1. General
This Safety, Health and Environment (SHE) plan is established based on Engineering and
Construction Industry practises and will be discussed and developed with COMPANY prior to
starting construction phases as may required. The SHE Management System will ensure the work
process from commencement through work completion are performed safely, healthy and
environmentally.
Contractor will base the site SHE programs on the following 5 pillar :
a) Identify/ recognize
• Site location & condition (live plant site, hill site, etc)
• Character of the operation on working tools, vehicles, all kind of equipment’s (operating
manual, routine maintenance, run test of equipment).
• Worker/ labor health condition (mental/ spirit, boring, health, fitness)
• Background of people involved in construction, with respect to safety awareness
• Over working hours (following Standard Working Hours, limited overtime of working)
• Loss of obidience of work/ safety regulation.
b) Culturization of safety (safety minded) by having:
• Safety messages for any kind of Site Construction Meetings
• Induction, Tool Box, and Management Meetings.
• Management Policy shown by brochures, banners, notification boards, bulletin, circular,
site management memo.
• Fire fighting and emergency evacuation training.
• Safety Training
• Safety Promotion and Awards
c) SHE Supervision
d) Refer to safe work procedures & rules
e) Safety inspection and audit in all activities

2. Corporate Safety Policy
CONTRACTOR Corporate Safety Policy which is established by CONTRACTOR Management,
will be implemented on all CONTRACTOR Projects. The policy reflects CONTRACTOR full
commitment to Health, Safety & Environmental Management.
All CONTRACTOR employees shall be responsible for ensuring that Health, Safety and
Environment policy is properly implemented in their works.
3. Project Safety, Health and Environment
CONTRACTOR will organize a safety committee which is consist Project Manager, Site
Manager, SHE/ Security Manager, Construction Manager and others.

3. COORDINATION
4.1 Internal Coordination
The role of Committe Secretary is to provide guideline to the management on the process
of work, planning, implementing, monitoring in sustaining a timely and cost effective
safety system.
The safety Committe goal is that CONTRACTOR shall achieve a level of consistenly
safe performance that assures CONTRACTOR recognition by COMPANY, Supplier’s/
Subcontractor’s and CONTRACTOR employees, and other stakeholders.
Membership of the safety committe includes representatives from COMPANY,
CONTRACTOR and all of Subcontractor’s Representatives that chaired by
CONTRACTOR General Site Manager.
This Committe should conduct safety committe meeting at least once a month, or if any
accident happen.
The effectiveness of the agenda meeting, the committe members shall carry out general
safety patrol one day prior to the meeting schedule.
4.2 External Condition
An external coordination shall be established with the Government such as police
department, local hospital, local government etc.
4. IMPLEMENTATION OF PROGRAMS AT SITE
The competent person should identify the potential problem or should asses the potential risk
prior the start of work.
There should be written instruction of work for the high-risk task of work.
Self-protection equipment should be provided and maintened properly during the execution of
work.
All signboard should be placed where the potential risk will occur and should be easily known by
everyone in the job site.

The Site Safety Programs takes into consideration the following elements as detailed in the

5.1 Basic SHE Rules
Based on the construction hazards analysis, basic safety, health and environment requirements are
developed and issued to ensure the safety on all operations planned.
These basic requirements which are listed below are documented in specific detail in the site
safety practices:
1. Prevention of Work Accident/ Injuries in :
• Electrical Work
• Work at height
• Grinding machines
• Welding and Oxygene Cutting
• Cartridge hammers
• Hazardous materials and products
• Dangerous machinery
• Excavation or earth moving and civil construction work

5.2. SHE Supervision
The job site teams from Foreman level up to Top Site Management shall continuously review site
condition, planned construction work, and other site activities to observe safety hazards and unsafe
acts by all personnel, and perform corrective mesures to eliminate any potential accident.
5.3. First Aid Procedures
Prevention is the key element used to minimize the severity of occupational injuries and illness.
However, unpredictable events do occur, and it is important that an emergency medical facility
shall be defined so that prompt and compotent medical treatmen is provided to an injured or ill
employee.
CONTRACTOR will provide First Aids Kits during the start of and during site works.
Appropriate protocols and reporting procedures are developed and pre arranged agreements are
established with the COMPANY Medical Facility and/ or with the nearest emergency medical
resources (doctors, hospitals, ambulances)

