A 67-year-old woman presented with left ankle pain after a motor vehicle collision where she slammed her feet on the brakes before impact. On examination, she had an obvious deformity of the left ankle with edema and tenderness. Imaging showed she had sustained tri-malleolar fractures of the left ankle, involving fractures of the lateral, medial, and posterior malleoli. She required immediate orthopedic consultation and surgery to repair the unstable fractures.
