event of inclement weather, etc. to communicate as necessary. 6. Press and hold Power/Info Button to Turn Off a. Radios MUST BE TURNED OFF TO CHARGE. Ensure light on charger illuminates. Radio will also announce battery level on the initial charge even when radio is switched off. b. For general Fike & Douthit radios, check radio back in via Welcome Center Staff.
  • 9. 9 9 Facility Emergencies Utility Emergencies • If you discover a significant water leak, gas leak, or other utility failure, call CUPD at 864-656-2222 • To report a non-emergency outage or repair request contact University Facilities at 864-656-2186 • Do not attempt to correct any problems unless trained and qualified to do so. • Turn off power to equipment such as computers and fitness machines to avoid damage once power is restored. • Assist patrons in dark areas cautiously to an area that has emergency lights. • If in an elevator use emergency button or telephone to alert the authorities. Do not attempt to open elevator car door or shake or jar the car to move. Follow all instructions from emergency personnel. • In the event of a power outage, all patrons should be advised to stop activity in places that may be too dark. The Recreation Supervisor should call the BSC to determine a plan of action. Elevator Failure • If you become trapped in an elevator, either call 911, use an "Emergency Call Box" to report incident, or activate the elevator emergency alarm within the elevator car. • Stay calm. Keep other occupants calm. • If you hear an elevator alarm, Call 911 or CUPD (864) 656-2222 and give the University Emergency Dispatcher the following information: o Name of the building. o Location within the building. o Where the car is stopped, if known. o If a medical emergency exists. • Do not attempt to exit the car unless directed to and assisted by emergency personnel. • Elevators have mechanical safety brakes that will operate in all situations, even during power failures. DO NOT attempt to open the elevator car door or in any way "shake" or "jar" the car to move. • The activation of an elevator lobby smoke detector will cause building elevators to return non-stop to the main floor and lock with the doors open. Never use an elevator to evacuate a building. Chemical Exposures & Spills • If toxic chemicals come in contact with your skin, immediately and continuously flush the area with clear water. Remove contaminated clothing, move to a safe area and contact University Emergency Dispatch Center at 864-656-2222 or 911 if medical assistance is needed. • For spills, releases or incidents requiring special training, procedures, equipment (PPE) that is beyond abilities of present personnel, take the following steps: o Immediately notify affected personnel and evacuate the spill area. Pull the building fire evacuation alarm if evacuation is required. o Call CUPD at 864-656-2222 or 911 to report incident. o On duty staff should evacuate the affected area at once, and seal it off to prevent further contamination of others until the arrival of emergency personnel. o Anyone who is contaminated by the spill should avoid contact with others as much as possible, remain in the vicinity. o Washing off contamination and any required first aid should be started immediately with eye wash stations or showers. o No effort to contain and/or clean up the spill should be made unless you are qualified to perform such work. o If an evacuation is required, follow the Facility Evacuation Plan. o Do not re-enter the area until directed by emergency personnel. o Safety Data Sheets for chemicals are located at the facility’s Right to Know Center, at the Fike Recreation Center Welcome Desk or at the CORE Office.
  • 10. 10 10 Fire Emergencies Response to fire alarms • When an alarm sounds, leave the building immediately. Never assume it is a false alarm. • Do not use elevators. • Do not return for personal belongings. • Help people who are unfamiliar with the building or people with disabilities escape the building. If you discover smoke or fire • If it is safe to do so, get everyone from immediate danger and close the door to the area to confine the smoke and fire. • Pull the building fire alarm along your evacuation route. • If the fire is small and you are properly trained, control the fire with available firefighting equipment. • Before you go through a door to leave a room, feel the door with your hand. Do not open any door that feels hot. • If there is smoke, stay low. • Use stairs to reach an exit floor. Do not use elevators. • After you are out of danger, call 911.
