This document presents a detailed case history of a 63-year-old male who developed transfusion-associated graft-versus-host disease (TA-GVHD) following coronary artery bypass grafting (CABG) and multiple blood transfusions. Despite aggressive medical management, the patient suffered from severe complications and ultimately died due to rapid decline in hemoglobin levels, leading to a clinical diagnosis of viral-induced hemophagocytic syndrome and suspected TA-GVHD. The document outlines the pathophysiology, risk factors, clinical presentation, diagnosis, management, and prevention strategies for TA-GVHD.
