Impression materials
Abutments:
• They are components of the implant system that screw directly into
the implant.
• They eventually support the prosthesis in screw retained restoration.
• In cement retained prosthesis abutments may be shaped as
conventional crown
Abutment classification:
• Abutments for fixed prosthesis
Definitive
• Prefabricated- solid and hollow
• Custom made- castable and CAD-CAM abutments
Provisional
• Abutments for removable prosthesis
Stud attachments

Magnetic attachments
Telescopic attachments
Provisional restorations:
• Immediate temporary abutment : It consists of
two parts : an abutment and a plastic coping.
• It is indicated for single tooth implant restoration
made chair-side. The temporary abutments or
copings are used for the fabrication of provisional
screw-retained restorations made of titanium or
plastic.
• Non engaging type are used for multi unit
implant restorations and engaging type are used
in single tooth implant restorations.
Permanent restorations:
• Snappy abutment : It is indicated for single
tooth or multiunit restorations. Ideal for
posteriors.
• Esthetic abutment : Indicated similar to
snappy abutment. This is designed with a
scalloped margin that profiles natural soft
tissue contours with various collar heights
available based on implant system.
• Multi-unit abutment : It is indicated for
multiple teeth when desire for greater ease of
retrievability is required. It is screw retained.
• Single tooth abutment : It is indicated for single
tooth restoration with limited interdental space.
• Easy abutment : Used for both single tooth or
multiple unit types. Ideal for posteriors.
• Gold adapt engaging : It is screw retained for single
tooth. Cement retained for single tooth and
multiunit restorations.
Overdenture abutments:
• Gold abutment bar : It is indicated for fully
edentulous arch with extensive bone loss. When
phonetics is concerned and also when soft tissue
support is necessitated.
• Ball abutment : It is indicated in fully edentulous
arch with extensive bone or soft tissue loss.
When compromised patient manual dexterity
and phoenetics is concerned.
• When implant is placed too far posterior with a
bar. It allows for misangulation of upto 300
between implants.
• Implant Replica : It is the replica of the
abutment or implant attached to the
impression coping after taking an
impression to help make an accurate
model.
• Try in abutments : These are used to help
select proper size and shape of abutment
for the patient. Made of plastic, these
abutments can be reused after having
cleaned and sterilized using normal
procedure
Implant super structures:
• This prosthesis is fabricated with the support of dental implants.
• They can be of following types:
• Fixed prosthesis: crowns, bridges
• Removable prosthesis: overdenture
• Fixed detachable prosthesis: hybrid dentures

predictable implant placement procedure.

placement needed to ensure a good clinical

Armamentarium in implantology

Editor's Notes

  1. The foremost requirement for an implant surgery is a complete dental chair unit with all its working components. A high suction apparatus is needed to aspirate blood, saliva and other secretions which might hamper the field of vision. An X-ray viewer is needed, which creates an illuminated background for radiographs to enable easier viewing and more accurate diagnosis.

  3. Diagnostic casts: It allows the operator to evaluate patients intra oral condition and plan treatment. Mounted casts help in assessing the spatial relationship and tooth morphology Surgical guide: : Surgical Guide or template is used to assist in proper surgical placement and angulation of dental implants by directing the implant drilling system.
  4. Plastic probes are used to probe around implants which prevent gogging of implant surface. They also enhance visibility because of the colour contrast.
  5. Dividers – These are available in 4 ½ inch radius and 6 inch radius. Crown Gauges are used for measuring wall thickness crowns. They are provided with markings from 0-10 cm.
  6. Boleys Gauge is used to measure the diameter of the crown or tooth. Microboleys Gauge is a variation of Boleys Gauge which has markings from 0-95 mm. It is also used to measure tooth dimension and interdental space.
  7. Calipers are used to find the depth, width and space between the teeth for preparation of implant site. Different types of calipers include : Ridge mapping caliper and Castroveijo caliper (0-40 mm markings).
  8. It is an instrument used for atraumatic extraction during immediate implant procedures.
  9. Artery forceps: They are used to ligate blood vessels so that bleeding is controlled during the procedure. The Carmalt forceps are the largest and heaviest forceps and the Halsted forceps are the smallest forceps. They are available in straight and curved designs.   Tissue holding forceps: These are used to stabilize or hold the soft tissue. Adsons forceps is the most commonly used. They can be toothed or non toothed. The non toothed forceps has serrations which provide proper grip while handling the tissues. They are also used to carry membranes.
  10. They are used to hold the suture needle during. Mayo-Heger needle holder and Castroveijo needle holder are commonly used.

