"Family–centered care" is a term heard often in healthcare settings and in nursing practice. Family–
centered care has been recognized as being an integral part to patient health, satisfaction and health
care quality (Kuo, et al, 2012). Family–centered care is implemented with the goal to increase
partnerships between, families, patients and providers; and has been prioritized as a core–concept of
quality healthcare (Gallo, Hill, Hoagwood & Olin, 2016) Many professionals, however, would be
hard–pressed to state what the term "family–centered care" actually means and how it applies to
nursing practice. They would be at a loss for how to implement family–centered care and what is
absolutely necessary to have in order for family–centered care to be successful. Advanced practice
nurses are faced with the challenge of adhering to family–centered care in their practice. Illness, both

experience? What would it look like? How would they benefit? I plan to discuss these hypotheticals
through research regarding Patient–Centered Care. After all, studies and surveys performed by the
Institute of Medicine and the Institute for Healthcare Improvement, read that engaged patients in any
health care setting, have better perceived health outcomes, thus higher actual quality patient outcomes.
Relating to the above, I believe that engaged learners have a better understanding of the material and
thus more effective practice outcomes. Because of this belief, I would use informal lecture coupled
with a series of patient narratives and class discussions regarding the patient–centeredness of care–
team and patient interactions. In teams, we would discuss patient outcomes, care team involvement
and behaviors, provider prompts, patient prompts, professional responsibility, what was and was not
patient–centered, barriers to care and areas of opportunity to meet patient needs. After discussion,
those learning would be responsible to research an assigned condition and to create a patient centered

be explained procedures thoroughly and they should also be involved in the evaluation of the care.
Spruce (2015) mentioned the importance of health literacy among patients in regards to patient
centered care (p. 35). This is beneficial because if the patient is able to understand the process of their
illness, then they will be better able to understand the treatments available and what they wish to do in

Family centered care (FCC) is an effective framework that many nurse practitioners have adopted in
the United States. [1] As early as 1966, researchers such as Claire Fagin PhD. have called for a
transformation in the role that pediatric nurse practitioners (PNPs) play in promoting bonding between
newborns and their mothers. Today, nurse practitioners have answered this call by implementing
family centered care, which blends clinical practice, caregiving, family principles in the health care
environment. Practitioners who implement FCC frameworks promote professional accountability for
the welfare of children, as well as the physical, emotional and psychological well–being of newborns.
An brief overview of the philosophy embraced by FCC nurse practitioners is outlined in the following
four sections.
Principle 1: Open Communication

academic setting, educators teach FCC principle during Family Centered Rounds (FCRs). FCRs

units that have adopted an open visitation policy is not formally recorded, however, the Canadian
Association of Critical Nurses recognizes that open visitation is not a standardized practice across the
country.3 Nurses play a large role in determining visitation practices. The culture of the critical care
unit, negative perceptions of family presence, gaps in knowledge about the beneficial effects of open
visitation, and the lack of a formal policy influences the decision–making processes to allow

system due to the demand of high–quality medical care (Schwarzkopf et al, 2013). Family–centered
care identifies that patients exist within a wider social networking of family members and friends
(Huffiness et al., 2013). In 2001, the Institute of Medicine published its seminal report, Crossing the
Quality Chasm, which depicted the involvement and support of family and friends as a dimension of
patient–centeredness (Institute of Medicine, 2001). Patients in the intensive care unit (ICU) often do
not have decision–making capabilities or are unable to communicate with the health care team due to
the severity of illness or the effects of treatment. Family members often assume responsibility and act

needed. However, I have to work directly with her family to understand communication with the

surpassed the basic client/health care provider relationship, FCC encompasses the needs of the entire
family as an entity. Benefits that have been noted include knowledge increase from the patient's
perspective, better self–care, and self–esteem of the caregivers, which lead to better outcomes for the

The age ranges from 23 weeks gestation to usually 40 weeks gestational. However, a readmission of
an infant within ten days post delivery may be readmitted if previously born within our facility. There
are no maximum lengths of stay. Our ultimate goal is one of the 40 weeks gestation.Early skin to skin
initiation is the preferred form of early bonding, and early initiation of teaching and family
participation is encouraged from the get go. Wellington serves a community that has a majority of
White Non–Hispanic population. The outlying communities that utilize our services compromise a
majority of Hispanic, Latino, Haitian, African American population. Currently, our staff is
compromised to meet the majority of the population; we lack in the services in clientele that have
recently migrated to the country or have limited understanding of the English language. We offer a
language line and certain staff members that are utilized to facilitate in translation. Unfortunately, they
are not always available. The author is employed as the Manager of the Neonatal intensive care unit.
The unit uses a primary nursing care model and an all registered nurse staff. The group is headed by a
nurse manager who reports to the Director of women's services. Currently, the unit achieves a daily
census of 12; our breastfeeding rate is 90 percent, and our focus is on family–centered care. Family–

Patient Centered Care Florence Nightingale paved the way for patient centered care in nursing. She
realized that patients should be treated with dignity, respect and treatments that are in the best health
interest of the patient. The Institute of Medicine defines patient–centered care (PCC) as "care that is
respectful of and responsive to individual patient preferences, needs and values," ensuring that patient
values guide all clinical decisions. (Finkelstein, 2012) Being hospitalized can be a stressful time for

