A Research Proposal For Nurses On A Trauma Unit After...
Qualitative Research Proposal: Workarounds
Nadine Davis
Stevenson University School of Graduate and Professional Studies
NURS 530
Abstract
Workarounds is a safety issue which poses a problem in nursing practice. The problem seems to arise when nurses are over worked and they need to
cut corners to get the job done but patient safety then becomes an issue. The purpose of this assignment is to develop an evidenced based research
proposal using the qualitative research method. The chosen topic for this research proposal is workarounds amongst nurses on a trauma unit after
implementation of new electronic method of charting and administration of medication. The selected group included 10 nurses who have been working
on a trauma care unit. Nurses may think that are doing a more efficient job by using workarounds especially to help with time management and
emergencies. However this may not be the case, nurses may be doing themselves an injustice for not embracing changes as it pertains to new
technology in healthcare in the twenty first century.
Introduction
Nurses may use workarounds with the best of intentions; they may need to get a task done in a minimal time frame. Getting a medication to a patient
quickly or providing more efficient care for multiple patients can make the difference between death and saving a life. For example; a patient having a
cardiac arrest, needs immediate medical attention and is a time sensitive matter. Getting critical

Health Care Delivery Model : Transitional Care Essay
the transition can be challenging for both the patient and family members, therefore, the need for PN (Chase–Ziolek, 2014). Although introduced
recently in the health care delivery model, transitional care has some elements of care provided by FCNs among faith communities (Breisch, et al.,
2013; Peterson, Atwood, & Yates, 2002; Ziebarth & Miller, 2010). For instance, the FCN can help in facilitating change related to increased knowledge
on disease management. Consequently, awareness can be extended to the community members and parishioners and enable them to control diseases.
Other than providing spiritual care to the community, FCN also provided counseling, active listening, referrals, education, advocacy, and prayer.
Therefore, FCNs can promote community awareness related to health issues, and develop community collaborations to enhance health promotion.
Breisch et al. (2013) established that advocacy for appropriate levels of care for susceptible populations and people with inadequate access to
healthcare is a role that can be played by FCNs. In addition, FCNs are responsible for the provision of awareness among church communities with
low–literacy skills to promote healthcare. Through the initiation of referrals for clinical treatment, providing assistance to access home care resources,
assisting with extended care appointments, and providing support to healthcare consumers, FCNs can promote improved quality care.
This DNP proposal will have a significant role in the

Evidence-Based Project
1.Demonstrate synthesis of the knowledge and skills acquired in preceding graduate nursing core and specialty curriculum content.
These last two courses of my master's program have worked well and hard to integrate all the previous core education, knowledge and skill acquired
during the entire program. The evidenced based project has been a wonderful tool to integrate all the nursing theory, research processes, as well as
demonstrating my ability to make a difference in quality patient care and to progress the nursing profession. For example using Lewi's Theory for my
change theory for implementation of my graduate project which I learned in a previous class (Butts & Rich, 2011), and spending time on each step
were an excellent tool to increase and cemented the education learned throughout and brought all the pieces together.
2.Evaluate current health policy and legal and ethical considerations in addressing the health promotion and disease prevention of a target population.
This course gave the opportunity to learn about healthy policy, general and specific guideline in the home health care, also gave me the possibility to

Audience is one of the most important pieces of communication and awareness that has the potential to positively or negatively effect change. The
target audience that this author will focus upon are physicians, registered nurses, interdisciplinary healthcare professionals and all ancillary staff.
Patient care is never delivered by one specific person or one group of persons. Each of these professionals directly interact with patients, and are
therefore a potential link within the chain of infection.
Evidenced Based Proposal: CAUTI Prevention
This author's evidenced based proposal topic was preventing Catheter Associated Urinary Tract Infection (CAUTI) on a neurological intensive care unit
within a large hospital. The hospital's CAUTI rate was 1.55%, which was well above the national benchmark of 1.00%. CAUTI is a healthcare
associated infection. Healthcare associated infections continue to arise, and outcomes associated with these infections can be devastating to the health
and well–being of the patient.

Capstone Project Reflection
The whole experience of working on my Capstone Project Proposal has been a challenge, but great learning opportunity. There are so many aspects
and factors that play major roles in research that I had no idea existed. Through this entire project I have been able to understand the hard work put
into nursing research and the importance of nursing research. According to Thorne (2016), "we need to repeatedly remind ourselves that the purpose
of nursing research is not simply to do it, but rather to use it in the service of our mission on behalf of society" (p. 763). The complex problems of health
, as well as, the health care system, are continually evolving in the world (). According to Throne (2016), it is up to our generation to utilize tools and

I remember reading the 12 steps of the research process and not understanding the application aspect. The 12 steps of the research process are the
following: identification of a problem, reading the related literature, research questions or hypothesis, theoretical framework or conceptual framework,
research design, sample, data collection, who, what, where, when and how questions, feasibility issues, analyzing data, determining finds, and
dissemination of findings (Keele, 2011). Identifying the problem was not hard, since it was something that interested me. My Capstone Proposal
Project will be about sleep disturbances associated with cardiac surgery patients. My proposed strategy consists of the idea that by increasing nursing
education and intervention there will be a reduction in sleep disturbances for the cardiac surgery population. I have found the small details to be more
challenging than preparing the main factors of the Capstone Project Proposal. For example, I thought the detailed Gantt chart was difficult because of

Capstone Approach To Study Nursing
When I first applied to the University of Mount Olive's Bachelors of Science in nursing, I had no idea what to expect. Although there are many
options for nurses seeking programs for Bachelors of Science of nursing, after doing my own research I knew the University of Mount Olive was
going to be the right choice for me. As soon as I reached out to University of Mount Olive, they made the process so easy and stress–free. There were
three major factors that influenced my decision, which were the following: faith–affiliated, completely online, and the acceptance of so much
educational credits I had already completed. The University of Mount Olive truly holds the goal of creating Bachelor level nurses. I believe the

