Syndromic Surveillance from Emergency Department Triage Notes
Background
Syndromic surveillance refers to reporting and tracking of reportable and unusual diseases to public health officials. Conventional surveillance strategies are often manual, or depend on confirmatory laboratory testing after a disease diagnosis. These traditional strategies often result in relatively late detection of an outbreak or public health emergency. Strategies for reliably accelerating surveillance are under active research.
The aim of our work is detection of specific syndromes in individual patient triage records in the hospital Emergency Department (ED). We focus on analysing the free text clinical notes written by a triage nurse during a brief pre-diagnostic assessment of a patient upon arrival in the ED. The system can detect patients that appear to have a disease of interest.
Methods
We work with a set of over 310,000 records collected in two Victorian EDs over a several-year period. Each patient triage record in our data includes (1) a free text note and (2) a diagnostic code from the International Classification of Disease (ICD-10) that was assigned after the fact. This data was used for training and testing of various classifiers, in a cross-validation scenario. We experimented with a range of different set-ups, including attempting direct prediction of ICD-10 codes for a given triage note, as well as prediction of “syndromes” defined by a specific set of ICD-10 codes. We also experimented with several different feature representations and machine learning models.
Results
In general, the performance of the models for syndromes was better than for direct ICD-10 category classification, suggesting that the syndrome definitions are clinically coherent. We observed substantial variation in performance across the various syndromes; several syndromes had too few examples in the dataset to build an effective classifier. The best performance on these tasks used a machine learning model that incorporates pre-processing of the texts to identify direct mentions of ICD-10 and SNOMED CT terms.
Conclusion
We have demonstrated that it is possible to build an effective syndrome detection tool for ED triage notes, where there is adequate and reliable training data available for a given syndrome of interest. We have shown that semantic abstraction of the text into “medical concept space” is of benefit for this task.

ABSTRACT:
After 13 years under private management, Hospital Prof. Fernando da Fonseca (HFF) committed to invest a solid EHR strategic approach aligned with best practices, due to management shift as public management hospital within the NHS network. HFF focused on creating a Clinical Commission for information that could support and lead the change on workflow and mindset of the hospital culture. This team combines the knowledge and experience of multidisciplinary roles, designing a EHR model that fully responds to the needs. Thus ensuring a successful phased implementation which started in the emergency department and was then deployed throughout the rest of the hospital. After three years of project, the hospital has achieved a high level (superior to 85%) of EHR adoption and has proven outcomes in the delivery of quality of care and Social ROI (e.g. direct data interoperability with medical emergency before hospital admission, algorithm implementation to improve organ donor and collection, integration with private clinics for outsourcing auxiliary services, including Lab and radiology prescriptions and results/reports). HFF continues to strongly work on this project and are dedicated to implementing their roadmap with the short term aim of achieving HIMSS Stage 6.

Informatics Tools and Patient Handovers
The patient handoff is a contemporaneous, interactive process of passing patient-specific information from one caregiver to another to ensure the continuity and safety of patient care. It is well recognized that the handoff is a point of vulnerability where valuable patient information can be distorted and omitted [1, 2]. A plethora of studies in the nursing literature have identified a variety of problems, including incomplete or inaccurate information [3-6], uneven quality [7], repeated interruptions and lack of anticipatory guidance [8]. Many reports have focused on characterizing the weaknesses with non-operative patient handovers, the use of handoff checklists and aviation safety models for specific groups of patients [1,5,9], and the pre- and post-implementation comparisons. [10-12] However, few studies have focused on prospective cohort studies validating and testing patient information management systems such as smart-templates in the setting of handover quality. [10]
Electronic templates containing patient information help to standardize the type of information conveyed during interactions, discourages ambiguous findings,[13] improves provider satisfaction and improves continuity of care.[14] Within the department, we developed the transfer template (T2) to address the issues in provider workflow and efficiency. With the press of a button, the T2 template automatically extracts live information from the anesthetic record, pertinent fields from the PAC note and laboratory values from IView, and provides a concise output of these relevant details.
