Lockout / Tagout
Definition – Process of achieving an electrically safe work
condition
Six step process:
1) Determine all possible sources of electrical supply
2) Open disconnecting devices
3) Visually verify all devices are fully open
4) Apply lockout/tagout devices
5) Test to verify circuits are in fact denergized
6) Ground phase conductors or circuit parts if there is
the possibility of induced voltages or stored
electrical energy

Principles of Execution
1) Employee Involvement – Each person who could be
exposed is involved in the process
2) Training – All persons who could be exposed shall be
trained
3) Plan – Shall be developed and use up-to-date
drawings
4) Control of Energy – Sources shall be controlled
5) Identification – Devices shall be unique and
identifiable
6) Voltage – Shall be removed and absence verified
7) Coordination – Coordinated with all

Simple – That involve only a qualified person(s)
deenergizing one set of conductors or circuit part
source for the sole purpose of performing work within
the limited approach boundaries on electrical
equipment shall be considered simple.

lockout/tagout

• Lockout defined as installing a lockout device, prohibiting
operation requiring forcible removal
• Tagout defined as installing tagout device, prohibiting
operation installed in the same position as a lockout
• If not possible to attach a lock to the disconnecting means, it
shall not be used as the only means to isolate
• Use of tagout without lock permitted only in cases where
equipment design does not use a lock
• One additional safety measure is required

Removal of Lockout/Tagout Devices
• Proceedure identifies details for removing locks or tags
when the installing individual is unavailable
• When locks or tags are removed by someone other
than the installer, the employer shall attempt to locate
that person prior to removing the lock or tag
• When the lock or tag is removed because the installer
is unavailable, the installer shall be informed prior to
returning to work

Release for Return to Service
• Identifies steps to take when job requiring LO or TO is done
• Before reenergization, tests and visual inspections are done
• Confirms removal of jumpers, shorts and grounds
• Ensures circuits are ready to be safely energized
• Notification of regular employees who operate equipment taken
out of service to participate in reenergization
• Inspection of the area to ensure nonessential items have been
removed such as keeping personnel clear from exposure to
dangerous conditions, blocked and/or grounded equipment is
prepared for return to service

Temporary Release for Testing/Positioning

 The basic safety rule governing safe clearance and
lockout/tagout procedures is that all conductors
and equipment are considered energized until:
o All sources of electrical energy have been
disconnected or otherwise prevented from
energizing the equipment or circuits being worked
on.
o A qualified person has operated the equipment
operating controls or otherwise verified that the
equipment cannot be restarted.
o A qualified person used test equipment to test the
circuit elements and electrical parts of equipment
to which employees will be exposed and verified
that the circuit elements and equipment parts are
de-energized.

 Establish safe clearance and lockout/tagout
procedures at each base or facility.
 If a device is listed on the Safe Clearance and is
capable of being locked out, then apply a lock. Use
of only tagout and not lockout must be justified on
the Safe Clearance.
 Apply Danger tags to prohibit changing the position
of devices by unauthorized persons. All energy-
isolating devices must be provided with a Danger
tag, even those locked out. Use a Danger tagout for
each Safe Clearance. Use Caution tags in connection
with a Safe Clearance to provide precautions
necessary before operation of a switch or other
device.

 The details and the person preparing the
Safe Clearance must include:
o Details of blocking, switching, tags, and locks.
Enter details in their proper sequence, reading
down the form. Include any switch operations
(such as opening or shutting) necessary to
transfer load or put other equipment into
operation.
o Supplemental direction, if necessary, to be
provided to the crew involved in the work to
ensure their understanding of boundaries of
coverage of the Safe Clearance.

• Only designated persons must be authorized to issue (approve)
Safe Clearances for work by qualified personnel. These persons
must be designated in writing in accordance with local procedures.
• The designated person in issuing (approving) a Safe Clearance
must ensure that the following objectives have been met:
• Inclusion of the correct switching and equipment operations
sequence.
• Provisions are included to discharge and ground capacitors
and other sources of stored electrical energy that might
endanger personnel.

o Provisions are included to discharge or block the
release of stored non-electric energy (such as
springs) in any device that could cause electric
circuits to re-energize.
o Selection of a qualified worker who is authorized
to receive the approved Safe Clearance and then
perform the required switching and operations.
o Arrangements have been made for any necessary
interruption of service. Notifications to the utility
company must be given to the person designated
by the utility company to receive such
information.

the hazards, the associated foreseeable tasks, and the
protective measures that are required in order to
maintain a tolerable level of risk, including the
following:
• Identifying and analyzing electrical hazards
• Identifying tasks to be performed
• Documenting hazards associated with each task
• Estimating the risk for each hazard/task pair
• Determining the appropriate protective measure
needed to adequately reduce the level of risk

UNCLASSIFIED UNCLASSIFIED  The basic safety rule governing safe clearance and lockout/tagout procedures is that all conductors and equipment are considered energized until: o All sources of electrical energy have been disconnected or otherwise prevented from energizing the equipment or circuits being worked on. o A qualified person has operated the equipment operating controls or otherwise verified that the equipment cannot be restarted. o A qualified person used test equipment to test the circuit elements and electrical parts of equipment to which employees will be exposed and verified that the circuit elements and equipment parts are de-energized.
  • 21. UNCLASSIFIED UNCLASSIFIED  Establish safe clearance and lockout/tagout procedures at each base or facility.  If a device is listed on the Safe Clearance and is capable of being locked out, then apply a lock. Use of only tagout and not lockout must be justified on the Safe Clearance.  Apply Danger tags to prohibit changing the position of devices by unauthorized persons. All energy- isolating devices must be provided with a Danger tag, even those locked out. Use a Danger tagout for each Safe Clearance. Use Caution tags in connection with a Safe Clearance to provide precautions necessary before operation of a switch or other device.
  • 22. UNCLASSIFIED UNCLASSIFIED  The details and the person preparing the Safe Clearance must include: o Details of blocking, switching, tags, and locks. Enter details in their proper sequence, reading down the form. Include any switch operations (such as opening or shutting) necessary to transfer load or put other equipment into operation. o Supplemental direction, if necessary, to be provided to the crew involved in the work to ensure their understanding of boundaries of coverage of the Safe Clearance.
  • 23. UNCLASSIFIED UNCLASSIFIED • Only designated persons must be authorized to issue (approve) Safe Clearances for work by qualified personnel. These persons must be designated in writing in accordance with local procedures. • The designated person in issuing (approving) a Safe Clearance must ensure that the following objectives have been met: • Inclusion of the correct switching and equipment operations sequence. • Provisions are included to discharge and ground capacitors and other sources of stored electrical energy that might endanger personnel.

UNCLASSIFIED UNCLASSIFIED o Provisions are included to discharge or block the release of stored non-electric energy (such as springs) in any device that could cause electric circuits to re-energize. o Selection of a qualified worker who is authorized to receive the approved Safe Clearance and then perform the required switching and operations. o Arrangements have been made for any necessary interruption of service. Notifications to the utility company must be given to the person designated by the utility company to receive such information.

UNCLASSIFIED UNCLASSIFIED • Risk assessment includes a comprehensive review of the hazards, the associated foreseeable tasks, and the protective measures that are required in order to maintain a tolerable level of risk, including the following: • Identifying and analyzing electrical hazards • Identifying tasks to be performed • Documenting hazards associated with each task • Estimating the risk for each hazard/task pair • Determining the appropriate protective measure needed to adequately reduce the level of risk
