Her history reveals rheumatic heart disease since age 12, a balloon mitral valvuloplasty 13 years ago, and no other significant medical history. On examination, she has an opening snap and mid-diastolic murmur at the apex. Echocardiogram shows mitral stenosis with a valve area of 2.0 cm2 and mild pulmonary hypertension. Outreach, Enrollment, Retention and Utilization
CCHI is a statewide association that advocates for and supports community organizations that help families access affordable health coverage. The CHI movement began in 2001 to provide low-cost health insurance to children not eligible for public programs. CHIs are local non-profit partnerships that focus on outreach, enrollment, retention, and utilization of health services in their counties. Their goals are to ensure all residents have comprehensive coverage through partnerships, enrollment events, and assisting individuals in accessing medical, dental and vision care.
AMATgreen
This document summarizes a multi-disciplinary process model used to increase organ donation rates among African American families. It involved a two-part strategy of community outreach and clinical response. For community outreach, the organ procurement organization engaged over 100,000 people at community events to raise awareness and establish trust. The intern met with their field supervisor and preceptor to discuss expectations and progress during the first week. No other major activities were reported during the initial onboarding period.

The Riley Hill Faith Partners for Healthy Communities is a collaborative formed between Riley Hill Baptist Church and Riley Hill Family Life Center to promote physical activity and healthy eating in their community. Some of their initiatives include installing a walking trail and playground at the church, adopting a healthy food policy, and hosting community events. They have reached about 400 people and formed 9 partnerships. Due to their success, they created a larger multi-church collaborative called Riley Hill Faith Partners for Healthy Communities to have an even greater impact.
