Experience with complex cases-
During my time in the under 12 team I was allocated to a case of three
young children presenting with complex needs whom various
professionals had concerns over. I worked with the family and built a
relationship no other had succeeded in through the families disguised
compliance. Through assessment I was able to enable a trusting
relationship where by disclosures were made by the children. I uncovered
generational sexual abuse that was kept within the family and missed by
other professionals throughout time. I was able to assist with the
achieving best evidence interview and provide written evidence for court.
The evidence I secured led to successful removal of the children and by
far a better future outcome.
I have also worked alongside a mother with learning disabilities who
fabricated induced illness in her young child. This took place from when
the child was four months old until she was three years of age. This
involved several trust hospitals and a level of induced illness that the child
had been heavily medicated even though there were in fact no medical
tests to determine any medical condition. I attended court for the fact
finding trial and worked the case from start to finish. Both of these cases
were made subject to serious case reviews due to case management
prior to my involvement. I was involved in the review and discussions with
the reviewing officer which was extremely beneficial to my learning and
development.

Care and support services
My role included providing practical and emotional support to
individuals within the community who were living in the hostels as
part of agreed support plans. This included individuals with;
• Drug and alcohol dependency.
• Mental health conditions.
• Learning disabilities.
• Individuals released from prison.
• Individuals with learning disabilities.
• Those suffering domestic violence.
• Individuals at high risk of offending behaviour.
My role was to support individuals to understand and sustain their
licences. This included assistance to move on successfully from the
schemes.
I provided support to individuals through a process of assessment,
support planning and review. I liaised with the local Council staff to
ensure the appropriate sharing of information and smooth running of
the referral and move on systems. I undertook initial sign-ups with
new customers.
I ensured that customers adhered to the requirements of their licence
or tenancy, including rent payment, and the scheme rules. I then
promoted and supported the integration of customers into the local
community. I then provided opportunities by which customers were
enabled to develop independent living skills.
My role included supporting individuals to access professional or
specialist services where necessary and liaise with those providers to
encourage their engagement with the scheme and individuals.
This role required accurately inputting data onto a client database in a
timely manner and ensuring records were maintained to evidence

and internal and external key performance indicators.

The Coach House physical disability home (ages 18-65 years) and
Galimore Lodge nursing home.
My role was to carry out needs and risk assessments, and complete
support plans. To provide support, and carry out reviews.
I held a caseload of clients, and provided support to them on a key
work basis. This included providing practical and emotional support to
customers to achieve the outcomes set out in their support plans.
I supported customers to maximise their income, by accessing
benefits, and supporting to budget to cover domestic bills and living
expenses. I supported customers to make full use of community
facilities, and play an active role in their community and take up
cultural, recreational, educational and employment opportunities.
This included support to maximise their physical and mental health,
through liaison with health services and the promotion of a healthy
lifestyle. In addition, I provided support to maintain positive and
effective relationships with their family, friends, carers and other
professionals.
Implementation of effective safeguarding practice for vulnerable adults
and children in accordance with the local authority.