Nursing Research Utilization Project Proposal
Nursing Research Utilization Project Proposal
In the recent years, nursing as a profession has faced several challenges and emerging issues that affect the health welfare of the society at large.
Despite the concerted efforts set to tackle the challenges, still the nursing industry proves to be difficult to fully embrace the changes outlined each
time. Example of challenges facing nursing includes nurses' etiquette while at work, commitment to their work, commitment to their profession as
nurses, meeting patients' expectations, shortage of nurses and commitment to patients.
Problem In the current century, shortage of nurses is the biggest challenge to health institutions in general, as a matter of fact it can be termed as a
global issue. It is approximated that in 2006, there was a shortage of 118,000 nurses and a projection of roughly one million shortage of nurses to be
experienced in 2020 (US Congress, 2007). This shortage can only be felt when there exists an imbalance between the available nursing skills and the
actual number of nurses utilizing their skills, this is experienced bearing that not all the nurses available actually take up nursing jobs. Trained nurses
often decline job offers due to issues like payment and benefits hence they opt for employment not related to nursing or even remain with no
employment at all. This shortage has a big impact on the health status of patients as well as the quality of heath care services that are provided. Nurses

Section D: Methods of Implementation Health care throughout the history has shown many changes. Every change brings in challenges in a workplace
because change is more often met with resistance. It also brings in innovative ideas that steers the organization's advancement and performance. The
major motivation of change in this organization is to sustain a successful future in its continuum. For change to succeed, brilliant strategy, and clever
implementation are necessary (Gandossy and Sonnenfield, 2004).

The staffs' participation is crucial because they are the ones who will embrace the change; therefore, teamwork is paramount to sustain the continuity of
change (Spector, 2010).
Use of Theoretical a Model The Lewin's theory of change is a fitted model for this intervention. Lewin depicted the three– stage process that will guide
the implementation successfully (Borkowski, 2005).
Unfreezing: The resisting forces perceive the idea of change; this phase is the preparation stage for the entire organization (Borkowski, 2005).
Change movement: this is the stage for training staff. Modification of policies for medication administration by nurse managers and the pharmacy
director will take place here (Borkowski, 2005).
Refreezing: The new goals are highlighted in this phase, and staff nurses start to embrace the change as they get comfortable with the new routine
(Borkowski, 2005).
Budget Proposal The budget was summarized in the appendix I section of this paper. The total financial cost to the department would be $6,840.00 for
the whole duration of the change implementation.
Practicality of Implementation Inclusion of evidence–based practice in nursing for this proposed solution fosters quality of care. The integration of
nursing knowledge with the best available evidence will improve patient outcomes and will save lives (Estabrooks, 1998). The implementation of the

The Impact Of Evidence Based Nursing Practice On Patient Care
Criteria and process used for evidence–based nursing practice, and the impact of evidence–based nursing practice on patient care.
The act of nursing practice requires the application of competent evidenced based practice (EBP) nursing care, supported by current research and theory
development. Through the critical analysis and evaluation of studies, nurse leaders evaluate the components of research for its effective use in the daily
practice of nursing. Moreover, this allows nurses to remain current with recommendations for patient care practices (Grove, Gray, & Burns, 2015). The
Alzheimer–Europe organization in 2009 compared the different types of research that impact patient care and are involved in the development of EBP.
The organization compared a top down and bottom up approach to differentiate the methods used for research. However, recognizing each method
impacts nursing practice and patient care. The use of the nursing research method of Outcomes research starts with a sample, then predict what might
occur. The exact data indicators, look at what occurred or the end results in programs, practice, or patient care strategies (Alzheimer–Europe.org, 2009).
This approach can inform thoughts about research and the process of forming conclusions or theories about the practice of nursing (Grove, Gray, &
Burns, 2015). While qualitative and quantitative research, begins with a defined population and then develops a research sample. This top down
approach for quantitative

Business Operations And Administration Services
My research of business operations and administration services in an inpatient skilled nursing facility expanded my knowledge of the responsibilities of
an affective administrator. Although there are various business operations within the healthcare facilities, environmental services is critical for the
aesthetics and maintenance of the facility. Therefore, I selected to research environmental services and how that department interfaces with
administration services. However, environmental services does not have direct patient care contact, but without this service there would be no patients.
Environmental services determines the aesthetics of the facility inside and outside. Consequently, environmental services has a lot of... Show more

Housekeeping is responsible for keeping the inside of the facility clean. Their duties consists of cleaning patient's rooms, common areas, and visitor's
bathrooms. The floor technicians are responsible for cleaning and sanitizing the different floor types in all common area, ancillary departments, and the
hallways. Floor care is very pertinent to the aesthetics of the facility. Therefore, all floors should be clean and free of offensive odors. Although floors
are maintained around the clock, usually the floors are stripped and waxed during the night when there is less traffic. Environmental services and
housekeeping is a business operation that has close scrutiny from administration. Henceforth, the lack of aesthetics inside and outside of the facility
will not attract all types of families and patients. Although, environmental services does not provide direct patient care, there would be no patients
without quality service from this business operation. In my research I mentioned that environmental services and housekeeping operates as one entity.
Consequently, in my SLP module I chose to interview the manger and a housekeeping aide. I wanted to get insight from the personnel delegating, and
the actual person performing the duties. Mr. Goralski is the environmental services manager, and services both environmental services and
housekeeping. Mr. Goralski has been employed with the inpatient skilled nursing facility for over twenty

Nursing Research Utilization Project Proposal
Nursing Research Utilization Project Proposal Abstract The issue of controlling and preventing hospital–acquired infections is a major problem in the
Healthcare system. Most patients admitted to hospitals are at some risk of contracting a hospital–acquired infection (Paterson, 2012). Some patients are
more vulnerable than others; these include the elderly, patients with defective immune systems, and premature babies. Hospital–acquired infections
remain a major concern, and they can occur in any care setting, including acute care within hospitals, outpatient surgery centers, clinics, and long–term
care facilities (such as nursing homes or rehab centers). Four categories account for 75% of all acquired infections in the acute care... Show more

