An adaptable, conscientious and dedicated Approved Mental Health Professional/Senior Social Worker
with over 15 years’ significant and progressive experience, carrying out Mental Health Act
Assessments, developing care plans and performing risk assessments for vulnerable adults in the
mental health sector. Possessing an in-dept understanding of relevant legislation, procedures and
techniques combined with extensive experience at a senior level, working in partnership with service

PROFESSIONAL EXPERIENCE:
March 2015 to Present: North East London Foundation Trust (NELFT)
Role: AMHP Social Worker
Currently working on a pilot project called Mental Health Direct (MHD). Mental Health Direct is the first
point of contact of communication with service users, their relatives, healthcare professionals and other
emergency services (i.e. EDT, Ambulance, Police etc.) during out of hours. The service operate
between the hours of 17.00hours to 08.00 hours Monday to Friday and 24 hours service on weekends
and bank holidays covering four Boroughs that are under the Umbrella of NELFT.
Mental Health Direct main objective is reducing the number of vulnerable service users that are regular A
& E attendance particularly service users that suffers with Mood, Anxiety and Personality Disorder.
MHD is a telephone service that is aimed at providing, basic advice, assessment, health information,
support and reassurance to service users calling the service. MHD also supports service users to
access the appropriate level of care including advice to manage their symptoms at home.
.
Core Responsibility:
Provide advice, defuses Crisis, provide support, reassurance as well as support with medical
intervention when needed, and sign-post appropriately.
* To support service users/patients manage their own condition at home, where possible.
* To recognize and work within scope of competence seeking support where appropriate.
* To liaise effectively between caller and third party in critical situations/areas of concern e.g. child and
adult abuse, where information is not consistent or may be disputed.
• To maintain a working knowledge of emergency procedures and escalate issues affecting service
delivery as necessary.
* Part of my role also incorporated people contacting us with varied issues such as, counselling,
medication administration and side-effects; which I sign-post to the relevant team or service. I follow it up
with emails and phone calls to ensure that the service users receive the appropriate or relevant service.
* I endeavor to follow referrals made by me up by contacting the services which the service user as
signposted to. The service users are also encouraged to access the services for follow-up.

My last assignment was with London Borough of Wandsworth. I worked as Principle Social Worker,
AMHP. I started with Wandsworth in March 10th 2014. My responsibility /duty included being the

Team’s Safeguarding lead person (SAM), supervising social workers in the Team, undertaking the social
care needs assessment/Personalisation and imputing information on Framework (i) system in
preparation for Panel, carrying small complex cases, taking part in weekly AMHP duty, Other duties
includes, supporting Team manager, prioritising Referral & Allocations and meeting agreed targets, risk
assessment and risk management, delegation, consultation and providing advice to staff with-in the
Multi-disciplinary team and other duties delegated by the Team Manager.

NORTH (CRT), COMMUNITY RECOVERY TEAM
DEPUTY MANAGER
Duties/ Responsibilities:
My job involved the managing of the health and social care staff, ensuring the duty Rota is appropriately
covered at all time. I oversee the day to day running of the team.
My responsibilities includes supervisory role, undertaking the supervision of health and social care staff.
Set and maintaining clear standards of performance and transformation and actively monitor progress
through statistical data analysis, i.e. up to date caseload activity targets, supportive to staff and line
manager and comply with Trust capability Policy. I have good understanding of -MCA 2005 and SGA
issues -Safeguarding issues. I chair and coordinated Adult Safeguarding meetings
Other duties include, prioritising Referral & Allocations and meeting agreed targets, risk assessment and
risk management, delegation, consultation and providing advice to staff with-in the Multi-disciplinary
team.
Assessment and establishing Care Plans and Packages as well signing off role. I also carry small
caseloads of complex client with multiplex needs /diagnosis.
Representing the team in multi-agency meetings i.e. Mappa, Marac and forensic interface meetings. As
an experienced AMHP, I advise and support to both Health & social care staff in legal matter relating to
Mental Health Assessment. Finally, I provide urgent responses when required.
May 2011-April 19 2013 LONDON BOROUGH OF HAMMERSMITH AND FULHAM
Approved Mental Health Professional – Adults (ASSESSMENT SERVICE)
I am currently working as an AMHP Social Worker with London Borough of Hammersmith and Fulham. I
work within the Assessment Team, the role involves, ongoing assessment and reviewing of service
users with enduring mental health problems. The Assessment Service comprises of two main sections,
the Duty i.e. Referral and Assessment and the Walk-in Clinic which is more of a self-referral, rapid
response for those who has no appointment or referral and feels the need urgent access to
support/services. Then there is the AMHP, which is done on a Rota basis. Within the Assessment
Team, I work as a link person for Older Adult client. Apart from the above mentioned processes, we also
carry cases on a short team bases.
We recently under gone organisational restructuring. However, before moving to current team, i.e
Assessment Team, I was working as a senior Social Worker within the older people's service at St
Vincent CMHT/Cognitive Impairment and Dementia Team also in, Hammersmith & Fulham. I therefore
have experience of working in an integrated team with health. I was managing a case load of 17, with
highly complex issues including Safeguarding. Every case I have worked on has provided an opportunity
to improve upon my knowledge and skills as well as developing my professional confidence
I have experience in the management of meetings in terms of coordinating, planning and setting
objectives for the meeting, e.g. Reviews, Case Conferences, Best Interest meetings and Multi-
disciplinary / Multi-Agency meetings for service users and their carers. I have also supported my
manager by attending workshops and meetings and feeding back to the team. I have extensive
experience of presenting cases to the Multi Agency Panel and have developed my communication skills.
I have knowledge of the relevant legislative framework and guidance documents that underpin work
within the Adults and Older People Services. I apply the legislative principles in my duties as well as our

