1. PURPOSE
This Environmental Health and Safety Plan (EHSP) have been written specifically for
the JSW Jaigarh Conveyor Project.
This document defines the key areas affecting the health and safety of Sandvik Asia
Pvt Limited (MISY) employees and documents the safety management system that
will be implemented to effectively eliminate or control workplace hazards and their
associated risks.
This EHSP is based on the SAPL (MiSy) corporate safety philosophy of “Zero Harm”
and our commitment to ensure a safe work environment for all project employees,
contractors and visitors.
This EHSP outlines the framework that SAPL (MiSy) will use to conduct our activities
whilst on the JSW Jaigarh conveyor Project site.
OBJECTIVES & TARGETS
SAPL (MiSy) understands that measurable criteria to evaluate safety performance
need to be implemented. Accordingly, SAPL (MISY)’s site performance will be
measured against the following criteria:
 Workplace Injuries (Lost Time Injuries, Restricted Work Injuries, Medical
Treatment Injury, First Aid, Minor Injury);
 Hazards and Near-Miss (Significant Potential) Incidents and corrective actions;
 Environmental Incidents;
 Attendance at PROJECT Safety and / or Toolbox meetings;
 Inspections and Observations undertaken;
The SAPL (MiSy) Site Manager shall ensure a monthly report is generated and
submitted in accordance with SAPL (MISY)’s monthly reporting requirements. It also
have objective of:
 Meeting Contractual agreements.
 To determine broad parameters of EHS management at site.
 Establish & define line of command for resolution of all hazard prevention issues.
 To ensure compliance with relevant applicable legislation.
 Continual EHS performance improvement by directing focus on the key areas for
improvement in consistent manner.

3. LEADERSHIP & COMMITMENT
SAPL (MiSy) recognises that JSW will hold the SAPL (MiSy) Project Manager
accountable for the actions of all SAPL (MiSy) employees, contractors and visitors.
SAPL (MiSy) accepts there will be a share accountability and responsibility role for
providing:
• Safe places of work;
• Safe systems of work;
• Safe plant and equipment;
• Safe & Environmentally compliant work practices and procedures;
• Adequate supervision; and
• Suitable training for SAPL (MiSy) personnel.
Accordingly, The SAPL (MiSy) Project Manager will be responsible for ensuring all
SAPL (MiSy) employees, contractors and visitors are familiar and fully comply with
the requirements of the EHSP and JSW site health, safety and environmental
practises and procedures.

7. EHS RISK ASSESSMENT
7.1 PURPOSE:
To assess the risk of the activity to be executed, rate the risk levels as per the risk
assessments matrix, and identify the control measures so as to bring the risk level to
ALARP (as low as reasonable practical).

09. GENERAL EHS GUIDELINES, RULES & REGULATIONS
This section shall detail the general EHS guidelines, rules & regulations to be adopted in
the site for the following areas.
PPE (Personal Protective Equipment’s)
Head Protection:-
All personals engaged for this job has to wear Safety Helmet along with Chin Strap.
Hand Protection:-
Appropriate Hand Gloves will be used for Materials handling, Concreting, Welding,
Grinding, Gas cutting, for Chemical Handling & Electrical work.
Eye Protection:-
Suitable goggles must be ensured for the personnel deployed for Welding (Face
Shield) , Grinding , Gas cutting , Concreting , Chipping etc. Work at site.
Ear Protection:-
The personnel engaged in the noise zone such as Compressor Operator, Pneumatic
Vibrators & Breakers, DG Operator etc. will be equipped with suitable ear protection.
Protection from Dust & other poisonous gases:-
Suitable nose mask, face shield are to be used by the individuals to protect them
from dust, fumes & poisonous and toxic gases.
Work at Height & Fall Protection:-
All personnel working over 2.0 meter height must have the safety belt tied in his waste
at the ground level. Further he will have to anchor the safety belt with a fixed structure
before starting the work at height. Some special type of Safety Belt like Fall Arrest
Devices also will be in used according to the nature & area of work. Safety net and life
line system will be adopted to protect the fall of man and material.
Access, Egress & Workstation Safety:-
Proper access to the work place will be provided by suitable means before engaging
workmen inside the work area. The accesses must be suitable, stronger & firm

