Jarak mandibula – Tiroid notch 6 cm
atau > 3 jari
Mallampati Class I
7. 36
Seorang ahli anestesi harus memahami bahwa
penanganan cedera otak sekunder berdampak secara
dramatis pada morbiditas, mortalitas, dan kualitas hidup
pasien trauma kepala, terlepas dari besarnya cedera otak
primer.
8. TRAUMATIC BRAIN INJURY
37
Manajemen jalan napas yang tidak
direncanakan dengan benar dapat
membahayakan dinamika intrakranial dan
meningkatkan morbiditas dan mortalitas
Prediktor kematian pada pasien cedera otak
dewasa adalah:
• Hipoksia (Mortalitas naik 50%)
• Hiperkarbia (mortalitas naik 67%)
• Hipotensi sistemik yang mengakibatkan
serebral Hipoperfusi / penurunan CPP
• Hipertensi intrakranial
• Hipotermia
9. Perkiraan KLINIS pada pasien cedera
kepala :
9
• Drowsyness dan bingung (GCS 13-
15): ICP = 20-30 mmHg
• Brain swelling berat (GCS 8): ICP 30
mmHg
10. PERHATIAN
10
1) Pada TBI pergerakan kepala harus
diminimalkan mengingat potensi cedera
servikal
2) Sangat penting untuk memastikan bahwa
pemantauan yang tepat ada selama manuver
jalan napas.
3) Perhatian khusus pada pemasangan alat
bantu jalan napas dasar yang dimasukkan
secara blind melalui rongga hidung jika
didapatkan fraktur basilar dan cedera sinus:
• NPA (nasopharyngeal airway) atau NTT
(nasotracheal tube)
• NGT untuk dekompresi lambung
• Kateter Foley untuk mengontrol epistaksis masif.
11. ETT di dalam Anterior
Cranial Fossa setelah
intubasi nasotrakeal “blind”
pada pasien dengan fraktur
basal tengkorak
11
13. 42
Pasien trauma diasumsikan memiliki "perut penuh",
jadi penting untuk mempertimbangkan risiko aspirasi
selama laringoskopi dan intubasi vs. dekompresi
lambung dengan pemasangan NGT
14. 14
Jika upaya intubasi gagal dan
ventilasi Face
Mask/Supraglottic Airway
menjadi tidak mungkin & jika
situasi memungkinkan, ahli
bedah atau ahli anestesi yang
terlatih harus siap untuk
melakukan teknik jalan napas
invasif dengan CEPAT
18. Evaluasi Klinis
Untuk menegakkan cedera servikal, kriteria berikut harus
dipenuhi:
1. GCS 15 dan berorientasi baik
2. Tidak adanya cedera yang dapat mengalihkan perhatian
dari Cedera servikal
3. Tidak terdapat pengaruh obat atau minuman keras yang
dapat mengganggu sensori pasien
4. Pada pemeriksaan leher tidak didapatkan :
• Nyeri pada midline
• Gerakan aktif leher normal
• Tidak ada defisit neurologis akibat cedera servikal
48
19. Plain Radiography
• Pada cedera servikal, cross-table
lateral view saja, meskipun dibaca
oleh seorang ahli, masih tak
terdeteksi 15%.
• Pendekatan terbaik: #-view
cervical series
• Cross table lateral view
• Open-mouth odontoid view
• Anteroposterior (AP) view
49
20. Normal odontoid
cervical spine x-
ray view

