INTRODUCTION:
Any removable dental prosthesis that covers and
rests on one or more remaining natural teeth , the
roots of natural teeth , and / or dental implants ; a
dental prosthesis that covers and is partially
supported by natural teeth , natural tooth roots , and
/ or dental implants (GPT – 8)
INDICATIONS:
• Severe morphologic compromise of denture supporting areas that
significantly undermine denture retention
• Poor oral muscular function
• Low tolerance of mucosal tissues
• Parafunctional habits leading to recurrent soreness and inability of
prosthesis
• Unrealistic prosthodontic expectations
• Active or hyperactive gag reflexes
CONTRAINDICATIONS :
• Recent myocardial infarction
• Valvular prosthesis
• Severe renal disorder
• Treatment – resistant diabetes
• Generalised secondary osteoporosis
• Chronic and severe alcoholism
• Radiotherapy in progress
• Severe hormone deficiency
• Drug addiction
• Heavy smoking habits
ADAVNTAGES :
• Minimum anterior bone loss ; prevents bone loss
• Improved esthetics
• Improved stability
• Improved occlusion
• Decrease in soft tissue abrasions
• Improved chewing efficiency and force
• Improved retention , support and speech
• Reduced prosthesis size
• Improved maxillofacial prosthesis
DISADVANTAGES :
• Does not satisfy the physiologic need of these patients
• It requires proper plaque control and dental hygiene
• It is more costly compared to complete dentures
• They are bulkier than many other restorations
• More load to the prosthesis
• The lack of sufficient interarch space makes an overdenture system more
difficult to fabricate and more prone to component fatigue and fracture
CASE REPORT :
• A 67-year-old male patient reported to the department of prosthodontics with the
chief complaint of ill fitting mandibular denture.
• He had no medical history
• Dental history revealed extraction of periodontally involved teeth and their
replacement with maxillary and mandibular immediate dentures which were
relined on severe occasions
• On clinical examination mandibular ridge exhibited a moderate degree of alveolar
ridge resorption in posterior region.
• Evaluation of existing denture revealed inadequate denture extensions , poor
retention and stability
• An OPG was adviced
• A treatment plan was prepared after a standard protocol which
included a conventional denture in maxillary arch and a 4 – free
standing implant supported overdenture in the mandibular arch
TREATMENT PROCEDURES :
Maxillary and mandibular dentures were
fabricated in conventional manner. Bilateral
balanced occlusal scheme was selected.
Deflecting contacts in both centric and dynamic
parafunction were eliminated.
The mandibular denture was duplicated in a 2-part
top and bottom poly vinyl siloxane
(ReprosilTMDentsply) using clear auto-
polymerized acrylic resin (DPI self cured Acrylic
resin, Clear).This was used as a surgical template.
Surgical template for implant placement
implants of 11.5 mm length and 3.75 mm diameter was
selected. It was decided to use ball and socket type of
attachment system.
Implant surgery was carried out in a 2-stage surgical protocol. Surgery
was performed under local anesthesia.The osteotomy sites were
prepared in the A, B, D and E region with the help of surgical template.
A guide pin was used to ensure that the second implant was as parallel
as possible to the first.
The selected implants were placed at the prepared sites. Surgical cover screws
were placed.The flaps were approximated with primary closure .The patient
was told not to wear the lower denture for two weeks following surgery.

