This document contains references to two appendices that provide assessment tools for measuring depression and mood. Appendix 14 refers to the Beck Depression Inventory, which is a commonly used psychological test to measure symptoms of depression. Appendix 15 refers to a Mood/Depression Assessment Questionnaire, which is likely another assessment tool used to evaluate a person's mood and symptoms of depression.