In some cases, patients die from the infections. The Centers for Disease Control and Prevention estimates that approximately two million people
acquire infections while in the hospital each year, and approximately 90,000 of these patients die as a result of their infections (Limaye, Mastrangelo,
& Zerr, 2008). All of these negative reports suggest it is very important and necessary for healthcare providers to focus on an evidence– based
confirmed clinical practice solution to help eliminate hospital–acquired infections. 3.Project objective that is specific, realistic, and measurable The
outcome objective for this project is for nurses from the cardiac unit of Greenville Hospital System who attend the infection control educational
classes to score a 95% or higher on the post test after each training session in order to evaluate employee knowledge on the topic. Any employee who
scores less than 95% on the posttest will need to repeat the training session. The infection class will begin first week of August for the cardiac staff
nurses and the last two weeks of July for selected charge nurses who will help facilitate during the implementation of the educational program. 4.Brief
solution description and rationale The way to reduce and eliminate infections in the hospital is to develop the proposed evidence–based solution of
implementing an educational program with different training sessions to

Although several national and professional organizations have developed evidence–based guidelines that set forth strategies for reducing falls,10,11
consistency in implementation of these strategies has been limited. Findings from 188 medical–surgical units in 48 hospitals across the United States
found that risk–speciп¬Ѓc interventions (such as ambulation and medication management) are not being implemented consistently.12 This п¬Ѓnding is
in line with our research where we have identified that elements of nursing care are being regularly missed in acute care hospitals. We have
conducted 3 studies of missed nursing care, the п¬Ѓrst being a qualitative focus group study on 5 patient units,13 the second a quantitative study in 3
hospitals,14 and the third, an expansion of the 3–hospital study in 10 hospitals with diverse characteristics (eg, size, teaching status).15 The results of
these studies showed that a substantial amount of standard required nursing care is being left undone and that the patterns of missed care are similar
across hospitals. Ambulation of patients 3 times per day (or as ordered) was the most frequently reported element of missed nursing care with 76% of
nurses reporting this action being frequently or always missed. Similarly, Callen and colleagues16 found that 73% of patients did not walk during their

Abstract
Purpose: The goal of this study is to explore the relationship between insulin administration in non–diabetic patients and their average length of stay in

Only research will tell, since "nursing research is systematic inquiry designed to develop knowledge about issues of importance to nurses, including
nursing practice, nursing education, and nursing administration" (Polit & Beck, 2006, p. 4).
Problem Statement "Hyperglycemia (typically defined as a blood glucose of >200 mg/dL) is a well–known phenomenon in critically ill patients
without a prior history of diabetes mellitus" (Langdon & Shriver, 2004, p. 163). While many studies have been completed with respect to management
of hyperglycemia in diabetic surgical patients, there has been minimal research regarding the impact that tight glycemic control has on non–diabetic
trauma and medical patients. With managed care requiring decreases in length of hospitalization and hospitals attempting to conserve cost expenditure,
placing non–diabetic patients on insulin drips immediately upon entry into the health care system may adversely affect both of these areas. Trauma
patients without a diagnosis of diabetes mellitus will require prolonged lengths of stay in the intensive care unit related to the fact that they must have
closely monitored blood glucose levels. In addition to the increased duration of their intensive care unit admission, they are subjected to administration
of medications they would not normally take at home. Furthermore, this patient population has

Nurse Driven, Sleep Hygiene Protocol, A Source Of Anxiety...
The inability to sleep is a source of anxiety and stress for patients in the intensive care unit (ICU); this inability can lead to cardiorespiratory
disturbances, immune system dysfunction, impaired wound healing, hormonal and metabolic imbalances, cognitive changes, and delirium. Through
the use of quiet times and nursing actions that encourage sleep, or, sleep hygiene bundles, nurses can assist in improving sleep quality in the ICU.
These bundles include noise and light reduction, scheduling routine procedures outside of reserved sleep times, the use of earplugs and eye masks, and
the grouping of nursing care activities to minimize sleep interruptions. The aim of this proposal, therefore, will be to implement a nurse–driven, sleep
hygiene protocol, in order to improve patient care by promoting quality sleep in ICU patients. Barriers to implementation include staff resistance to the
concept, concerns regarding patient care during sleep times and difficulties in coordinating care with other units. Studies used to gather information for
this proposal were published between the years of 2011 and 2016 and were found in several databases: Academic Search Complete, Medline, and
OVID Journals.

Introduction
The health and well–being of society is ultimately impacted by the integrity of scientific records, such as the value, accuracy, and completeness
(Horner & Minifie, 2011). As a result, researchers are obligated and entrusted to use the highest standards possible when proposing, reviewing,
performing, or reporting research (Horner & Minifie, 2011). Consequently, there may be ethical concerns for a variety of different research topics and

Qualitative Critical Appraisal, Attitudes Knowledge and...
I have chosen to critique Rydon's (2005) article 'The attitudes, knowledge and skills needed in mental health nurses: The perspective of users of
mental health services' because I am interested in finding out service user's outlook of mental health nurses. When I qualify as a mental health nurse I
would need to acquire the correct attitudes, knowledge and skills as this would make the nursing process more effective, because as McCabe and
Timmons (2006) suggest I will be more sensitive to the client's needs and preferences. Therefore being able to transfer findings to my own practice is
highly substantial. The Author of the article, Sharon E. Rydon, has undertaken this research as a partial fulfilment of a Master's degree, training to be...

