of integrity before the academic school year. The code of integrity is an agreement between the
student and the university to follow rules and promote academic honesty to make Grand View a better
place to be. The university then defines academic dishonesty as a misrepresentation of personal efforts
and honest work. This includes plagiarism, cheating, fabrication and facilitation of dishonesty (65 66).
For the first occurrence the student is under the discretion of the professor. A second offense means

returning to the real world. It s an expression of love, hence the word ode, to the nightingale and her

of the waves are symbolic of the transition between imagination and reality. Keats desire to escape is

so they can return to their homes and communities. Over the past decade, inpatient admissions for
pediatric patients have significantly increased by 68% due to the exhibition of self injurious behaviors
and increased suicidal ideation (YoungMinds, 2015). The focus of this paper is to introduce the quality
improvement proposal of implementing a safety plan for staff to adhere to in order to promote
ultimate patient safety on an inpatient child and adolescent psychiatric unit. The following information
will also reflect the practice and challenges of the psychiatric team at St. Elizabeth Hospital in order to
achieve ultimate patient safety.
St. Elizabeth Hospital, owned by Ascension Health, is one of the few facilities in Wisconsin which