enough as per the standard norm. The stacking of materials on the access points
should be avoided so as to facilitate the workmen for free movement. The
housekeeping & illumination on the accesses will be also ensured. The work area will
be made clear from unwanted materials. Suitable platforms are to be provided before
allowing any workmen to work at height. The housekeeping & illumination of the work
area will also be ensured in the work area. The elevated platforms are to be provided
with adequate supports so as to ensure the stability of the working platforms. The
openings & cut outs are to be strictly avoided in the work area or working platforms to
avoid fall of person & materials from Height.
Electrical Safety:-
Electrical distribution system plan & sketch of DB’s & cable laying is to be prepared.
The cables for power supply must be routed through underground with cable
indicators so as to locate & maintain the same will be convenient. If the cable is to be
routed above ground, make sure it is not lying on ground instead wooden/bamboo
poles shall be used for cable laying. Distribution Boards are to be inspected regularly
by the concern person (Electrician) at least once in a week. The megger values of all
cables are to be checked in a regular interval to avoid electrical hazards. All the
power supply to site for portable tools must be routed through ELCB / RCCB. All the
installations, DB’s, SDB‘s at site are to be earthed as per specification. Log out tag
out (LOTO) system to be followed at site. All power driven hand tools are to be
inspected jointly by safety personal and electrician at regular intervals.
Hot work (Welding & Gas cutting) and Work Permit System:-
While hot work ensures the following points:
 All welders and cutters have adequate training.
 All welders and cutters are trained in correct use of PPE.
 Adequate warning signs displayed.

 All welding machines are inspected, properly earthed and ELCB/RCCB installed.
 Welding and gas cutting DO’s and DON’ts are to be instructed through daily tool
box / Pep talk.
The following work permit systems for the site are to be followed:
 Hot work & Height Work.
Use, Maintenance and Inspection of Plant, Machinery & Vehicle:-
All the tools, tackles & portable machinery are to be thoroughly inspected & checked
before issuing them for site use. Further the maintenance of all such machinery are to
be conducted regularly in a proper interval (once in a week) to ensure better utilization
of the same at site. The maintenance of plant & machinery like Cranes, Hydra etc. are
to be conducted properly (once in a week). However the inspection for the same
equipment will also carried out jointly by EHS & Operator once in a month to ensure
the safe use of the same in construction activities. The tools & tackles are to be
thoroughly checked by the concern supervisor for its capability & suitability before
deployment of the same at site. Maximum speed limit at site should not be more
than15 KM/Hour while driving.
Scaffold & Work Platforms:-
All steel scaffolds must satisfy the specified standards. These should be checked
before erection of the same at site. During erection of the same the standard norms
for scaffoldings are to be followed at site. Scaffold should not be tampered by any
physical agents. These are to be maintained properly in regular intervals to ensure
safety of the Personnel working over it & for its longer life. These are to be handled
properly at site. Throwing them from height is to be restricted as there are chances of
developing cracks & getting damage. The Shifting & placement of the same at site
are to be carried out by the supervision of an Engineer / Supervisor so as to ensure
the safe use of the same at site. Adequate supports for scaffold are to be given at site

for their stability. Site EHS Officer will be authorized to certify the safe/unsafe
condition of scaffold prior to use. Upon inspection green/red tag shall be installed to
identify the condition of scaffold.
Erection & Rigging:-
 There must be a fixed system of signaling, clearly understood by crane operator
 All cranes, lifting tools and tackles are to be tested and certified by a competent
person.
 Do not leave loads suspended.
 Do not drop freely, snatch, lift or shock load.
 Do not swing loads into position, they must be straight lifted.
 Temporary ladders to cabins must be lashed.
 Test the load before making the full lift, never exceed the safe working
load of the crane and avoid over winding.
 Make sure that it is safe to commence lifting and that all personnel are
clear of the load.
Mobile Crane Operation:-
 Before starting, make sure that all controls are in neutral.
 Ensure that the safe load indicator is working satisfactorily.
 Check all round visibility and note any particular hazards such as overhead
power lines.
 Make sure that your emergency escape system is in working order.
 Ensure that youth hook is clear of obstructions before you move it.
 Hoist, lower and traverse smoothly to prevent the load swinging.
 Make sure that the fire extinguisher is in proper place and that the operator
knows how to use it.