In consequence of that, differences may have influenced the outcomes of the results and bias may have formed (Parahoo, 2006). This type of bias is
called selection biases. Some research bias is inevitable but the researcher should have shown their understanding of this and taken this into account
in the data analysis. Therefore it also clearly indicates the researcher has failed to maintain consistency in the article and because of the errors the
validity of the article has also been threatened. Further clarification is needed to understand what she actually did (Whittaker & Willamson, 2011,
Parahoo, 2006, Litwin, 1995). It is imperative for a study to be valid, for the results to be accurately applied and interpreted. From this article it is
difficult to say whether the study reflects the truth. Insufficient information on how the participants were selected; how participants got involved in the
study; what questions were asked; whether or not the researcher's body language and style of questioning influenced the results; and how trust was
gained with participants leaves the reader questioning the validity of the article (Litwin, 1995, Merriam, 2009). The researcher "drew on the

Middle Range Theory Of Postpartum Depression Theory Essay
Middle Range Theory Paper: Analysis of Postpartum Depression Theory
Description of Theory– In 1993, Beck published a middle range theory on postpartum depression, entitled Teetering on the Edge. Beck, (1993, p. 44)
which describes "walking on a fine line between sanity and insanity". Beck determined a limited amount of qualitative research available and few
instruments for measurement of postpartum depression, postpartum psychosis, and "maternity blues". Beck interviewed 12 women for 18 months
detailed their experiences, and determined the main perceived problem was a loss of control (Beck, 1993). Beck's theory is one that is centered in
phenomenology and produced a concise and testable theory (Marsh, 2013).
Loss of control is broken down into four stages: encountering terror, dying of self, struggling to survive, and regaining control (Beck, 1993). Beck,
2002, has developed two screening tools (Postpartum Depression Screening Scale and Postpartum Depression Inventory System) to evaluate risk
factors and symptoms. Encountering terror is an initial response to feeling out of control, described as being trapped with no means of escape
(Beck, 1993). Stage II is highlighted by a reported feeling of loss of former self and feeling internally void of caring or emotion, likened to that of a
robot (Beck,1993). Struggling to survive is recognized by the inability to carry on activities of daily living, including eating and bathing. This is the
stage where many seek assistance and

While, balancing work, family life, and education can be extremely challenging, the ability to obtain an advanced degree and practice at a level that
allows one to utilize evidence and research is a reward that can only be achieved through education. The Masters of Nursing (MSN) program at
American Sentinel University (ASU) has been a journey that is worth embarking. Further, an education that is explicitly curtailed to a speciality
program, can enhance one's ability to be an expert in the area of practice. The MSN speciality track of the infection prevention and control (IPC) at

It is evident, one out of every 25 patients in an acute care setting acquires a HAI the United States alone (Bubb, et al., 2016). Further, HAIs result in a
significantly higher morbidity, and mortality, lengths of stay, and cost ofhealth care (McNeill, L. 2017); not to mention the negative health outcomes on
families, and communities at large (bubb, et al., 2016). In addition, globalization is another factor that has perpetuated the transmission of infectious
diseases as the world population has become more mobile.
All the reasons discussed above and many more demand for a well–trained and versatile IP. It is apparent, healthcare organizations ought to consider
infection prevention as a priority for patient safety efforts, as the burden of HAIs increase and to respond to the globalization phenomenon (Bubb, et
al., 2016). Hence, an advance education with IPC speciality, would allow nurses to pursue career advancement possibilities and also significantly
improve the quality and safety of patient care through research and EBP.

Personal Reflection
According to Polit and Beck (2017), "the ultimate goal of research is to develop and expand knowledge" (p. 3). Through my education in the
Doctorate of Nurse Practitioner program, I plan to expand on the knowledge I already have and develop new knowledge that will help me become a
successful student and nurse practitioner. I was first introduced to the concept of research during my undergraduate nursing program. During the
program, I took a class that taught the basic knowledge of research and how it is used in our daily practice. It was in this class that I learned about
evidence–based practice studies and how to find articles that were based on nursing research. I have no professional experience being involved in a
research project or completing research on my own. I have been a part of implementing evidence based practice (EBP) interventions at work. Some
of the interventions included implementing the ambulatory blood pressure monitoring system, new practices for lab draws and ways to decrease
infections in central lines. The goals I will make for this class will address Bloom's Revised Taxonomy of Learning. According to Clark (2015), "the
cognitive domain involves knowledge and the development of intellectual skills." Using this domain, my goal is to be able to analyze and evaluate
research effectively, and apply what I learn to help expand my education and daily practice. I will accomplish this goal by, distinguishing between facts

The Five Fundamental Patterns Of Knowing In Nursing
Patterns of knowing The five fundamental patterns of knowing are empirical, aesthetical, personal, ethical and sociopolitical. The patterns or ways of
knowing help with the development and application of nursing knowledge. Empirical knowing embodies "factual and publicly verifiable descriptions,
empiricism and theoretical explanations or predictions" (Carper, 2012 p.25). In my Oncology setting the science of nursing is very relevant to practice.
I use science daily in calculating body surface area for chemotherapy drugs, chemotherapy protocols and febrile–neutropenia protocols with the
initiation of specific antibiotics are some examples. Empirical knowing provides facts from empirical research for nursing care and interventions.
Aesthetical knowing is known as the "art and act" of nursing, it highlights empathy. This pattern of knowing recognizes the nurse's perception of what
is significant in an individual patient's behaviour (Carper, 2012). I utilize the art of nursing by acknowledging the patient's feelings and being present
for them during a difficult time and the ability to establish a meaningful connection with the patient. Personal knowing refers to the "expressions of the
quality and authenticity of the interpersonal process between each nurse and each patient, it looks at the interpersonal relationships of nursing"
(Fawcett, Watson, Neuman, Walker, Fitzpatrick, 2001, p. 25). Personal knowing in Oncology setting is shown by forming therapeutic

Orientation Of The New Graduate Nurse
Orientation of the New Graduate Nurse: A Change Proposal A newly graduated registered nurse is well prepared to pass NCLEX for licensure;
however, being prepared for the acute care setting in a hospital is a completely different issue. Nursing schools develop the foundation of nursing
knowledge, creates critical thinking skills, and touches on nursing tasks. All of these are great tools to possess, but they do not prepare the new
graduate nurse for safe, acute patient care. At Boulder Community Health (BCH) on the orthopedic neurology unit, new graduate nurses are given
five weeks of orientation. Expecting a newly licensed nurse to learn all of a hospitals policies and procedures concerning patient care and how to
incorporate the policies into their practice after five weeks is highly risky and leads to unsafe patient care. On the orthopedic neurology unit at BCH
the new nurse trains with several nurses during the five week orientation period. This does not allow for the nurse to learn a consistent system to
follow or incorporate into their personal practice as each nurse has their own system. This can become frustrating for the novice nurse. This paper will
propose a change in the orientation program at BCH. A one year new graduate nurse residency (NGNR) and mentorship program will be presented to
the management administration at BCH. The proposal will serve to prove that a residency program will actually save the hospital money by retaining
quality nursing staff through

colleges and universities look for research grants to support their educational research mission (Education advisory Board, 2014). According to licklider