5.4. Personnel Training
This section outlines the types of information and training required for all employees and
supervisors prior to the start of and during site work, especially to develop the local workers.
CONTRACTOR shall provide copies of training records or completion certificates for each
employee, documenting date of completion for each training session.
5.4.1. New Hire Orientation/ Safety Induction
The project program for employees and supervisors includes all workers rgardless of
number of years worked in construction.
The orientation-induction program will include, but not limited to :
• Personnel Identification
• Basic SHE Rules
• PPE
• Emergency preparadness
• COMPANY SHE – Security Regulations.
5.4.2. Supervisor Orientation
All Supervisors assigned to the project are indoctrinated to their responsibilities. They are
required to learn and are requested to enforce all CONTRACTOR/ COMPANY rules
applicable to their work. They are expected to set an example for their sub-ordinates and
co-workers by their compliance with work rules and their aggressive leadership in safety.
Subject covered during indoctrination sessions includes ( in addition to induction
materials as above in article 5.4.1 herein before ) :
• Supervisors Safety Responsibilities
• “ STOP “ training and enforcement of safety rules
• Safety-motivation
• Safe practices for spesific crafts
• Accident Investigation
• Conducting effective safety meetings
• Employee right-to-know law hazardous substances
• Cient requirements
• Effect of accidents on productivity
• Drug & alcohol abuse awareness training
• Government requirement

5.4.3. Supervisor Safety Training
Safety Training which is considered to be an integral part of CONTRACTOR safety
program and part of the productivity improvement program, is impelemented on the
project.
All supervisors on the project attend and complete the safe task assignment and
enforcement training (STOP) program as well as other required technical safety
instruction courses.
5.5. Work Permitting
In order to monitor and control potentially hazardous work on the Job Site, work permits are
required to perform work of any nature within the limits of any processing unit or in new
construction where special hazards may occur. Work permit from company shall be as per
COMPANY regulation.
Minimum conditions that must be complied with include:
• Locating all buried utility/ lines before excavation as applicable.
• Providing fire protection and observing no smoking rules as applicable.
• Providing personal protective equipment.
5.6. Fire Prevention/ Protection Plans
In order to prevent fire from occuring, emphasis is placed on pre planning; hot-work permit
controls; flammable gas, liquid and material control; control of smoking, training and use of
warning signs; proper electrical wiring; and proper waste storage and removal. Specific
procedures are outlined in the construction site safety and health plan for the project.
5.7. Emergency Response
Emergency response procedures are developed for all potential incidents including fire, explosion,
weather disturbances, etc. This procedure contain details on cumminication, fire fighting, medical,
security, evacuation, resumption of operations and other details as may be deemed required.
The employees are periodically reinforced through meetings (e.q, tool box meetings), subsequent
training and drills, and procedures that are posted.

5.8. Housekeeping
Good housekeeping is an important part of the site safety program. It is the responsibility of all
employees to practice good housekeeping before, during, and after all activities in order to
maintain a clean, orderly job site.
Work sites, shop areas, and all laydown areas are kept clear of scrap and unnecessary material.
Materials necessary for work are properly stacked and kept out of the roadways to allow proper
access for fire fighting equipment and ambulance.
Adequate toilet facilities, cleaning and disposal services, and sources of portable water are
supplied as required.
5.9. Environment Protection
Supervisor shall utilize appropriate procedures and protective equipment control measures to
reduce and eliminate all identified sources of significant environment hazards to supervisors.
Workers are trained in these procedures as required.
5.10. SHE Inspection and Audits
The main emphasis of CONTRACTOR Safety Program is the prevention of accidents and injury.
The safety procedure provide guidance on how this goals can assess each project progress and if
needed, improve progress toward these goals, SHE Inspections of the entire project. The result of
these inspections, as with the monthly inspections are recorded on the loss prevention inspection
report. Copies of the inspections report and summary sheet are forwarded to the appropriate
manager for corrective action as necessary.
Safety Audits
The audits conducted by CONTRACTOR Safety Department. The purposes of the audits are :
• Ensuring compliance with CONTRACTOR and record keeping requirements.
• Reviewing and Update training activities.
• Reviewing claim management and loss prevention control efforts.

• Discerning any potential trends in Safety Violations or injuries that may apply to the overall
project area.
• Ensuring compliance with Government and COMPANY Safety Regulations.
5.11. Accident/ Incident Investigations
All incident resulting in injury, loss, or “near misses” are investigated and documented by the
Supervisor in charge and reviewed by the General Site Manager and the Site Safety/ Security
Superintendent. These investigations.
6. Security Rules
Security Rules should be established at the Site to ensuring Security System around the project
area.
Security scope includes the following items:
• Parking
• Entrance gate
• Walks and road ways
• Indentification
• Material passes
• Vehicles & equipment Stickers
• Searches person and/ or property.
• Cameras
• Liquor, drugs, fire arms and explosives
• Traffic regulations
• Security Control
The Security rules will strictly comply with COMPANY Regulations