 Never allow unauthorized person to operate your crane.
Lifting Tools/Tackles Safety (Chain Slings, Wire Rope Slings etc.):-
 Never overload any lifting tackle.
 Use the lifting tackles properly on the load.
 Do not use damaged tackles.
 Do not use any tackle without safe working load marked on it.
Critical lift plan, Handling & Storage of material:-
 Before commencing any critical erection, a lift plan is to be prepared by concern
engineer.
 Proper storage of all materials is to be ensured and safe material handling
procedure is to be instructed by concern engineer / site EHS Officer.
Housekeeping:-
Housekeeping on site is daily/continual job and is to be based on the following steps:
 Designate / demarcate place for each type of material (scrap, Hazardous waste,
Non Hazardous Waste etc)
 Plan access in such a way that each place is easily accessible.
 Clean up work place upon completion of work.
Firefighting & prevention:-
Portable fire extinguishers shall be provided for firefighting. Quantity and type shall be
determined as per site requirement. At least 10 percent of total workforce should be
covered in basic firefighting training.

Gas Cylinder Storage & Handling:-
 Gas cylinder whether in use or empty/filled must be secured in vertical position.
 Cylinder storage area to be top covered, well ventilated and away from hot work.
 Use flash back arrestors at both ends.
 All pipe attachments to be done by clamps/nozzles.
 Not to use LPG for hot work, instead use DA.
Physical Fitness of Workers:-
All new workers should go through a medical checkup to determine his health condition.
Environment Protection/Waste Management:-
The SAPL (MiSy) Site Manager shall ensure all environmental aspects and impacts that
affect SAPL (MiSy) operations on site are identified and included within the SAPL (MiSy)
and site risk register. This will highlight the risk and potential significance of the particular
aspect and list the control measures to minimize the potential risk. All work activities shall
be carried out in such a way to both manage and reduce waste, and to prevent or
minimize contamination to land, soil and ground water. Site Managers shall ensure all
waste is managed according to the Waste Management procedures. The materials shall
be disposed of appropriately and corrective action shall be undertaken.

Regular safety inspection of various installations and work process shall be conducted by
Site EHS Officer and reports should be submitted to sub-contractor for corrective action.
Monthly site EHS audit shall be conducted by EHS Manager and report should be
submitted to Site Manager for corrective action.
Engineering & Design Studies:-
The SAPL (MiSy) Site Manager shall ensure engineering and design studies are
conducted, when required to identify hazards to people, property and the environment
that may arise during construction, commissioning, and operation and maintenance of
plant. These studies shall be translated into design, construction methods that eliminate,
reduce and control, far as practicable, hazardous tasks.

10. EMERGENCY RESPONSE PLAN
Emergencies can happen at any time of the day and night. The initial actions of an
individual can play an important part in coping with the incident to minimize injury and
damage. To this end, it is very important that the individual knows what to do and how to
do it.
What to do in case of fire:
1. Sound the alarm or otherwise inform others by shouting ‘fire, fire‘. Who may be in
danger, call ___________________ for help.
2. With the minimum personal risk, attack the fire with the fire extinguishers provided
nearby, using them as instructed.

fighting’.

2. When transmitting a message clearly identify yourself by giving:
 Your name & your exact location
 Nature of emergency
 Service required and repeat the message

b. Report to Site EHS Officer/Supervisor at once on your safe arrival to assembly

What to do in case of medical emergency:
1. Do not try to move an injured person if you are not fully trained on first aid.
2. Call on ___________________ to get necessary medical help.
3. The scene of an incident must be left undisturbed.
4. In some case the incident site must be rendered safe so as not to contribute to
further incidents. Hence cordon off the area and do not allow anyone to enter.
5. Retain the eyewitnesses of the incident until the investigation team has arrived to
the spot.

One dedicated emergency vehicle shall be available at site during work hours.
Adequate numbers of First Aid Kits & Trained first aider shall be available at site.