This process allows for the faculty members to focus on writing the science portion of the grant, while CNHP's Research team becomes the responsible
party for all of the administrative responsibilities. According to Cellier and Krenkel (2014), universities that provide faculty with research support, have
a chance of improving their future proposal writing outcomes (p8).
Purpose of Evaluation
The evaluation is being conducted to ensure that CNHP "hands free submission" process is actually meeting the needs and expectations of the research
active faculty. In addition to ensuring that the intended outcomes are being measured correctly and are successfully met. According to Preskill and
Russ–Eft (2009), "Evaluations are conducted to serve the interests and needs of several individual or groups of people" (p165). Therefore, the primary
purpose of the evaluation is to make certain that the active faculty are receiving the administrative assistance they need. As well as confirming that the
administrative staff are doing all they are capable of doing to provide the best outcomes possible.
In addition to, the research active faculty support, this process was also introduced to motivate and encourage the non–research active faculty to apply
for grants. See the College of Nursing and Health Professions is a unique college, as there are 8 distinctive different disciplines intertwined within

level nurse is creating an individualized plan of care. The assigned nurse prepares to educate the patient on worsening infection and hygiene promotion.
A written copy of signs and symptoms of infection are administered to the patient and family. Due to the area of the wound, the nurse educates a family
member on wound care and dressing changes. The nurse requires that the family member return a demonstration to prove understanding of the task.
The bachelor level nurse acknowledges the need for follow up care in the home setting. This nurse utilizes a discharge planner and recommends a home
health nurse to follow up on wound care. The nurse acknowledges that the patient was septic during his hospitalization and requires extra attention in
preventing further illness. An associate level nurse may not discharge a patient to the degree of a higher educated RN. They may not collaborate with
other health care workers and may not utilize the assistance available for the patient. As stated in Essential VII (2008), the baccalaureate prepared nurse

Roy's Adaptation Model Nursing has developed into a profession, in which many theories have been developed and presented in order to lend guidance
for purposeful and meaningful practice. In general, nursing theories are based on four concepts that collectively make up the nursing metaparadigm:

nursing diagnosis, goal setting, interventions, including a plan of care, and lastly, evaluation of the intervention and how it relates to the nursing goals

(RAM) guides nurses in developing effective care through the use of reasoning, critical thinking, and decision–making. With a focus on adaptation,
RAM can be utilized as a framework to conceptualize and plan individualized patient care or may be applied in a limited context to develop
interventions for distinct clinical populations (Senesac, 2013). Through the nursing process, patients are assisted in adapting, or coping, to changes
within their environments. The process begins with an assessment of the patient's adaptive behavior as well as stimuli in the external and internal
environments. Based on these evaluations, the nurse develops nursing diagnoses and interventions to guide goal setting and increase successful
adaptations or coping mechanisms (Chitty & Black, 2007). Simply stated, the nurse modifies the environment to promote and facilitate patient
adaptation and restore balance. Adaptation is observed through modifications in behavior. In the context of RAM, four modes of

Reducing Central Line Infections?
Reducing Central Line Infections Central lines are a one of the major risk factors for infections in the bloodstream and many occur outside of the
intensive care units (Medina, Serratt, Pelter, & Brancamp, 2014). An increase in the amount of patients who are admitted to the hospital with central
lines has increased the amounts of central line infections. Reducing the amount of central line infections on a general Medical–Surgical unit with in a
facility is very important for both patients and healthcare facilities.
Preparation
The issue is the increase in the number central line infections on the general Medical–Surgical floor. The method used to determine the issue on the
Medical–Surgical unit is personal and professional experience. The nursing staff on the Medical–Surgical unit has recently experienced an increase in
patients with central lines. This is clinically significant, because an increase in central lines increases the risk for increased central line infections.
Infections in the bloodstream are attributed to increased length of stay and increasing costs, including the reduction in reimbursement to the facilities
(Medina et al., 2014). Reducing central line infections will reduce hospital costs, reduce stress for the patients, and allow the nursing staff to provide
patient safe practice.

Social And Economic Themes In Jonathan Swift's A Modest...
In A Modest Proposal, by Jonathan Swift he discusses a solution to poverty in Ireland during the 18th century. Swift's bizarre, yet well thought out
plan for helping families of Ireland, who were in poverty and desperately in the need of money, was fattening children and selling them to the
wealthy landowners. He thought that this would be an excellent way for children not to be a burden on their parents but be beneficial. Swift even
went on to write a pamphlet telling the parents just how they could fatten their babies and the perfect age and weight for the kids to be before selling
them. One of Swift's friends goes on to give some helpful/ tasty ways that the kids can be cooked before eaten. Swift's proposal does not only financially

There only remain a hundred and twenty thousand children of poor parents who are annually born"(5). By Swift including these statistics shows that
he has well thought out this plan, and Swift has done plenty of research before coming to his conclusion. Swift later gives credibility to his friend
from London by saying, "a young healthy child well nursed, is, at a year old, a most delicious nourishing and wholesome food"(9). All three
rhetorical devices that he used were great ways to persuade the people of Ireland ,that were in poverty, that this was the best option to help them prosper.
Swift expresses upon the issue that merchants would rather the parents sell their children as babies because they are then more valuable. Swift then
adds some credible information proving that he has done his research, "I am assured by our merchants, that a boy or girl before twelve years old, is
no saleable commodity, and even when they come to this age they will not yield above three pounds." Swift uses his statics to prove that his solution
was well thought out by saying, "computed the charge of nursing a beggar's child to be about two per annum." This shows that swift had done his
research for finding out not only the population of the city, but he found out the pricing for nursing a child that was soon to be sold and eaten. Swift's

A Research Proposal For Nurses On A Trauma Unit After...