  • 11. 11 11 Aquatic Emergency Procedures * In the event of a rescue or other medical emergency in the pool, the following procedures will occur in addition to the regular facility emergency action plan. * Primary Lifeguard: The lifeguard who recognizes and responds first to an emergency. Procedures: • Recognize the emergency. • Activate the EAP by blowing whistle three (3) LONG and LOUD blasts and announce “CLEAR THE POOL!” • Survey the scene and determine the safety of the area. • Approach victim and perform the appropriate rescue. • Remove victim from water with secondary lifeguard and backboard (if needed). • Further assess the status of the victim and provide care as needed. • Comfort and aid the needs of the victim until EMS arrives. Secondary Lifeguard: Procedures: • Use the radio to notify the Recreation Supervisor that there is an emergency in the pool. • Assist in further clearing both bodies of water and announce “CLEAR THE POOL!” • Retrieve the AED, CPR equipment, and Backboard. • Provide appropriate assistance with care or removing the victim from the water. • Help with crowd control. Tertiary Lifeguard (if available): Procedures: • Assist the primary and secondary rescuers as needed. • Serve as communicator between Recreation Supervisor and on-scene staff. • Meet EMS and escort them using quickest path to the area of the incident. After the victim is rescued and/or extricated from the water: • Use basic precautions to prevent disease transmission, provide necessary care, and monitor vital signs until EMS arrives and takes over. • Keep the victim comfortable and alert if they are responsive. • Take a brief patient history using S.A.M.P.L.E. (Signs and symptoms of the incident, Allergies, Medications, Pertinent past medical history, Last oral intake, Events leading up to the incident). • Provide relevant information to EMS as they arrive and allow them access to victim. • While EMS is working with the patient provide crowd control and room to work. After Emergency Situation concluded: • Provide all relevant information to Recreation Supervisor for them to fill out an Incident Report. • Write own statement or sign statement of Recreation Supervisor. Debrief the staff: make sure equipment is cleaned, returned to original position, and in good working order, lifeguards are mentally, physically, and emotionally ready to return to work, and the Recreation Supervisor is satisfied. At this point the pool can be reopened.
  • 12. 12 12 Severe Weather & Natural Emergencies Emergency Siren Warning Two warning siren systems are present on the campus of Clemson University. When a siren sounds, staff and users should: • Stop what you are doing • Evaluate your surroundings and immediately seek information • Be prepared to take immediate action for safety Oconee Nuclear Warning Siren These sirens serve the 10-mile planning zone surrounding the Oconee Nuclear Station north of Seneca. These sirens, located on campus and throughout the Clemson and Seneca communities, are used to notify citizens of a variety of events that may occur at the plant. When activated, do not attempt to immediately evacuate the area. Tune in to local media or other information sources for details before taking action. Clemson University Outdoor Sirens These sirens are located at Memorial Stadium, Fike Recreation Center, Brynes Hall, and the band practice field on Perimeter Road. They are intended to warn individuals outside campus buildings of impending severe weather and other emergencies. They may not be heard throughout campus or inside buildings. When activated, seek shelter first then gather information to determine the nature of the emergency. Severe Weather Warnings Clemson University receives targeted weather information from SkyGuard. SkyGuard, a customized weather management service, provides detailed information and delivers it via text to university officials and via a web browser for those with sign-on credentials. In the event of a severe weather warning, the Fike Welcome Center staff will receive a web notification and inform the Recreation Supervisor. Either the Recreation Supervisor or professional staff members will communicate with student staff to take appropriate response. In the event of a lightning warning within 8 miles of campus, staff is to suspend all formal outdoor programming, inform drop-in users on all Campus Recreation supervised outdoor fields, and clear all users of the bodies of water in the natatorium. Staff will encourage patrons to seek shelter immediately. The Recreation Supervisor will inform drop-in users, while specific programs will be responsible for informing their participants. If severe weather results in the issuing of a tornado warning, patrons will be directed towards Tornado Safer areas. • Fike Recreation Center: Lower Fitness studio, hallway between the club gym and Main Atrium, and the Locker rooms. • Douthit: Locker rooms, hallway to the back emergency exit, Back left corner of the Functional Training Space • Snow: The Bath House • Seeking shelter will be communicated from the Recreation Supervisor to all staff per Clemson University communication through CU Alert Systems, www.clemson.edu/cusafety, or www.twitter.com/cusafety. If a tornado is visually present without issuance of shelter seeking from Clemson University shelter will be sought.
