しない.

 High-volume hemofiltraion
 Hemoadsorption (or hemoperfusion)
endotoxin,cytokineの吸着
 plasma exchange, plasma filtration
 Hybrid system
coupled plasma filtration adsorption
(CPFA) with resin cartridge
これらに関する報告は,みな規模が小さく,盲検化していない,
biasのリスクの高いものばかり.患者の選択が不明瞭で,方法
間のばらつきが大きい.

 目的:敗血症に対する血液浄化法は,動物実験
では予後を改善すると報告されているが,臨床
研究では,様々である. RCTをメタ解析して,血
液浄化法と死亡率の関係を明らかにする.
 Data源:1966年1月から2012年3月までの
各Database.

 全体では,血液浄化法で死亡率を50.1%から35.7%へ減少するが,日
本の単施設のPMX-B関連報告が影響している.

仏の多施設RCT The ABDOMIX Group
 目的:PMX-Bのhemoperfusion(HP,血液灌
流)が腹腔感染由来の敗血症性ショックの死亡率
や臓器障害を減少するかを調べる.
 方法:2010年10月から2013年3月までに,フラン
スの18のICUで行われた前向き,多施設RCT. 消
化管穿孔の為に行われた緊急手術症例243例.
術後12時間以内に,以下の2群に振り分けられた.
 PMX HP群
 標準治療群
主たる転帰は,28日後の死亡率,2次的転帰は90日後の
死亡率と,SOFA scoreの減少.

Protocol design
PMX-B:
• 膜:PMX-B5mg/polystyrene繊維1gをふくむToramyxinTM
• 1session 1時間
• 抗凝固:未分画ヘパリン2000-4000単位.
• 血流量:80-120mL/分.

 目的:Coupled plasma filtration
adsorption (CPFA)が炎症性メディエーターの
除去目的に提案されてきた. 本研究では,多施設
RCTでその有効性を検証する.
 デザイン:前向き,多施設,無作為,open-label,
並行2群,優位性,臨床試験
 設定:イタリアの18の成人一般ICU
 対象:敗血症性ショック患者192名
 CPFA群 10時間以上/日,5日間
 標準治療群