  • 1. A Research Proposal For Nurses On A Trauma Unit After... Qualitative Research Proposal: Workarounds Nadine Davis Stevenson University School of Graduate and Professional Studies NURS 530 Abstract Workarounds is a safety issue which poses a problem in nursing practice. The problem seems to arise when nurses are over worked and they need to cut corners to get the job done but patient safety then becomes an issue. The purpose of this assignment is to develop an evidenced based research proposal using the qualitative research method. The chosen topic for this research proposal is workarounds amongst nurses on a trauma unit after implementation of new electronic method of charting and administration of medication. The selected group included 10 nurses who have been working on a trauma care unit. Nurses may think that are doing a more efficient job by using workarounds especially to help with time management and emergencies. However this may not be the case, nurses may be doing themselves an injustice for not embracing changes as it pertains to new technology in healthcare in the twenty first century. Introduction Nurses may use workarounds with the best of intentions; they may need to get a task done in a minimal time frame. Getting a medication to a patient quickly or providing more efficient care for multiple patients can make the difference between death and saving a life. For example; a patient having a cardiac arrest, needs immediate medical attention and is a time sensitive matter. Getting critical ... Get more on HelpWriting.net ...
  • 2. Health Care Delivery Model : Transitional Care Essay the transition can be challenging for both the patient and family members, therefore, the need for PN (Chase–Ziolek, 2014). Although introduced recently in the health care delivery model, transitional care has some elements of care provided by FCNs among faith communities (Breisch, et al., 2013; Peterson, Atwood, & Yates, 2002; Ziebarth & Miller, 2010). For instance, the FCN can help in facilitating change related to increased knowledge on disease management. (2013) established that advocacy for appropriate levels of care for susceptible populations and people with inadequate access to healthcare is a role that can be played by FCNs. In addition, FCNs are responsible for the provision of awareness among church communities with low–literacy skills to promote healthcare. Through the initiation of referrals for clinical treatment, providing assistance to access home care resources, assisting with extended care appointments, and providing support to healthcare consumers, FCNs can promote improved quality care. These last two courses of my master's program have worked well and hard to integrate all the previous core education, knowledge and skill acquired during the entire program. The evidenced based project has been a wonderful tool to integrate all the nursing theory, research processes, as well as demonstrating my ability to make a difference in quality patient care and to progress the nursing profession. For example using Lewi's Theory for my change theory for implementation of my graduate project which I learned in a previous class (Butts & Rich, 2011), and spending time on each step were an excellent tool to increase and cemented the education learned throughout and brought all the pieces together. 2.Evaluate current health policy and legal and ethical considerations in addressing the health promotion and disease prevention of a target population. • 5. Evidence Based Proposal Audience is one of the most important pieces of communication and awareness that has the potential to positively or negatively effect change. The target audience that this author will focus upon are physicians, registered nurses, interdisciplinary healthcare professionals and all ancillary staff. Patient care is never delivered by one specific person or one group of persons. Each of these professionals directly interact with patients, and are therefore a potential link within the chain of infection. Evidenced Based Proposal: CAUTI Prevention This author's evidenced based proposal topic was preventing Catheter Associated Urinary Tract Infection (CAUTI) on a neurological intensive care unit within a large hospital. The hospital's CAUTI rate was 1.55%, which was well above the national benchmark of 1.00%. CAUTI is a healthcare associated infection. • 6. Capstone Project Reflection The whole experience of working on my Capstone Project Proposal has been a challenge, but great learning opportunity. There are so many aspects and factors that play major roles in research that I had no idea existed. Through this entire project I have been able to understand the hard work put into nursing research and the importance of nursing research. According to Thorne (2016), "we need to repeatedly remind ourselves that the purpose of nursing research is not simply to do it, but rather to use it in the service of our mission on behalf of society" (p. 763). The complex problems of health , as well as, the health care system, are continually evolving in the world (). I remember reading the 12 steps of the research process and not understanding the application aspect. Identifying the problem was not hard, since it was something that interested me. My Capstone Proposal Project will be about sleep disturbances associated with cardiac surgery patients. My proposed strategy consists of the idea that by increasing nursing education and intervention there will be a reduction in sleep disturbances for the cardiac surgery population. Although there are many options for nurses seeking programs for Bachelors of Science of nursing, after doing my own research I knew the University of Mount Olive was going to be the right choice for me. As soon as I reached out to University of Mount Olive, they made the process so easy and stress–free. • 8. Nursing Research Utilization Project Proposal Nursing Research Utilization Project Proposal In the recent years, nursing as a profession has faced several challenges and emerging issues that affect the health welfare of the society at large. Despite the concerted efforts set to tackle the challenges, still the nursing industry proves to be difficult to fully embrace the changes outlined each time. Example of challenges facing nursing includes nurses' etiquette while at work, commitment to their work, commitment to their profession as nurses, meeting patients' expectations, shortage of nurses and commitment to patients. Problem In the current century, shortage of nurses is the biggest challenge to health institutions in general, as a matter of fact it can be termed as a global issue. It is approximated that in 2006, there was a shortage of 118,000 nurses and a projection of roughly one million shortage of nurses to be experienced in 2020 (US Congress, 2007). This shortage can only be felt when there exists an imbalance between the available nursing skills and the actual number of nurses utilizing their skills, this is experienced bearing that not all the nurses available actually take up nursing jobs. Trained nurses often decline job offers due to issues like payment and benefits hence they opt for employment not related to nursing or even remain with no employment at all. This shortage has a big impact on the health status of patients as well as the quality of heath care services that are provided. Nurses have a ... Get more on HelpWriting.net ...

