2. Karpāla kanāla sindroms (KKS)
Definīcija: mediānā nerva (n.medianus) jušanas un motoras
darbības traucējumi, kas rodas, tam ejot cauri karpālājam
kanālam.
3. Epidemioloģija:
Incidence: pieaugušie 0.15/1000
Prevalence: vīrieši: 28/1000 sievietes: 46/1000
Lielāka incidence 25-64 vecumā grupā
Sievietēm 2-5 reizes biežāk
Kā ģenētiskā patoloģija reta
Bieži sastopama ofisa strādniekiem, datu ievadītājiem, programmētajiem, kā arī
strādniekiem ,kas izmanto vibrinstrumentus , produktu līnijas strādnieki.

7. Patoģenēze:
Nerva kompresijas ietekme uz intraneirālo asinsapgādi
20-30mmHg samazina venozo atteci nervā
Aksonālais transports pasliktinās pie 30mmHg
Neirofizioloģiskas izmaiņas (motora un sensora disfunkcija) paradās pie
40-60mmHg – sensorais un motorais bloks
60-80mmHg – intraneirāla asins plūsma apstājas.
KKS – demielinācija, vaskulāra skleroze, tūska, amiloīda depozīti
8. Sūdzības:
Jušanas traucējumi pirkstos, par kuriem atbild mediānais nervs, un plaukstas vidus daļā, kā
darbības traucējumi sīkos plaukstas muskuļos.
Sāpes pa cīpslu gaitu, nespēja pilnā apjomā pakustināt pirkstus, spēcīgas sāpes plaukstas
locītavā (stenozējošs ligamentīts).
Diskomforts pastiprinās pie plaukstas kustībām.
Dienas laikā - jušanas traucējumi, tirpšana, sīki kustību traucējumi.
Naktīs - bieži vien jāmostas, lai roku izvingrinātu spēcīgas tirpšanas un plaukstas sāpju dēļ.
Sajūta, ka no rīta roka ir pietūkusi.
Var būt dedzinošas sāpes plaukstā un pirkstos, sāpes var izstarot elkoņa virzienā.
Samazināta manuāla aktivitāte un pavājināts rokas tvēriens.
Diagnostika
12. Ārstešana
Konservatīva ārstēšana:
Aktivitāšu modifikācijas (ergonomika, aktivitāšu maiņa,
ortozes)
Pirkstu, karpāla kanāla stiepjoši vingrinājumi.
NSPL: 2-3 nedēļu kurss
(ibuprofen ,diclofenac , naproxen , indomethacin) atvieglo
simptomus mazinot tūsku karpālā tunelī
Lokālas steroīdu injekcijas – atvieglo simptomātiku
Vitamīni (B komplekss)
Zemas devas orāli kortikosteroīdi
Fizikāla terapija: Akupunktura , Hidroterapija,
Ultrasonografija , Elektromagenetiska lauka terapija
Mērķis ir samazināt tirpšanu un nejutīgumu mierīgs naktsmiegs ; novērst motoro funkciju
pasliktināšanos; novērst ikdienas diskomfortu ; Kollesa lūzuma gadījumā novērst kanāla
imobilizāciju

14. Ķirurģiska ārstēšana:
Vaļēja ķirurģiska dekompresija – retinaculum flexorum pārdalīšana (85-90%)
Izmanto gan reģionālo, gan lokālo ,gan vispārējo anestēziju.
Asiņošanas novēršanai operācija tiek veikta ar žņaugu.
• Griezums ir ap 5cm, kas ļauj redzēt fasciju.
• Plaukstas šķērssaite tiek pārgriezta proksimāli

16. Divu incīziju tehnika:
Trula dissekcija ( distāla kanāla gala identifikācija)
Pirksti un plauksta ir izstiepti un nostiprināti.
Anterogrādi troakārs ar nazi iet cauri proksimālai
incīzijai līdz distālai.
Endoskops caur proksimālo incīziju distāla
saites puse grieztā ar speciālu nazi (no distāla
gala)
Pēc naža otras izņemšanas tiek izņemta
proksimāla saites daļa.
Ja vizualizācija ir nesekmīga, procedūra tiek atcelta un tiek pārveidotā par ķirurģisku kanāla atvēršanu

Biežāk sievietēm
Nav datu par UTS incidenci un prevalenci
Sastopams krietni retāk par karpālo kanāla sindromu vai kubitālo tuneļa sindromu
Manifistējas bieži kā motora neiropatija bez sāpēm
23. Ārstēšana
Konservatīva:
Kombinācija NSPL vai COX-2 inhibitori + fizikāla terapija
Ierobežot atkārtotas plaukstu kustības, kas sekmē sindroma attīstību
Lokālas anestēzijas injekcijas + steroīdi ulnārā nervā
24. Ķirurģiska ārstēšana:
Kad kompresijas iemesls ir veidojums vai neefektīva konservatīva ārstēšana
Reģionālo, lokāla vai vispārējo anestēziju.
Operācija tiek veikta ar žņaugu.
Pacienta roka novietota supinatio pozā
25. Palmaris brevis tiek proksimali griezts
Dziļāk tiek atvērta antebrachiāla fascija virs proksimālās plaukstu locītavas, lai radītu ulnāra

