A 71-year-old woman presented to the emergency room with confusion and coffee ground vomitus. Imaging revealed multiple gas pockets within her bladder wall, and she was diagnosed with emphysematous cystitis. Urine culture grew E. coli and she was treated intravenously with antibiotics. Her symptoms gradually improved and a follow up ultrasound showed a normal bladder wall. Emphysematous cystitis is a severe urinary tract infection characterized by gas accumulation in the bladder wall due to bacterial or fungal fermentation. It requires aggressive treatment with antibiotics and bladder drainage.