Lewin depicted the three– stage process that will guide the implementation successfully (Borkowski, 2005). Unfreezing: The resisting forces perceive the idea of change; this phase is the preparation stage for the entire organization (Borkowski, 2005). Change movement: this is the stage for training staff. Modification of policies for medication administration by nurse managers and the pharmacy director will take place here (Borkowski, 2005). Refreezing: The new goals are highlighted in this phase, and staff nurses start to embrace the change as they get comfortable with the new routine (Borkowski, 2005). Budget Proposal The budget was summarized in the appendix I section of this paper. The total financial cost to the department would be $6,840.00 for the whole duration of the change implementation. • 12. The Impact Of Evidence Based Nursing Practice On Patient Care Criteria and process used for evidence–based nursing practice, and the impact of evidence–based nursing practice on patient care. The act of nursing practice requires the application of competent evidenced based practice (EBP) nursing care, supported by current research and theory development. Through the critical analysis and evaluation of studies, nurse leaders evaluate the components of research for its effective use in the daily practice of nursing. Moreover, this allows nurses to remain current with recommendations for patient care practices (Grove, Gray, & Burns, 2015). The Alzheimer–Europe organization in 2009 compared the different types of research that impact patient care and are involved in the development of EBP. The organization compared a top down and bottom up approach to differentiate the methods used for research. However, recognizing each method impacts nursing practice and patient care. The use of the nursing research method of Outcomes research starts with a sample, then predict what might occur. The exact data indicators, look at what occurred or the end results in programs, practice, or patient care strategies (Alzheimer–Europe.org, 2009). This approach can inform thoughts about research and the process of forming conclusions or theories about the practice of nursing (Grove, Gray, & Burns, 2015). Nursing Research Utilization Project Proposal Nursing Research Utilization Project Proposal Abstract The issue of controlling and preventing hospital–acquired infections is a major problem in the Healthcare system. Most patients admitted to hospitals are at some risk of contracting a hospital–acquired infection (Paterson, 2012). Some patients are more vulnerable than others; these include the elderly, patients with defective immune systems, and premature babies. Hospital–acquired infections remain a major concern, and they can occur in any care setting, including acute care within hospitals, outpatient surgery centers, clinics, and long–term care facilities (such as nursing homes or rehab centers). Four categories account for 75% of all acquired infections in the acute care... Show more content on Helpwriting.net ... In some cases, patients die from the infections. All of these negative reports suggest it is very important and necessary for healthcare providers to focus on an evidence– based confirmed clinical practice solution to help eliminate hospital–acquired infections. 3.Project objective that is specific, realistic, and measurable The outcome objective for this project is for nurses from the cardiac unit of Greenville Hospital System who attend the infection control educational classes to score a 95% or higher on the post test after each training session in order to evaluate employee knowledge on the topic. Any employee who scores less than 95% on the posttest will need to repeat the training session. The infection class will begin first week of August for the cardiac staff nurses and the last two weeks of July for selected charge nurses who will help facilitate during the implementation of the educational program. 4.Brief solution description and rationale The way to reduce and eliminate infections in the hospital is to develop the proposed evidence–based solution of implementing an educational program with different training sessions to ... Only research will tell, since "nursing research is systematic inquiry designed to develop knowledge about issues of importance to nurses, including nursing practice, nursing education, and nursing administration" (Polit & Beck, 2006, p. 4). Problem Statement "Hyperglycemia (typically defined as a blood glucose of >200 mg/dL) is a well–known phenomenon in critically ill patients without a prior history of diabetes mellitus" (Langdon & Shriver, 2004, p. 163). While many studies have been completed with respect to management of hyperglycemia in diabetic surgical patients, there has been minimal research regarding the impact that tight glycemic control has on non–diabetic trauma and medical patients. With managed care requiring decreases in length of hospitalization and hospitals attempting to conserve cost expenditure, placing non–diabetic patients on insulin drips immediately upon entry into the health care system may adversely affect both of these areas. Trauma patients without a diagnosis of diabetes mellitus will require prolonged lengths of stay in the intensive care unit related to the fact that they must have closely monitored blood glucose levels. In addition to the increased duration of their intensive care unit admission, they are subjected to administration of medications they would not normally take at home. The inability to sleep is a source of anxiety and stress for patients in the intensive care unit (ICU); this inability can lead to cardiorespiratory disturbances, immune system dysfunction, impaired wound healing, hormonal and metabolic imbalances, cognitive changes, and delirium. Through the use of quiet times and nursing actions that encourage sleep, or, sleep hygiene bundles, nurses can assist in improving sleep quality in the ICU. These bundles include noise and light reduction, scheduling routine procedures outside of reserved sleep times, the use of earplugs and eye masks, and the grouping of nursing care activities to minimize sleep interruptions. The aim of this proposal, therefore, will be to implement a nurse–driven, sleep hygiene protocol, in order to improve patient care by promoting quality sleep in ICU patients. Barriers to implementation include staff resistance to the concept, concerns regarding patient care during sleep times and difficulties in coordinating care with other units. Studies used to gather information for this proposal were published between the years of 2011 and 2016 and were found in several databases: Academic Search Complete, Medline, and OVID Journals. Middle Range Theory Of Postpartum Depression Theory Essay Middle Range Theory Paper: Analysis of Postpartum Depression Theory Description of Theory– In 1993, Beck published a middle range theory on postpartum depression, entitled Teetering on the Edge. Beck, (1993, p. 44) which describes "walking on a fine line between sanity and insanity". Beck determined a limited amount of qualitative research available and few instruments for measurement of postpartum depression, postpartum psychosis, and "maternity blues". Beck interviewed 12 women for 18 months detailed their experiences, and determined the main perceived problem was a loss of control (Beck, 1993). Beck's theory is one that is centered in phenomenology and produced a concise and testable theory (Marsh, 2013). Loss of control is broken down into four stages: encountering terror, dying of self, struggling to survive, and regaining control (Beck, 1993). Beck, 2002, has developed two screening tools (Postpartum Depression Screening Scale and Postpartum Depression Inventory System) to evaluate risk factors and symptoms. Encountering terror is an initial response to feeling out of control, described as being trapped with no means of escape (Beck, 1993). Stage