The sutures were removed in two weeks. Dietary restrictions were lifted after
2 weeks of implant placement.
The patient was seen on a regular follow-up visits and the denture was
relined as needed.Three months later and after confirmation of the
osseointegration, the patient was presented for the second stage surgery.
The intaglio surface of the denture was relieved. Soft tissue conditioning material
(GC Reline SoftTM) was applied to the intaglio surface of the denture according to
the manufacturer’s directions and the excess liner material was trimmed.The
denture was finished, polished and inserted into the patient’s mouth
At this stage, the implants were exposed, the surgical cover screws were
removed and the sites were irrigated with sterile normal saline. Healing
collars were placed, and the gingival tissues were allowed to mature for one
month. Mandibular denture was relined with a soft-tissue conditioning
material .
After one month , the comfort and fit of the denture was checked before
addition of attachments. Ball and socket over-denture abutment of 2 mm
diameter was selected. Seating of the abutments was verified
Acrylic resin from the intaglio surface of the denture was removed to allow passive
fit of the denture against the tissue. Pressure indicating paste (Mizzy Prestige
Dental Products) was used to verify that no contact of the denture base with
abutment or attachment.
A pick up space was vented lingual to the denture teeth and was filled with
CG pattern resin and mandibular denture was placed over the abutments
The complete seating of the denture was verified and
the patient was asked to maintain light occlusal pressure
in the centric relation position while the resin
polymerizes.
The patient was trained to place and remove the prosthesis properly. First recall
was attended after 24 hours.The regular follow up was advised every six
months. Patient was instructed to remove their prosthesis at night. A soft single-
tufted brush was indicated to keep attachments free from plaque and calculus.
EXTRA ORALVIEW – POSTTREATMENT
DISCUSSION
The implant-supported overdenture remains in place during mandibular movements which
allows the tongue and perioral musculature to resume a more normal function since they
are not required to control mandibular denture movements.
As with any treatment modality, aftercare and maintenance is vital if the overdenture is to
be successful.The patient must be advised of this and reviewed regularly. Optimal surgical
implant positioning is essential for the success of implant supported restorations.
An implant-retained overdenture requires meticulous treatment planning than a
conventional complete denture. Final placement of the implants should follow the
principles of ideal implant parallelism and maximum initial stabilization, and path of
placement and removal.
CONCLUSION
•Restoration of the edentulous mandible is a challenge. Among different
treatment options, an implant-retained overdenture is a simple, cost effective
solution in the rehabilitation of the edentulous mandible. clinicians and dental
technicians have to adhere to sound design principles such as simplicity in
fabrication, ease of maintenance and repair and cost control.

Implant Supported Overdentures

INTRODUCTION: Any removable dental prosthesis that covers and rests on one or more remaining natural teeth , the roots of natural teeth , and / or dental implants ; a dental prosthesis that covers and is partially supported by natural teeth , natural tooth roots , and / or dental implants (GPT – 8)
  • 4. INDICATIONS: • Severe morphologic compromise of denture supporting areas that significantly undermine denture retention • Poor oral muscular function • Low tolerance of mucosal tissues • Parafunctional habits leading to recurrent soreness and inability of prosthesis • Unrealistic prosthodontic expectations • Active or hyperactive gag reflexes
  • 5. CONTRAINDICATIONS : • Recent myocardial infarction • Valvular prosthesis • Severe renal disorder • Treatment – resistant diabetes • Generalised secondary osteoporosis • Chronic and severe alcoholism • Radiotherapy in progress • Severe hormone deficiency • Drug addiction • Heavy smoking habits

ADAVNTAGES : • Minimum anterior bone loss ; prevents bone loss • Improved esthetics • Improved stability • Improved occlusion • Decrease in soft tissue abrasions • Improved chewing efficiency and force • Improved retention , support and speech • Reduced prosthesis size • Improved maxillofacial prosthesis