  13. Implant systems are available from different manufacturers and each system possesses its own specifications for use. Each implant system has an implant console, a motor-hand piece and foot control.
  14. The front panel of the console shows control panel or display, a pump-arm, motor connection socket and a stand holder. The rear panel has connecting socket for the attachment of foot control. P1, P2 and P3 display the speed in rpm. P4 and P5 display the torque in Ncm. The desired program is activated by pressing the corresponding button. Changing the speed (P1-P3) The normal speed range varies from 300-40000 rpm. Change in the speed can be achieved by pressing and holding the plus or minus buttons. Changing the torque (P4-P5) P4 : Torque ranges from 5-70 Ncm P5 : Torque range from 20-60 Ncm These buttons increase or decrease the torque in 5 Ncm steps or 10 Ncm steps. Changing the flow rate : To change the flow, press the P2 button for 4 seconds. Continue this with + button to increase the flow and – button to decrease the flow. Foot control: coolant flow, program, forward/ reverse , speed
  15. Initial marking
  16. It is the first rotary instrument to be used. It cuts the bone at tip as well as the sides. It is also used to mark the osteotomy site. : Implant drills must be used sequentially with increasing diameter to allow precise gradual widening of the osteotomy. These drills mostly cut at tip. Depth marks over the drills correspond to implant length.
  17. (Direction indicator) : This is used after creating first narrow osteotomy. It shows the direction of osteotomy. It often features a hole to tie a floss to prevent aspiration. It is sometimes referred to as ‘Dummy Implant’ to show where the implant would be.
  18. Counter sink when the implants are placed into the bone, they may need an additional step in bone preparation with a crestal counter sink drill to increase the osteotomy only at that region
  19. They are used to deliver implant to osteotomy site. They are of two types : One for use in implant hand piece Other for use in implant wrench Different sizes of drivers are available for different implant platforms.
  20. They form critical piece of equipment that are used to screw or unscrew various components on to the fixture head. Depending on the type of implant system, they can be slotted, hexagonal or star shaped.
  21. Bone tap is used when the implant engages cortical bone. It used is torque of 30 nm
  22. Used to achieve desired torque, Step 1 : Assemble the manual torque wrench by inserting the screw driver. Step 2 : Adjust the direction indicator; such that arrow is facing towards level arm. Then screw is rotated clock wise. Step 3 : Adjust the direction indicator such that arrow is facing away from the level arm. Then the screw is rotated counter clockwise.
  23. They are used to wedge donor block grafts from chin and ramus. Curettes are for harvesting bone
  24. They are used for crushing or grinding larger bone fragments into smaller particles for grafting purposes. They are of two types : Top held and hand held
  25. short-handled instruments used mainly to drive a chisel or wedge.
  26. Sinus curettes and elevators : These instruments are used for lifting the sinus cavity. Elevators feature a more rounded and smoother tip for lifting the sinus.
  27. Osteotomes are used to expand bone in ridges with reduced thickness and also indirect sinus lift procedure. They are of two types as shown below. Concave osteotomes : They are used to raise the floor of maxillary sinus. They also allow for introduction of graft material. They are available in straight and curved patterns. Convex osteotomes : They are used to break the floor of sinus. They are available with or without stops. Stops are meant for maintaining proper distance in millimeters.
  28. also called as gingival former and permucosal extensions. They extend above the soft tissue into the oral cavity and form a gingival cuff around the implant. Usually placed for 2-5 weeks depending on the healing, following which are removed and impression procedures are commenced

Abutment is fitted to the analog and prosthesis is fabricated in the laboratory That component of dental implant system used to provide a spatial relationship of an endosteal dental implant to the alveolar ridge and adjacent dentition or other structures. The coping is attached to the implant fixture during implant procedures using an impression screw. Following impression making the coping is removed from the implant fixture and attached to the implant analogue, to pour a cast. It is also called as impression post, impression pin or transfer coping. It facilitates transfer of intraoral location of the implant or abutment to a similar position on laboratory cast. Types: 1. Transfer 2. Pickup. It provides a spatial relationship of an endosteal dental implant to the alveolar ridge and adjacent dentition or other structures.
  30. The walls of abutment are usually smooth, polished and straight sided Length may vary from 1-10 mm The choice of abutment size depend on the vertical distance between the fixture base and opposing dentition, the existing sulcular depth and the esthetic requirements in the area being stored.
  31. Application of Lasers in Clinical Practice Use of lasers can be divided among four phases: • Pre-surgical • Surgical • Post-surgical • Prosthetic. Flap –co2 lateral window sinus lift: er yag flaps: all three osteotomy er yag second stage: diode laser and er yag also act as low level laser therapy and enhaces biostimulation prosthetic: recontouring for final fit
  32. Tooth brushes soft, rounded bristles to be used as the implant surfaces are easily damaged. Gauze strips or super floss for cleaning inter-proximal areas. Irrigators can also be used as adjunctive aids.
  33. These are the instruments used to clean and maintain implants. The tip design of these curettes prevent scratches or damage to the implant surface while removing build up.
  34. These are designed to help organiz and protect instruments from damage during sterilization, transportation and storage. They are available in single or double tray configurations. Silicone raised mat provides efficient ventilation to facilitate drying.