Talllon et al. conducted a study to see if there is an alternative health care model more effective than
family–centered care. The paper criticized family–centered care and its extreme focus on attachment
there and its inability to look at other relevant research theories. The alternative model focused on the
physical and emotional influences that parents and family have on their children's' health. This
alternative model would combine knowledge from many disciplines to create optimal care from
nurses. Nurses would be able to enhance care and health care outcomes of children and their families.

regarding patient and family centered care and leadership. The views that the respiratory therapist had
in regards to the subject were very similar to my own views. The respiratory therapist stated that

Current Practice Setting
The Family Health Center is a clinic that is under the umbrella of Bear Valley Community Hospital, a
small rural hospital. The hospital is located in a ski resort town in the mountains above San
Bernardino County. The next closest medical facility is an approximate hour drive on curvy mountain
roads. The hospital is a standalone hospital, not associated with any other hospitals or chains. Bear
Valley Community Hospital provides emergency room services, has eleven acute beds and a skilled
nursing facility. Bear Valley Community Hospital accepts a wide variety of insurances and sees all
ages and types of patients. The Family Health Center has a variety of specialty providers, as well as
family practice. They have doctors with specialties in internal medicine, family practice,
obstetrics/gynecology, pain control and chiropractic services (Family Health Center, n.d.). There is a
telemedicine program that currently provides psychology services through telemedicine (Family
Health Center, n.d.). There are several Family Nurse Practitioners and a Pediatric Nurse Practitioner
(Family Health Center, n.d.). As a result they see patients from pregnancy through adulthood. The

During a patients' stay in the hospital the most important care given is to encourage family centered
care. Last year, my younger sister had recurring strep throat. Which lead to the decision to get her
tonsils removed. My sister is terrified of needles and the idea of a hospital makes her jump out of her

From being admitted, all the way to recovery, the nurses really made the experience less frightening.
The staff focused on having my Mom and I present before and after her procedure. As a student, I
observed the way the nurses were acting towards my sister. The nurses exhibited the proper way I
should be to my patients and to the patient's family: respect, patience and a calm environment to help
promote healing. Having the family at the patient's bedside and teaching family how to care for their
loved one, is the most important step in a patient's recovery.
The main component of family centered care is to ensure a patient, their family and health care

This is particularly important in the pediatric population. The family is a huge part in a child's healing
and comfort. A hospital can be overwhelming for a child so in turn the child will want to be
surrounded by family, this will help to ensure quicker recovery and help comfort their anxiety. Nurses
need to attempt to involve the child's family as much as possible, ultimately the child's stay becomes
comfortable and helps to relieve any anxieties the child would be feeling. Sometimes a simple act of
allowing the child to try on their stethoscope and listen to their parent's heartbeats, will create a sense
of trust and relieve the child's nerves.

The Five Core Competencies In The Healthcare System
The five core competencies identified by IOM and the sixth added by QSEN, safety, are believed to
be necessary to improve both quality and safety of the healthcare system within which nurses work
(Multimethod teaching). The six core competencies outlined are patient–centered care, teamwork and
collaboration, evidence–based practice, quality improvement, informatics, and safety (Diffusing
Qsen). While all competencies are significant to the healthcare system, patient centered care is vital to
positive patient outcomes and focuses on the patient's perspective within the healthcare system.
Patient–Centered Care Patient–centered care encompasses ideas, skills, and attitudes that promote
greater patient empowerment, responsibility, and prioritizing patients' needs and experience of
healthcare in influencing how care is delivered (Lorig, 2012). Patient–centered care focuses on
individualizing care to meet the health care needs of individual patients but also encourages patient's
to be active participants in their own care. Important aspects of patient–centered care that focuses on

Family Centered Care in the Postpartum Setting Although an admission into a facility with the
purpose of bringing new life into the world is a joyous time for a family; any visit to a hospital setting
may still evoke stress and anxiety for both for the family and the patient. Utilizing family centred
care, this stress and anxiety can be decreased by collaborating and ensuring that the patients' needs are

woman from Thailand and married to an American husband. We have two children, a twelve–year–old
daughter and a ten–year–old son. We move here eight years ago from Bangkok, Thailand. Nursing is
my second career after being an environmental engineer, a sale executive, and a stay–at–home mom. I
went back school after my son went school. I earned Advance Degree in Nursing from Valencia
College. After graduated, I started my nursing career at Florida Hospital Orlando. Over a year after
working on my first unit, I enrolled in a BSN program and received my BSN from University of
Central Florida in May 2015. My first unit, where I started working a new graduate nurse (GN), was a
multi–system progressive care unit. I have had opportunities to learn about many diseases from all
body systems. I also had the opportunities to take care of many wounds. I found that wound care is
very interesting field. Most of my novice on this unit I learnt about time and task management and
many nursing skills. I also have the opportunity to work for Florida Hospital at another campus.
Although it is the multi–system PCU as the same as my first unit, the population of patients are
different. Majority of patients are geriatric patients with high acuity level and comorbidities. I found

Out of the 101 articles that were identified by the researchers were 25 distinct frameworks and models
that dealt with patient–centered care. The authors wanted to demonstrate the effectiveness of
application of each of the 25 models or a combination of them. The three tenants that were envisaged
by the Constand and others were health promotion, partnership and communication. The different
frameworks were weighed and tested against each other in order to try and develop one framework or
identify a single framework that would be generically applied in patient–centered care. The