11. TRAINING, COMMUNICATION & REPORTING
EHS Induction:-
SAPL (MiSy) shall ensure that all visitors, Staff & workmen on site undertake a SAPL
(MiSy) Site Specific EHS Induction before entering into the site/commencing work.
Daily Tool Box Talk:-
SAPL (MiSy) shall ensure that all workmen on site is attending the tool box talk given by
supervisors in the morning prior to start the work.
Task-Specific EHS Training:-
SAPL (MiSy) shall ensure task-specific EHS training is provided to all SAPL (MiSy)
workers and Sub-Contractors as required. Training may include topics such as Risk
Assessment Team (RATS), Incident Investigation, First Aid, Fire Fighting, Confined
Space Entry, Working at Heights, Defensive Driving, Electrical Safety, Welding/Cutting
Safety, etc.
SAPL (MiSy) shall ensure that appropriate training material is developed for the level and
scope of the work and activities undertaken by SAPL (MISY). A Site Training Matrix is
developed and forwarded to the Project Manager/EHS Manager for future reference.
Ensure all training undertaken on site is undertaken and recorded in accordance with
SAPL (MiSy) Policy.
Cascading any EHS messages down the line is vital for the success of any EHS
Management System and to ensure that all personnel are aware of EHS issues the
following technique shall be adopted.

Incident/Injury Reporting:-
All Incident/Injury causing loss or damage (including damage to the environment), and
incidents where there was the potential to cause loss or damage, shall be reported to the
Site Manager/Engineer/EHS Officer, who in turn will prepare and submit an incident
report within 24 hours. Requirements for recording and reporting of Incident/Injury shall
be consistent with SAPL (MiSy) Standards. Separate systems shall be maintained to
satisfy both requirements Incident and Hazard Notification, Reporting and Investigation.
In addition all SAPL (MiSy) Incidents/Injury are to be recorded using CINTELLATE
(Sandvik online EHS reporting).

11. EHS MANAGEMENT OF CONTRACTORS
Assessment:-
SAPL (MiSy) Contractors shall be evaluated to identify those who can demonstrate safety
performance that will complement our “Zero Harm” philosophy. The SAPL (MiSy) Site
Manager shall formally evaluate and assess the performance of each contractor in
accordance with SAPL (MiSy) Policy Standard: Contractor Management. The evaluation
shall consist of past performance and interviews with key personnel and site inspections
as necessary.
Kick-Off Meetings:-
SAPL (MiSy) shall conduct a contractor “Kick-Off” meeting with all SAPL (MiSy)
contractors. All safety matters pertaining to the particular contractor’s scope of work will
be reconfirmed at this meeting. Risk assessments shall be conducted with contractor as
part of the kick-off process in order to identify generic hazards and the risks associated
with their scope of work. Documentation relating to these “Kick-Off” meetings shall be
retained on the SAPL (MiSy) site specific EHS directory.
EHS Plan for contractor:-
A SAPL (MiSy) contractor is liable to follow this EHS Plan. Contractor’s EHS performance
shall be reviewed by project manager/site manager/ehs manager regularly.
Weekly Contractor Co-ordination Meetings:-
The SAPL (MiSy) Site Manager and Site EHS Officer shall attend weekly Contractor
Coordination Meeting to plan activities and co-ordinate interfaces with other contractors.

The SAPL (MiSy) Site Manager shall table hazard logs and action items arising from
audits for review and close out at these meetings. Action plans will be agreed and
documented to correct areas of underperformance. These hazard logs and action plans
shall be retained on the SAPL (MiSy) site specific EHS directory.
Contractor EHS Officer/Steward:-
SAPL (MiSy) shall ensure a full-time EHS Officer/Steward whenever twenty (20) or more
personnel are engaged in, or conducting manual work per each Contractors contract
scope. Resumes of proposed Contractor EHS Officer/Steward shall be submitted to the
Project Manager/EHS Manager for approval and, where necessary, interviews conducted
prior to mobilization.
The SAPL (MiSy) Project Manager shall ensure the SAPL (MiSy) Site EHS Officer and
Contractor EHS Officer/Steward attends the Weekly meeting to coordinate construction
and operations activities and develop and implement incident prevention and safety
improvement strategies. Minutes and actions arising from this meeting shall be retained
on the SAPL (MiSy) site specific EHS directory.