DISADVANTAGES : • Does not satisfy the physiologic need of these patients • It requires proper plaque control and dental hygiene • It is more costly compared to complete dentures • They are bulkier than many other restorations • More load to the prosthesis • The lack of sufficient interarch space makes an overdenture system more difficult to fabricate and more prone to component fatigue and fracture
  • 8. CASE REPORT : • A 67-year-old male patient reported to the department of prosthodontics with the chief complaint of ill fitting mandibular denture. • He had no medical history • Dental history revealed extraction of periodontally involved teeth and their replacement with maxillary and mandibular immediate dentures which were relined on severe occasions • On clinical examination mandibular ridge exhibited a moderate degree of alveolar ridge resorption in posterior region. • Evaluation of existing denture revealed inadequate denture extensions , poor retention and stability
  • 9. • An OPG was adviced • A treatment plan was prepared after a standard protocol which included a conventional denture in maxillary arch and a 4 – free standing implant supported overdenture in the mandibular arch
  • 10. TREATMENT PROCEDURES : Maxillary and mandibular dentures were fabricated in conventional manner. Bilateral balanced occlusal scheme was selected. Deflecting contacts in both centric and dynamic parafunction were eliminated. The mandibular denture was duplicated in a 2-part top and bottom poly vinyl siloxane (ReprosilTMDentsply) using clear auto- polymerized acrylic resin (DPI self cured Acrylic resin, Clear).This was used as a surgical template.
  • 11. Surgical template for implant placement implants of 11.5 mm length and 3.75 mm diameter was selected. It was decided to use ball and socket type of attachment system. Implant surgery was carried out in a 2-stage surgical protocol. Surgery was performed under local anesthesia.The osteotomy sites were prepared in the A, B, D and E region with the help of surgical template. A guide pin was used to ensure that the second implant was as parallel as possible to the first.
  • 12. The selected implants were placed at the prepared sites. Surgical cover screws were placed.The flaps were approximated with primary closure .The patient was told not to wear the lower denture for two weeks following surgery. OPG after implant placement Intra oral view The sutures were removed in two weeks. Dietary restrictions were lifted after 2 weeks of implant placement.
  • 13. The patient was seen on a regular follow-up visits and the denture was relined as needed.Three months later and after confirmation of the osseointegration, the patient was presented for the second stage surgery. The intaglio surface of the denture was relieved. Soft tissue conditioning material (GC Reline SoftTM) was applied to the intaglio surface of the denture according to the manufacturer’s directions and the excess liner material was trimmed.The denture was finished, polished and inserted into the patient’s mouth
  • 14. At this stage, the implants were exposed, the surgical cover screws were removed and the sites were irrigated with sterile normal saline. Healing collars were placed, and the gingival tissues were allowed to mature for one month. Mandibular denture was relined with a soft-tissue conditioning material . After one month , the comfort and fit of the denture was checked before addition of attachments. Ball and socket over-denture abutment of 2 mm diameter was selected. Seating of the abutments was verified
  • 15. Acrylic resin from the intaglio surface of the denture was removed to allow passive fit of the denture against the tissue. Pressure indicating paste (Mizzy Prestige Dental Products) was used to verify that no contact of the denture base with abutment or attachment. A pick up space was vented lingual to the denture teeth and was filled with CG pattern resin and mandibular denture was placed over the abutments
  • 16. The complete seating of the denture was verified and the patient was asked to maintain light occlusal pressure in the centric relation position while the resin polymerizes. The patient was trained to place and remove the prosthesis properly. First recall was attended after 24 hours.The regular follow up was advised every six months. Patient was instructed to remove their prosthesis at night. A soft single- tufted brush was indicated to keep attachments free from plaque and calculus.
  • 17. EXTRA ORALVIEW – POSTTREATMENT
  • 18. DISCUSSION The implant-supported overdenture remains in place during mandibular movements which allows the tongue and perioral musculature to resume a more normal function since they are not required to control mandibular denture movements. As with any treatment modality, aftercare and maintenance is vital if the overdenture is to be successful.The patient must be advised of this and reviewed regularly. Optimal surgical implant positioning is essential for the success of implant supported restorations. An implant-retained overdenture requires meticulous treatment planning than a conventional complete denture. Final placement of the implants should follow the principles of ideal implant parallelism and maximum initial stabilization, and path of placement and removal.
  • 19. CONCLUSION •Restoration of the edentulous mandible is a challenge. Among different treatment options, an implant-retained overdenture is a simple, cost effective solution in the rehabilitation of the edentulous mandible. clinicians and dental technicians have to adhere to sound design principles such as simplicity in fabrication, ease of maintenance and repair and cost control.
