This document is a questionnaire for families about creating a good school environment. It asks for background information on parents and then asks parents to rate different aspects of the school environment like relationships between teachers/families and students/teachers. It also asks parents to identify common conflicts like bullying, absenteeism, or vandalism. Parents are asked their views on responsibility for education and how they handle conflict situations. Finally, it asks parents to rank the importance of values like respect, obedience, and responsibility for their child's education. The goal is to get parental input to prevent and improve problems to create a better school environment plan.