EHS CODE OF PRACTICE FOR CONTRACTORS

 No worker below 18 years and above 60 years of age shall be engaged for a job.
 All workers shall be screened (Basic EHS Induction Training) before engaging them
on the job. Physical fitness of certain workers, assessed by medical practitioner shall
be submitted. The final decision rests with the project management to reject any
person on the ground of physical fitness.
 Daily tool Box Talk is the mandatory requirement of Sandvik, it must be practiced
religiously.
 Usage of mobile phones is strictly prohibited during height work. Supervisor shall
collect all mobile phones at the time of height work.
 contractors shall ensure adequate supervision at workplace. They shall ensure that
all persons working under them shall not create any hazard to self or co – workers.
 Nobody is allowed to work without wearing Safety helmet with chin strap and Safety
Shoes. Other PPE’s as appropriate shall be used by all workers while at work.
 Workers should wear full body harness with double lanyard for works above 2mts.
height. Full body harness should be anchored to a rigid structure wherever possible.
Where there is no anchorage point, contractor shall arrange proper life line (8mm
wire rope or 20/25mm PP rope) for fastening the lanyard. The employee should hook
his full body harness once he reaches the work spot.
 Adequate illumination at workplace shall be ensured before starting the job at late
hours/night (if applicable).
 All the dangerous moving parts of the portable / fixed machinery being used shall be
adequately guarded.
 Ladders being used at site shall be rigid in condition and shall be adequately secured
at bottom and top. Ladder shall not be used as work platforms. Wooden Ladders &
Make-shift ladders are not allowed. Only aluminum or MS ladder is allowed.
 Horseplay is strictly prohibited and also running at workplace except in case of
emergency.
 Material shall not be thrown from the height & it should only be lowered through
proper arrangements. If required the area shall be barricaded and one person shall
be posted outside the barricaded area for preventing the trespassers from entering
the area.
 Other than the electricians, no one is allowed to carry out electrical connections,
repairs on electrical equipment or other jobs related thereto.
 Power supply to any equipment shall be routed through Residual Current Circuit
Breakers (RCCB/ ELCB) of 30mA sensitivity.
 Only sheathed flexible (Double Insulated) cable to be used for area lighting and all
the electrical equipment & power generator to be given two distinct earthlings.

Inserting of bare wire for tapping the power from electrical socket is completely
prohibited & Metal Clad Plug Tops must be used.
 Ensure all scaffoldings / work platforms shall be strong enough to take the expected
load. Use of Bamboo materials for scaffolding is strictly prohibited at our site. The
width of the working platform and fall protection arrangements shall be maintained as
per the site engineer’s recommendation. Approach ladder (MS or Aluminum) is to be
provided for the entire working platform.
 All tools and tackles shall be inspected before every use. Defective tools are to be
removed from service immediately. No lifting equipment / lifting tackle shall be used
unless it is Tested / inspected and approved by Sandvik. Lifting equipment /tools and
tackles should be supported by valid certificates issued by the Competent Person
and a copy of certificate shall be submitted to Site Manager/Site EHS Officer.
 Good housekeeping must be maintained at area of work in such a way that
Passages shall not be blocked with materials. Materials shall not be stacked on the
landing area of the staircases or by blocking the passage to firefighting equipment’s/
fire hydrants.
 Welding Cable and electrode holders must be fully insulated and return lead shall be
connected to the job/ work piece. Power cables must be safely routed either below
ground or above ground without affecting the safe passage. Cable markers shall be
provided for underground cables.
 Debris, scrap and other materials must be cleared time to time from the workplace
and at the end of work day all the debris and waste must be disposed at designated
places.
 All the unsafe conditions, unsafe acts and Near Miss Incidents identified by
contractors shall be reported to site Engineers and / or Site EHS Officer & it must be
corrected on priority basis.
 Contractor should follow Sandvik specified EHS Standards & specification for the
procurement of any EHS material. Repeated noncompliance report raised by Site
EHS Officer may result in a violation memo (Penalty).
 Workers deployed for height work job shall be specially screened (Induction) and
only selected persons who are medically fit, experienced and EHS inducted shall be
deployed at site. Workers who have acrophobia are proscribed from going to height
works.
 Proper access and working platform shall be provided to reach the work spot.
 Workers shall carry tools in a bag or in a box within the site premises & no tools shall
be taken out of the site unless otherwise required for repair with prior permission.
 Welder should wear welding shield for face and eye protection with safety helmet
while doing welding work. Welding electrode bit rods shall be collected in a separate
box and disposed through an authorized vendor.
 Welding machines must be provided with power cutoff switches with over load
tripping device (RCCB).
 Welding cable joints must be securely made without any loose joints. Welding cable
connectors or lugs shall be used to ensure the secured connections. Joints made of
lugs shall be properly insulated.

