Bilateral vestibulopathy is a chronic vestibular syndrome characterized by unsteadiness when walking or standing that worsens in darkness or on uneven surfaces. It is caused by a bilateral impairment of the vestibular system and affects balance and causes symptoms like oscillopsia. Diagnosis involves demonstrating reduced or absent vestibular function in both ears. While the cause is often unknown, management involves avoiding ototoxins, vestibular suppressants, and vestibular rehabilitation therapy. It typically causes some permanent impairment.