II is highlighted by a reported feeling of loss of former self and feeling internally void of caring or emotion, likened to that of a robot (Beck,1993). Struggling to survive is recognized by the inability to carry on activities of daily living, including eating and bathing. This is the stage where many seek assistance and ... • 29. The Five Fundamental Patterns Of Knowing In Nursing Patterns of knowing The five fundamental patterns of knowing are empirical, aesthetical, personal, ethical and sociopolitical. The patterns or ways of knowing help with the development and application of nursing knowledge. Empirical knowing embodies "factual and publicly verifiable descriptions, empiricism and theoretical explanations or predictions" (Carper, 2012 p.25). In my Oncology setting the science of nursing is very relevant to practice. I use science daily in calculating body surface area for chemotherapy drugs, chemotherapy protocols and febrile–neutropenia protocols with the initiation of specific antibiotics are some examples. Empirical knowing provides facts from empirical research for nursing care and interventions. Aesthetical knowing is known as the "art and act" of nursing, it highlights empathy. This pattern of knowing recognizes the nurse's perception of what is significant in an individual patient's behaviour (Carper, 2012). I utilize the art of nursing by acknowledging the patient's feelings and being present for them during a difficult time and the ability to establish a meaningful connection with the patient. Personal knowing refers to the "expressions of the quality and authenticity of the interpersonal process between each nurse and each patient, it looks at the interpersonal relationships of nursing" (Fawcett, Watson, Neuman, Walker, Fitzpatrick, 2001, p. 25). Orientation Of The New Graduate Nurse Orientation of the New Graduate Nurse: A Change Proposal A newly graduated registered nurse is well prepared to pass NCLEX for licensure; however, being prepared for the acute care setting in a hospital is a completely different issue. Nursing schools develop the foundation of nursing knowledge, creates critical thinking skills, and touches on nursing tasks. All of these are great tools to possess, but they do not prepare the new graduate nurse for safe, acute patient care. At Boulder Community Health (BCH) on the orthopedic neurology unit, new graduate nurses are given five weeks of orientation. Expecting a newly licensed nurse to learn all of a hospitals policies and procedures concerning patient care and how to incorporate the policies into their practice after five weeks is highly risky and leads to unsafe patient care. On the orthopedic neurology unit at BCH the new nurse trains with several nurses during the five week orientation period. This does not allow for the nurse to learn a consistent system to follow or incorporate into their personal practice as each nurse has their own system. This can become frustrating for the novice nurse. This paper will propose a change in the orientation program at BCH. Due to the area of the wound, the nurse educates a family member on wound care and dressing changes. The nurse requires that the family member return a demonstration to prove understanding of the task. The bachelor level nurse acknowledges the need for follow up care in the home setting. Applying Roy's Theory in Nursing Practice To support excellence in nursing practice, Roy's Adaptation Model (RAM) guides nurses in developing effective care through the use of reasoning, critical thinking, and decision–making. With a focus on adaptation, RAM can be utilized as a framework to conceptualize and plan individualized patient care or may be applied in a limited context to develop interventions for distinct clinical populations (Senesac, 2013). Through the nursing process, patients are assisted in adapting, or coping, to changes within their environments. The process begins with an assessment of the patient's adaptive behavior as well as stimuli in the external and internal environments. Based on these evaluations, the nurse develops nursing diagnoses and interventions to guide goal setting and increase successful adaptations or coping mechanisms (Chitty & Black, 2007). Simply stated, the nurse modifies the environment to promote and facilitate patient adaptation and restore balance. Adaptation is observed through modifications in behavior. • 40. Reducing Central Line Infections? Reducing Central Line Infections Central lines are a one of the major risk factors for infections in the bloodstream and many occur outside of the intensive care units (Medina, Serratt, Pelter, & Brancamp, 2014). An increase in the amount of patients who are admitted to the hospital with central lines has increased the amounts of central line infections. Reducing the amount of central line infections on a general Medical–Surgical unit with in a facility is very important for both patients and healthcare facilities. Preparation The issue is the increase in the number central line infections on the general Medical–Surgical floor. The method used to determine the issue on the Medical–Surgical unit is personal and professional experience. The nursing staff on the Medical–Surgical unit has recently experienced an increase in patients with central lines. This is clinically significant, because an increase in central lines increases the risk for increased central line infections. Infections in the bloodstream are attributed to increased length of stay and increasing costs, including the reduction in reimbursement to the facilities (Medina et al., 2014). Reducing central line infections will reduce hospital costs, reduce stress for the patients, and allow the nursing staff to provide patient safe practice. Swift's bizarre, yet well thought out plan for helping families of Ireland, who were in poverty and desperately in the need of money, was fattening children and selling them to the wealthy landowners. He thought that this would be an excellent way for children not to be a burden on their parents but be beneficial. Swift even went on to write a pamphlet telling the parents just how they could fatten their babies and the perfect age and weight for the kids to be before selling them. One of Swift's friends goes on to give some helpful/ tasty ways that the kids can be cooked before eaten. Swift's proposal does not only financially ... Show more content on Helpwriting.net ... There only remain a hundred and twenty thousand children of poor parents who are annually born"(5). By Swift including these statistics shows that he has well thought out this plan, and Swift has done plenty of research before coming to his conclusion. Swift later gives credibility to his friend from London by saying, "a young healthy child well nursed, is, at a year old, a most delicious nourishing and wholesome food"(9). All three rhetorical devices that he used were great ways to persuade the people of Ireland ,that were in poverty, that this was the best option to help them prosper. Swift expresses upon the issue that merchants would rather the parents sell their children as babies because they are then more valuable. Swift then adds some credible information proving that he has done his research, "I am assured by our merchants, that a boy or girl before twelve years old, is no saleable commodity, and even when they come to this age they will not yield above three pounds." Swift uses his statics to prove that his solution was well thought out by saying, "computed the charge of nursing a beggar's child to be about two per annum." This shows that swift had done his research for finding out not only the population of the city, but he found out the pricing for nursing a child that was soon to be sold and eaten. Swift's uses some content to get an emotional reaction from the reader so that they can be more on board with his proposal. For example, Swift ... Get more on HelpWriting.net ...
