The document discusses various methods for assessing fetal well-being during pregnancy, including clinical evaluation, biochemical tests, and biophysical tests. Clinical evaluation involves monitoring the mother's weight, blood pressure, fetal growth and movement. Biophysical tests examine the fetus's heart rate and movement patterns in response to stimuli. These include the non-stress test (NST), biophysical profile (BPP), and contraction stress test. Ultrasound evaluates fetal anatomy and growth, while Doppler ultrasound assesses blood flow in vessels. Together these tools provide information on the fetus's health and help guide management to improve birth outcomes.
This document discusses endometrial hyperplasia, a condition characterized by an excessive proliferation of endometrial glands. It can progress to endometrial carcinoma if left untreated, making atypical hyperplasia a premalignant lesion. Risk factors include unopposed estrogen exposure from sources like estrogen therapy, obesity, or hormone-secreting tumors. The document classifies hyperplasia into simple, complex, and atypical subtypes based on histological features and lists their risks of progression to cancer, with atypical forms having the highest risk.
This document provides an overview of the clinical management of nonobstructive azoospermia (NOA). It begins by defining NOA and explaining its challenges. It then discusses the diagnostic evaluation and differentiates between obstructive and nonobstructive causes. For NOA due to spermatogenic failure, the document outlines that the condition is irreversible and reviews sperm retrieval techniques and their success rates depending on the underlying etiology. It also notes that while biomarkers can reflect testicular function, they cannot definitively predict whether sperm will be found for retrieval.
This document discusses male sexual dysfunction and erectile dysfunction. It begins with the anatomy of the penis including its nerve, arterial, and venous supply. It then discusses the physiology of erection and ejaculation involving autonomic, somatic, and central nervous system innervation and signaling. Types of erections are defined. The mechanism and evaluation of erectile dysfunction is explained involving history, examinations, questionnaires, and potential investigations. Finally, the various management options are outlined including psychosexual therapy, drug therapies like PDE5 inhibitors, intraurethral and intracavernosal injection therapies, vacuum devices, surgery, penile prostheses, and an algorithm for treatment.

Luteinizing hormone (LH) in synergy with follicle stimulating hormone (FSH) stimulates normal follicular growth and ovulation. FSH is frequently used in assisted reproductive technology (ART). Recent studies have facilitated better understanding on the complementary role of the LH to FSH in regulation of the follicle; however, role of LH in stimulation of follicle, optimal dosage of LH in stimulation and its importance in advanced aged patients has been a topic of discussion among medical fraternity. Though the administration of exogenous LH with FSH is obligatory for controlled ovarian stimulation in patients with hypogonadotropic hypogonadism, there is still a paucity of information of its usage in other patient population.A Brief introduction of Lh polymorphism in ovarian stimulation
This document summarizes research comparing outcomes of fresh embryo transfers versus frozen embryo transfers (FET). It finds that FET results in better pregnancy and implantation rates than fresh transfers, likely due to ovarian stimulation negatively impacting endometrial receptivity in fresh cycles. Specifically, FET cycles have higher success rates for slower developing embryos and embryos transferred in cycles with premature progesterone elevation. FET outcomes in young patients can rival fresh donor egg cycles. Randomized trials show significantly higher pregnancy rates with FET compared to fresh transfer in normal responders.
This document defines premature ovarian failure as the loss of normal ovarian function before age 40, affecting 1% of women. It can be caused by genetic disorders, autoimmune diseases, chemotherapy/radiation, or unknown factors. Symptoms include irregular periods, hot flashes, and fertility issues. The condition is diagnosed through blood tests of follicle-stimulating hormone and estradiol levels. Treatment focuses on hormone replacement therapy and calcium/vitamin D supplements to prevent osteoporosis and relieve symptoms.