  • 1. PT. KARINDA DAYA PERKASA SAFETY, HEALTH, ENVIRONMENT AND SECURITY PLAN Doc. No. : KDP-SHEP-BCD-003 Sheet No. : 2 of 10 Revisi : 0 1. General This Safety, Health and Environment (SHE) plan is established based on Engineering and Construction Industry practises and will be discussed and developed with COMPANY prior to starting construction phases as may required. The SHE Management System will ensure the work process from commencement through work completion are performed safely, healthy and environmentally. Contractor will base the site SHE programs on the following 5 pillar : a) Identify/ recognize • Site location & condition (live plant site, hill site, etc) • Character of the operation on working tools, vehicles, all kind of equipment’s (operating manual, routine maintenance, run test of equipment). • Worker/ labor health condition (mental/ spirit, boring, health, fitness) • Background of people involved in construction, with respect to safety awareness • Over working hours (following Standard Working Hours, limited overtime of working) • Loss of obidience of work/ safety regulation. b) Culturization of safety (safety minded) by having: • Safety messages for any kind of Site Construction Meetings • Induction, Tool Box, and Management Meetings. • Management Policy shown by brochures, banners, notification boards, bulletin, circular, site management memo. • Fire fighting and emergency evacuation training. • Safety Training • Safety Promotion and Awards c) SHE Supervision d) Refer to safe work procedures & rules e) Safety inspection and audit in all activities

Corporate Safety Policy CONTRACTOR Corporate Safety Policy which is established by CONTRACTOR Management, will be implemented on all CONTRACTOR Projects. The policy reflects CONTRACTOR full commitment to Health, Safety & Environmental Management. All CONTRACTOR employees shall be responsible for ensuring that Health, Safety and Environment policy is properly implemented in their works. 3. Project Safety, Health and Environment CONTRACTOR will organize a safety committee which is consist Project Manager, Site Manager, SHE/ Security Manager, Construction Manager and others. COORDINATION 4.1 Internal Coordination The role of Committe Secretary is to provide guideline to the management on the process of work, planning, implementing, monitoring in sustaining a timely and cost effective safety system. The safety Committe goal is that CONTRACTOR shall achieve a level of consistenly safe performance that assures CONTRACTOR recognition by COMPANY, Supplier’s/ Subcontractor’s and CONTRACTOR employees, and other stakeholders. Membership of the safety committe includes representatives from COMPANY, CONTRACTOR and all of Subcontractor’s Representatives that chaired by CONTRACTOR General Site Manager. This Committe should conduct safety committe meeting at least once a month, or if any accident happen. The effectiveness of the agenda meeting, the committe members shall carry out general safety patrol one day prior to the meeting schedule. 4.2 External Condition An external coordination shall be established with the Government such as police department, local hospital, local government etc. 4. IMPLEMENTATION OF PROGRAMS AT SITE The competent person should identify the potential problem or should asses the potential risk prior the start of work. There should be written instruction of work for the high-risk task of work. Self-protection equipment should be provided and maintened properly during the execution of work. All signboard should be placed where the potential risk will occur and should be easily known by everyone in the job site.

No. : KDP-SHEP-BCD-003 Sheet No. : 5 of 10 Revisi No. : 0 The Site Safety Programs takes into consideration the following elements as detailed in the paragraphs herein after : 1. Basic SHE Rules 2. SHE Supervision 3. First Aid/ Medical Procedures 4. Personnel Training 5. Work Permitting 6. Fire Prevention/ Protection 7. Emergency Response 8. Housekeeping 9. Environmental Hazard 10. Hazard Communication 11. SHE Inspection and Audits 12. Incident Investigations 13. Security Rules 5.1 Basic SHE Rules Based on the construction hazards analysis, basic safety, health and environment requirements are developed and issued to ensure the safety on all operations planned. These basic requirements which are listed below are documented in specific detail in the site safety practices: 1. Prevention of Work Accident/ Injuries in : • Electrical Work • Work at height • Grinding machines • Welding and Oxygene Cutting • Cartridge hammers • Hazardous materials and products • Dangerous machinery • Excavation or earth moving and civil construction work

SHE Supervision The job site teams from Foreman level up to Top Site Management shall continuously review site condition, planned construction work, and other site activities to observe safety hazards and unsafe acts by all personnel, and perform corrective mesures to eliminate any potential accident. 5.3. First Aid Procedures Prevention is the key element used to minimize the severity of occupational injuries and illness. However, unpredictable events do occur, and it is important that an emergency medical facility shall be defined so that prompt and compotent medical treatmen is provided to an injured or ill employee. CONTRACTOR will provide First Aids Kits during the start of and during site works. Appropriate protocols and reporting procedures are developed and pre arranged agreements are established with the COMPANY Medical Facility and/ or with the nearest emergency medical resources (doctors, hospitals, ambulances)