  • 13. 13 13 Earthquake Unlike other emergencies, the procedures to deal with an earthquake are much less specific. Since earthquake magnitude cannot be predetermined, everyone must initiate emergency precautions within a few seconds after the initial tremor is felt, assuming the worst possible case. • Take cover. • Call 911 or use "Emergency Call Box" if emergency assistance is necessary. • Evacuate if alarm sounds or if told to do so by emergency personnel. The best earthquake instruction is to take precaution before the earthquake (e.g., secure or remove objects above you that could fall during an earthquake). During the Earthquake: • Remain calm and act, don't react. • If indoors, seek refuge under a desk or table or in a doorway and hold on. Stay away from windows, shelves, and heavy equipment. • If outdoors, move quickly away from buildings, utility poles, overhead wires, and other structures. CAUTION: Avoid downed power or utility lines as they may be energized. Do not attempt to enter buildings until you are advised to do so by the proper authorities. • If in an automobile, stop in the safest place available, preferably an open area away from power lines and trees. Stop as quickly as safety permits and stay in the vehicle for the shelter it provides. After the Initial Shock: • Be prepared for aftershocks. Aftershocks are usually less intense than the main quake, but can cause further structural damage. • Protect yourself at all times. • Evaluate the situation and call 911 for emergency assistance, if necessary. • Do not use lanterns, torches, lighted cigarettes or open flames since gas leaks could be present. • Open windows, etc., to ventilate the building. Watch out for broken glass. • If a fire is caused by the earthquake, implement the fire procedures.
  • 14. 14 14 Unique Situations Active Shooter An active shooter is an individual actively engaged in killing or attempting to kill people in a confined and populated area; in most cases, active shooters use firearm(s) and there is no pattern or method to their selection of victims. Active shooter situations are unpredictable and evolve quickly. Typically, the immediate deployment of law enforcement is required to stop the shooting and mitigate harm to victims. Because active shooter situations are often over within 10 to 15 minutes, before law enforcement arrives on the scene, individuals must be prepared both mentally and physically to deal with an active shooter situation. Good practices for coping with an active shooter situation include: • Be aware of our environment and possible dangers. • Take note of the two nearest exits in any facility you visit. • If there is an accessible path, attempt to evacuate the premises. (RUN) • If you are in an office, stay there and secure the door. If you are in a hallway, get into a room and secure the door. (HIDE) • As a last resort, attempt to take the shooter down. When the shooter is at close range and you cannot flee, your chance of survival is much greater if you try to incapacitate him/her. (FIGHT) Facility Lockdown A lockdown is used when there is an immediate threat of violence in or around the university. A lockdown minimizes access and visibility and shelters students, faculty, staff, and visitors in secure locations. Faculty/Staff members are responsible for students and ensuring that no one leaves the safe area. Lockdown procedures would only be invoked in situations, which constitute life-threatening events, and where a facility evacuation could be fatal. A lockdown will be called by the President or his designee, the Clemson University Police Department or other emergency responders. The Building Security Coordinator, Recreation Supervisors, Public Safety, and/or Facilities Services will secure building entrances. Suspicious Mail or Packages • To ensure the safety of the campus population, all suspicious packages and bomb threats will be considered real until proven otherwise. • If a suspicious object or potential bomb is discovered, remain calm. DO NOT HANDLE THE OBJECT. CLEAR THE AREA. • Call 911 immediately to notify the Campus Emergency Dispatch Center. • In most cases, these incidents are meant to disrupt normal activities. • Building evacuation is not a decision for anyone to make except for proper authorities. • Do not use cell phones, radios or other wireless devices. • Any employee who had direct contact with the package should wash their exposed skin with soap and water after reaching a place of safety.
  • 15. 15 15 Bomb Threat (See next page for detail bomb threat check list) • Remain Calm. • Keep the caller on the line as long as possible. • Listen carefully. Note the time of the call and the telephone number displayed if you have Caller ID. • Ask the following questions: o Where is the bomb? o When will it explode? o What does the bomb look like? o What kind of bomb is it? o What will cause it to explode? o Did you place the bomb? o Why? o What is your name? o Are you an employee? o Where are you calling from? • Write down any pertinent information such as background noises, gender of caller, and voice pitches and patterns. • Call 911 immediately to notify CUPD.