This document discusses IVF treatment for polycystic ovary syndrome (PCOS). It begins with an overview of PCOS prevalence, definitions, and diagnostic criteria. IVF is indicated for PCOS patients who fail to conceive after ovulation induction or have other fertility factors. Patient preparation, gonadotropin protocols and monitoring, triggering ovulation, embryo transfer, and luteal phase support are discussed. Outcomes are better with GnRH antagonist protocols for PCOS patients due to lower gonadotropin doses and risk of ovarian hyperstimulation syndrome (OHSS). Primary and secondary prevention of OHSS includes metformin use, coasting, cryopreservation of embryos, and GnRH agonist triggering of ovulation.
Integrated approach to infertility work upnermine amin
This document outlines an integrated approach to infertility workup. It discusses evaluating the male partner, ovarian function, tubal function, and the uterus. For the male partner, a history, examination, and semen analysis are recommended. Ovarian function is assessed through menstrual history, ultrasound, and hormone levels. Tubal function can be evaluated with HSG, laparoscopy, or hysteroscopy. The uterus is examined with imaging like HSG, ultrasound, or hysteroscopy and potentially endometrial biopsy. A complete workup considers factors from both the male and female partners.
ICSI as it is presently performed is far from an ideal solution because the selection of sperm is based on the judgement of an embryologist, who is looking for the most normal appearing sperm available.
The document discusses the evaluation and treatment of erectile dysfunction. It states that the primary goals in managing ED are to determine the underlying cause, treat reversible factors through lifestyle changes and managing comorbidities, and treat the condition rather than just the symptom. The most common first-line treatment is phosphodiesterase type 5 inhibitors, but other options include penile injections, vacuum devices, and penile prostheses if other treatments fail.
The document provides information about andrology laboratory services for male infertility evaluation and treatment. It discusses:
- Tests offered including semen analysis, specialized tests of sperm function and morphology, sperm processing for infertility treatments, and cryopreservation.
- Procedures for semen sample collection, transport, and analysis following WHO standards, including macroscopic examination of volume, pH, and microscopic examination of motility, concentration, vitality, and morphology.
- Uses of semen analysis to diagnose infertility issues, identify treatment options, and assess effectiveness of treatments like vasectomy reversal. Computer-assisted semen analysis is also discussed.
Infertility is typically defined as the inability to achieve pregnancy after
one year of unprotected intercourse. If you have been trying to conceive
for a year or more, you should consider an infertility evaluation.
This document discusses endometriosis and its relationship to infertility. It covers several key points:
1. Endometriosis has three main types - peritoneal, ovarian, and rectovaginal - which are different entities.
2. Endometriosis can result in infertility through mechanical effects, endocrine abnormalities, changes to peritoneal fluid, immune system issues, and defects in oocytes.
3. Diagnosis is confirmed through laparoscopy, and mild or minimal endometriosis associated with infertility can be treated through laparoscopic destruction, expectant management, or GnRH agonists. Surgery aims to decrease inflammation and toxicity.
4. For endometriomas,
This document discusses poor ovarian response, which is defined as the failure to develop sufficient mature follicles following ovarian stimulation for IVF. Poor responders represent a heterogeneous group that can be divided into three categories based on age and hormonal profiles. Assessments of ovarian reserve like FSH, AMH, antral follicle count and CCCT help identify patients at risk. Treatment protocols aim to improve response through customized stimulation, LH supplementation, and adjuvants like DHEA, growth hormone, and anti-oxidants. Further research on tools like 3D Doppler and therapies like growth hormone may help optimize outcomes for poor responders.
there is a change in attitude for monofollicular ovulation induction to treat infertility: previously clomiphene citrate was the standard drug to start with : Now it is different
The document discusses delayed puberty, defined as the absence or incomplete development of secondary sex characteristics past a certain age. It then focuses on hypogonadotrophic hypogonadism, which affects about 1 in 10,000 male births and can be associated with loss of smell. Sixteen gene defects have been linked to Kallmann syndrome. Short term goals of treatment include attaining appropriate sexual development and growth, while long term goals include maintaining normal hormone levels and inducing fertility. Pretreatment with FSH before GnRH therapy has shown promise in inducing testicular growth and fertility in men with congenital hypogonadotropic hypogonadism and underdeveloped testes.

[Gross S. , 1887; Gallo L. et al., 2010 ].
- По сенсорной теории данная форма заболевания возникает из-за
врожденной повышенной чувствительности головки полового
члена [Shapiro B., 1943].
Причины ВПС:
- Хронический простатит (ХП) в 56% случаев обуславливает ПС
[Screponi E. et al., 2001].
- Гипертиреоз (ГТ) в 50% наблюдений может сопровождаться ПС
[Carani C. et al., 2005].