Personnel Training This section outlines the types of information and training required for all employees and supervisors prior to the start of and during site work, especially to develop the local workers. CONTRACTOR shall provide copies of training records or completion certificates for each employee, documenting date of completion for each training session. 5.4.1. New Hire Orientation/ Safety Induction The project program for employees and supervisors includes all workers rgardless of number of years worked in construction. The orientation-induction program will include, but not limited to : • Personnel Identification • Basic SHE Rules • PPE • Emergency preparadness • COMPANY SHE – Security Regulations. 5.4.2. Supervisor Orientation All Supervisors assigned to the project are indoctrinated to their responsibilities. They are required to learn and are requested to enforce all CONTRACTOR/ COMPANY rules applicable to their work. They are expected to set an example for their sub-ordinates and co-workers by their compliance with work rules and their aggressive leadership in safety. Subject covered during indoctrination sessions includes ( in addition to induction materials as above in article 5.4.1 herein before ) : • Supervisors Safety Responsibilities • “ STOP “ training and enforcement of safety rules • Safety-motivation • Safe practices for spesific crafts • Accident Investigation • Conducting effective safety meetings • Employee right-to-know law hazardous substances • Cient requirements • Effect of accidents on productivity

Supervisor Safety Training Safety Training which is considered to be an integral part of CONTRACTOR safety program and part of the productivity improvement program, is impelemented on the project. All supervisors on the project attend and complete the safe task assignment and enforcement training (STOP) program as well as other required technical safety instruction courses. 5.5. Work Permitting In order to monitor and control potentially hazardous work on the Job Site, work permits are required to perform work of any nature within the limits of any processing unit or in new construction where special hazards may occur. Work permit from company shall be as per COMPANY regulation. Minimum conditions that must be complied with include: • Locating all buried utility/ lines before excavation as applicable. • Providing fire protection and observing no smoking rules as applicable. • Providing personal protective equipment. 5.6. Fire Prevention/ Protection Plans In order to prevent fire from occuring, emphasis is placed on pre planning; hot-work permit controls; flammable gas, liquid and material control; control of smoking, training and use of warning signs; proper electrical wiring; and proper waste storage and removal. Specific procedures are outlined in the construction site safety and health plan for the project. 5.7. Emergency Response Emergency response procedures are developed for all potential incidents including fire, explosion, weather disturbances, etc. This procedure contain details on cumminication, fire fighting, medical, security, evacuation, resumption of operations and other details as may be deemed required.
  • 8. The employees are periodically reinforced through meetings (e.q, tool box meetings), subsequent training and drills, and procedures that are posted. PT. Housekeeping Good housekeeping is an important part of the site safety program. It is the responsibility of all employees to practice good housekeeping before, during, and after all activities in order to maintain a clean, orderly job site. Work sites, shop areas, and all laydown areas are kept clear of scrap and unnecessary material. Materials necessary for work are properly stacked and kept out of the roadways to allow proper access for fire fighting equipment and ambulance. Adequate toilet facilities, cleaning and disposal services, and sources of portable water are supplied as required. 5.9. Environment Protection Supervisor shall utilize appropriate procedures and protective equipment control measures to reduce and eliminate all identified sources of significant environment hazards to supervisors. Workers are trained in these procedures as required. 5.10. SHE Inspection and Audits The main emphasis of CONTRACTOR Safety Program is the prevention of accidents and injury. The safety procedure provide guidance on how this goals can assess each project progress and if needed, improve progress toward these goals, SHE Inspections of the entire project. The result of these inspections, as with the monthly inspections are recorded on the loss prevention inspection report. Copies of the inspections report and summary sheet are forwarded to the appropriate manager for corrective action as necessary. Safety Audits The audits conducted by CONTRACTOR Safety Department. The purposes of the audits are : • Ensuring compliance with CONTRACTOR and record keeping requirements. • Reviewing and Update training activities.
  • 9. • Reviewing claim management and loss prevention control efforts. PT. KARINDA DAYA PERKASA SAFETY, HEALTH, ENVIRONMENT AND SECURITY PLAN Doc. No. : KDP-SHEP-BCD-003 Sheet No. : 10 of 10 Revisi No. : 0 • Discerning any potential trends in Safety Violations or injuries that may apply to the overall project area. • Ensuring compliance with Government and COMPANY Safety Regulations. 5.11. Accident/ Incident Investigations All incident resulting in injury, loss, or “near misses” are investigated and documented by the Supervisor in charge and reviewed by the General Site Manager and the Site Safety/ Security Superintendent. These investigations. 6. Security Rules Security Rules should be established at the Site to ensuring Security System around the project area. Security scope includes the following items: • Parking • Entrance gate • Walks and road ways • Indentification • Material passes • Vehicles & equipment Stickers • Searches person and/ or property. • Cameras • Liquor, drugs, fire arms and explosives • Traffic regulations • Security Control The Security rules will strictly comply with COMPANY Regulations