27. AIM:
Current study planned to investigate histologic findings and differences in carpal tunnel syndrome (CTS)
diabetes mellitus, thyroid disease and idiopathic CTS patients.
MATERIAL AND METHODS:
Subsynovial connective tissue samples of 51 idiopathic CTS patients, 58 patients with diabetes mellitus and
CTS, 16 patients with hypothyroidism and CTS were evaluated for this study.
The histopathologic examination parameters were number of fibroblasts, size of collagen fibers, vascular
changes (vascular proliferation, intimal thickening and changes of vessel structures), edema and
infiltration.
RESULTS:
Non-inflammatory fibrosis was observed in subsynovial connective tissue when general histomorphological
evaluation in majority of tissue samples taken from all three patient groups.
Number of fibroblasts, collagen fiber diameter and lengths were found statistically different at diabetic CTS
patients when compared with other both groups.
Considering the overall results, neovascularization in subsynovial connective tissue is observed significantly
more intense in the cases of diabetic CTS and severe edema is found in the cases of hypothyroidism and
CONCLUSION:
Increased pressure of carpal tunnel may be as a result of reduction of the space size or increase in the
of tunnel contents depending on cause such as fibrosis or edema with different etiopathogenesis.
The better understanding of the causes of this entrapment neuropathy appears to be helpful to physicians
and these causes have to take into consideration during preoperative evaluation.

Ja karpālā kanāla sindroma rašanās iemesls ir bijis cīpslu iekaisums, tad vēlākās stadijās var būt novērojamas sūdzības, kas saistās ar cīpslu darbību
sīkiem kustību traucējumiem (piem., grūti ievērt adatā diegu, aizpogāt pogu, saskaitīt naudas monētas u.c.),

Sensora:
Monofilaments of increasing diameter are pressed perpendicularly against the palmar aspect of each finger until the monofilament bends to determine the sensory threshold for each finger;
A 256-Hz tuning fork is struck against an object, causing it to vibrate, and the fork's prong is then placed against the patient's fingertips; the median and ulnar fingers of both hands are tested, and the test is considered positive if decreased sensation is perceived
Static and moving two-point discrimination - This is the minimum separation between two points (either static or moving) that can be perceived; failure to discriminate more than 6 mm (static) or 5 mm (moving) is a positive finding

Carpal compression test - Direct application of pressure of 150 mm Hg or even pressure from both thumbs of the examiner is exerted on the patient's carpal canal and maintained for 30 seconds; the test result is positive if pain, numbness, or paresthesia develops in the radial-side digits
Positive Tinel sign (tapping over the median nerve at the wrist will reproduce the patient's symptoms and may cause tingling and/or painful dysesthesias)
Positive Phalen sign (hyperflexing the hand at the wrist passively for up to 60 seconds reproduces the patient's symptoms)
Positive reverse Phalen sign (hyperextending the hand at the wrist for up to 60 seconds reproduces the patient's symptoms)
Direct compression of the median nerve over the wrist can elicit or exacerbate symptoms
In advanced cases, wasting of the thenar eminence and weakness of thumb abduction (usually first) and opposition (usually later)
(MRI) has consistently shown the greatest sensitivity and specificity in the diagnosis and evaluation of carpal tunnel syndrome.
electrodiagnostic tests such as EMG and nerve conduction studies are 85-90% accurate in patients with carpal tunnel syndrome, with a false-negative rate of 10-15%.
Nerve conduction studies and electromyography confirm the diagnosis and differentiate carpal tunnel syndrome from cervical radiculopathy, thoracic outlet syndrome, median nerve compression in the proximal forearm (pronator teres syndrome) or at the level of the elbow (from a ligament of Struthers or bony spicule), and generalized peripheral neuropathy
Magnetic resonance imaging (MRI) of the hand can be used in atypical cases or surgical failures; can also rule out tumors and other structural abnormalities. The median nerve configuration within the carpal tunnel may be proximally enlarged and distally flattened. Signal on T2 and STIR pulse sequences is increased within the median nerve due to edema and/or demyelination
Alternatively, ultrasonography of the hand also can detect abnormal enlargement or flattening of the nerve within the carpal tunnel, which indicates ongoing compression of the nerve. Reduced movement of the median nerve within the carpal tunnel during flexion and extension of the hand at the wrist may indicate tethering. Ultrasonography can visualize the median nerve during dynamic movement maneuvers

Because organic lesions are often implicated in the cause, surgical exploration and decompression of the ulnar tunnel is a common treatment modality.
Demonstration of the fibrous archlike origin of the hypothenar muscles. The hook of the hamate is ink-marked. The deep motor branch of the ulnar nerve courses deep to the fibrous arch.

After completion of the decompression, the tourniquet is deflated and adequate hemostasis is ensured. The wound is closed and a bulky dressing or plaster splint is applied. Sutures are frequently removed at 10 to 14 days postoperatively.