policies and procedures. I am familiar with the new Choice and Control pathway in service delivery and
have attended workshops as part of the implementation.
I have managed many and varied Safeguarding of Vulnerable Adults cases and has experience of
attending strategic/planning meeting, investigating and writing comprehensive reports according to the
Hammersmith and Fulham Multi-Agency Safeguarding policy and procedures. I undertake assessments
under the Mental Capacity Act 2005 and liaise with other agencies, carers and families as appropriate. I
have continued to enhance my listening, observation and interviewing skills through the investigation of
many forms of abuse and am fully aware of the importance of risk management.
I am a qualified Practice Assessor with experience of providing work place learning. I plan the induction
and arrange weekly supervision. I ensure that the student is allocated appropriate cases and given
opportunities to learn. This role requires completing the various reports for the University and Training
Team. I have therefore developed supervisory skills which I am able to transfer to members of staff as
appropriate. I am in the position to guide and support students with interest in social work with Mental
Health and Adult and Older People and to promote work place learning on behalf of my organisation.
This experience has also enhanced my autonomous working.
I perform my tasks efficiently through effective planning, organisational and time management skills and I
am able to meet targets and manage risk.
I have always used supervision, colleagues and personal coping skills to manage any stress so that it
does not have negative effect on my health. I have a good sickness record.

PROFESSIONAL EXPERIENCE: March 2015 to Present: North East London Foundation Trust (NELFT) Role: AMHP Social Worker Currently working on a pilot project called Mental Health Direct (MHD). Mental Health Direct is the first point of contact of communication with service users, their relatives, healthcare professionals and other emergency services (i.e. EDT, Ambulance, Police etc.) during out of hours. The service operate between the hours of 17.00hours to 08.00 hours Monday to Friday and 24 hours service on weekends and bank holidays covering four Boroughs that are under the Umbrella of NELFT. Mental Health Direct main objective is reducing the number of vulnerable service users that are regular A & E attendance particularly service users that suffers with Mood, Anxiety and Personality Disorder. MHD is a telephone service that is aimed at providing, basic advice, assessment, health information, support and reassurance to service users calling the service. MHD also supports service users to access the appropriate level of care including advice to manage their symptoms at home. . Core Responsibility: Provide advice, defuses Crisis, provide support, reassurance as well as support with medical intervention when needed, and sign-post appropriately. * To support service users/patients manage their own condition at home, where possible. * To recognize and work within scope of competence seeking support where appropriate. * To liaise effectively between caller and third party in critical situations/areas of concern e.g. child and adult abuse, where information is not consistent or may be disputed. • To maintain a working knowledge of emergency procedures and escalate issues affecting service delivery as necessary. * Part of my role also incorporated people contacting us with varied issues such as, counselling, medication administration and side-effects; which I sign-post to the relevant team or service. I follow it up with emails and phone calls to ensure that the service users receive the appropriate or relevant service. * I endeavor to follow referrals made by me up by contacting the services which the service user as signposted to. The service users are also encouraged to access the services for follow-up. NORTH (CRT), COMMUNITY RECOVERY TEAM DEPUTY MANAGER Duties/ Responsibilities: My job involved the managing of the health and social care staff, ensuring the duty Rota is appropriately covered at all time. I oversee the day to day running of the team. My responsibilities includes supervisory role, undertaking the supervision of health and social care staff. Set and maintaining clear standards of performance and transformation and actively monitor progress through statistical data analysis, i.e. up to date caseload activity targets, supportive to staff and line manager and comply with Trust capability Policy. May 2011-April 19 2013 LONDON BOROUGH OF HAMMERSMITH AND FULHAM Approved Mental Health Professional – Adults (ASSESSMENT SERVICE) I am currently working as an AMHP Social Worker with London Borough of Hammersmith and Fulham. I work within the Assessment Team, the role involves, ongoing assessment and reviewing of service users with enduring mental health problems. The Assessment Service comprises of two main sections, the Duty i.e. Referral and Assessment and the Walk-in Clinic which is more of a self-referral, rapid response for those who has no appointment or referral and feels the need urgent access to support/services. Then there is the AMHP, which is done on a Rota basis. Within the Assessment Team, I work as a link person for Older Adult client. Apart from the above mentioned processes, we also carry cases on a short team bases. We recently under gone organisational restructuring. However, before moving to current team, i.e Assessment Team, I was working as a senior Social Worker within the older people's service at St Vincent CMHT/Cognitive Impairment and Dementia Team also in, Hammersmith & Fulham. I therefore have experience of working in an integrated team with health. I was managing a case load of 17, with highly complex issues including Safeguarding. Every case I have worked on has provided an opportunity to improve upon my knowledge and skills as well as developing my professional confidence I have experience in the management of meetings in terms of coordinating, planning and setting objectives for the meeting, e.g. Reviews, Case Conferences, Best Interest meetings and Multi- disciplinary / Multi-Agency meetings for service users and their carers. I have also supported my manager by attending workshops and meetings and feeding back to the team. I have extensive experience of presenting cases to the Multi Agency Panel and have developed my communication skills. I have managed many and varied Safeguarding of Vulnerable Adults cases and has experience of attending strategic/planning meeting, investigating and writing comprehensive reports according to the Hammersmith and Fulham Multi-Agency Safeguarding policy and procedures. I undertake assessments under the Mental Capacity Act 2005 and liaise with other agencies, carers and families as appropriate.