50
In low-risk patients, plain
radiography is an
efficient diagnostic
examination with
specificity of 100%.
In high-risk patients, plain
radiography + CT scan =
sensitivity of 93.3% and
specificity of 95%.
21. Computed Tomography
• CT scan seluruh servikal mengurangi risiko missed
hingga < 1%.
• CT Heliks.
51
22. AIRWAY TECHNIQUES FOR ELECTIVE PATIENTS WITH UNSTABLE CERVICAL SPINE
1. Awake flexible fiberoptic intubation
2. Nasal intubation (if without basal skull &/or sinus fractures)
3. Indirect rigid laryngoscopy: Bullard, Wu, Upsher & TruView laryngoscopes
4. Videolaryngoscopy [DCI (direct coupled interface)/CMOS/wireless systems;
channeled/non-channeled]
5. Direct laryngoscopy with in-line stabilization
6. Fiberoptic intubation using appropriate SGAs as conduit (e.g. ILMA, ILA)
7. Lightwands (e.g. Trachlight)
8. Fiberoptic optical stylets [rigid/semi-rigid/”hybrids”]: Bonfils, Shikani, Clarus
video system, SensaScope, StyletScope, IntubaidFlex
9. Retrograde intubation
10. Percutaneous/Surgical airway: Cricothyrotomy, Tracheostomy
52
23. Alternative airway devices when the head
must remain immobilized in an emergent
setting
53
1. Video laryngoscopes
2. Indirect rigid laryngoscopes
3. Supraglottic airway as conduit for
Fiberoptic Intubation
4. Fiberoptic stylets
(rigid/malleable/”hybrid”)
5. “Invasive”
24. • Retrograde intubation dianggap
sebagai teknik alternatif pilihan
lokasi dengan sumber daya
terbatas.
25. Immobilization Options
1. Manual in-line immobilization (MILS)
2. Immobilisasi kepala diantara 2 sandbags
3. Rigid cervical collar and spinal board
• Morbiditas dan mortalitas meningkat
signifikan
• Kesulitan intubasi dan kompromi jalan napas
meningkat
• Risiko aspirasi
• Tidak selalu melindungi pergerakan pada
oksipito-servikal dan serviks-toraks

30. Unique challenges
1. Manajemen jalan napas dalam menghadapi
hipertensi intrakranial dan intrakranial yang
non komplians
2. Selama proses mencapai, mempertahankan,
dan/atau menyelamatkan jalan napas harus
dipikirkan juga :
• menyeimbangkan dan mempertahankan
hemodinamik SSP (CBF, CBV, CMRO2,
dinamika CSF),
• Cegah peningkatan ICP
• Mempertahankan perfusi serebral /
medulla spinalis
14
31. • Pada situasi klinis dapat terjadi:
• Obstruksi jalan napas dan
kesulitan ventilasi dapat menyebabkan
hypercarbia, hypoxemia, and
peningkatan CBF yang memicu
Hipertensi intracranial
• Laryngoscopy dan intubasi
menyebabkan peningkatan akut ICP
dan MAP
15
33. 1) Airway Assessment (histori dan
pemeriksaan fisik) pada pasien
neurosurgery membutuhkan :
33
a) Riwayat kesulitan manajemen jalan
napas sebelumnya (ventilasi masker,
laringoskopi, dan/atau intubasi)
memerlukan perhatian khusus.
34. b) Pasien dengan tanda dan gejala insufisiensi
vaskular intrakranial harus mendapat
perhatian khusus pada posisi leher tidak
hanya selama intubasi & pembedahan tetapi
juga pada periode perioperatif

39. • Pada praktik klinis, unexpected
difficulties airways terjadi pada 25-30%
kasus
50% dari jumlah ini terdapat “pseudo-
difficulties” yang disebabkan:
1) unskilled operators,
2) manuver yang tidak tepat
3) kurangnya pedoman/protokol kerja
39
40. 2) Selain anamnesis dan pemeriksaan
fisik, dibutuhkan pemeriksaan
• plain radiographs,
• computed tomography (CT)
• magnetic resonance imaging (MRI), &
• angiography
Dapat memberikan informasi
mengenai status intrakranial

46. Tindakan Rutin untuk mencapai
kontrol jalan napas yang optimal
&/atau intubasi “smooth" meliputi:
1) proper head positioning*,
2) preoxygenasi,
3) Dosis agen induksi (hipnotik,
opioid) dan relaksan (dengan
atau tanpa agen adjuvant)
yang tepat
30
47. • Teknik yang saat ini digunakan untuk
"menumpulkan" respons simpatis
terhadap laringoskopi dan intubasi:
1) Tambahan dosis thiopental atau
propofol &/atau opioid,
2) penggunaan beta-blocker atau agen
antihipertensi lainnya, dan
3) penggunaan lidokain intravena (IV)
47
48. PRINSIP UMUM DALAM PENATALAKSANAAN ANESTESI
UNTUK MENGHINDARI PENINGKATAN TIK
Technique Precaution(s)
Cegah Hipertensi Waspada terhadap perubahan derajat rangsangan nyeri.
Pastikan kedalaman anestesi yang memadai sebelum
intubasi upaya atau upaya bedah/prosedur.
Cegah hypoxia
Waspadai status pernapasan pasien.
Lakukan tindakan pencegahan untuk menghindari
aspirasi.
Preoksigenasi sebelum induksi anestesi atau intubasi
trakea.
Cegah hypercapnia
Waspadai status pernapasan pasien.