 Electrode holder shall be of good quality with proper insulation .The welder should
never be allowed to weld when he is wet. Usage of proper personal protective
equipment’s shall also be ensured.
 During rainy season, welding / grinding work shall not be carried out in an open
atmosphere.
 Combustible and inflammable materials shall be removed from the place where
welding is to be done. Oil or Paint Drums (empty or with contents) shall never be
kept in the vicinity of the welding area. Gunny Bags are not allowed to use in the hot
work area.
 Welding cables shall not be laid along with other electrical cables as; they may
damage insulation of other cables.
 Work permit system shall be followed as per the requirement of the job.
 Allow the workmen to attend safety meetings, EHS trainings and awareness
programs as and when required.
 Incidents which may cause environmental damage must be reported to Site EHS
Officer.
 Drip trays must be used to avoid the oil spillage on the ground.
 Clean drinking water & toilet facilities shall be provided to all workmen.
 Dust suppression measures shall be taken adequately as per the site Safe Work
Procedure.
 MSDS shall be maintained for all the chemicals entering into the site and Site EHS
Officer must be informed about the storage of such chemicals. The hazards and
environmental impact of such chemical shall be communicated to all workmen.
 Ensure workers responding to mock drills conducted at site.
Under taking:
I have understood all the above mentioned EHS code of practices and abide to follow the
same without fail. I hereby authorize Sandvik to initiate any disciplinary action in case of any
failure from our end.

 Pro-active hazard identification and reporting;
 Planned Job Observations;
 TAKE 5 Card, Re-Start Card use and Supervisor review;
 Lost Time Injuries (includes Lost Work Day, Severity Rate and Restricted Work Day
injuries);
 All Injury Frequency Rates (Lost Time Injuries and Medical Treatment Cases)
 Percentage of compliance based audit results.

Lagging/Leading Indicators
Lagging indicators: such as injury statistics, are important measures but generally reflect
the results of past actions. There is often a time lag between action taken by an
organization to improve performance and any change in actual performance. Lagging
indicators may hide potential risks. For example, having a low incidence of LTI does not
necessarily mean that adequate safety systems and controls are in place. Examples of
Lagging Indicators currently used by Sandvik:
· LTIFR
· TRIFR
· Workers Compensation costs
· Number of days lost
Leading indicators: are measures of proactive safety activities that are intended to reduce
the risk of injury, damage or loss. When used effectively, leading indicators can be used as
a gauge of good practice by measuring how well arrangements for workplace health and
safety are performing. Examples of Leading Indicators currently used by Sandvik:
· Observations/audits/inspections conducted
· Percentage trained in Risk Assessments
· Number of near misses / hazards reported
· Percentage of significant incidents reviewed and closed out
· Percentage of hazards rectified

  • 3. EHS Plan, JSW Jaigarh Page 3 of 36 1. PURPOSE This Environmental Health and Safety Plan (EHSP) have been written specifically for the JSW Jaigarh Conveyor Project. This document defines the key areas affecting the health and safety of Sandvik Asia Pvt Limited (MISY) employees and documents the safety management system that will be implemented to effectively eliminate or control workplace hazards and their associated risks. This EHSP is based on the SAPL (MiSy) corporate safety philosophy of “Zero Harm” and our commitment to ensure a safe work environment for all project employees, contractors and visitors. This EHSP outlines the framework that SAPL (MiSy) will use to conduct our activities whilst on the JSW Jaigarh conveyor Project site. OBJECTIVES & TARGETS SAPL (MiSy) understands that measurable criteria to evaluate safety performance need to be implemented. Accordingly, SAPL (MISY)’s site performance will be measured against the following criteria:  Workplace Injuries (Lost Time Injuries, Restricted Work Injuries, Medical Treatment Injury, First Aid, Minor Injury);  Hazards and Near-Miss (Significant Potential) Incidents and corrective actions;  Environmental Incidents;  Attendance at PROJECT Safety and / or Toolbox meetings;  Inspections and Observations undertaken; The SAPL (MiSy) Site Manager shall ensure a monthly report is generated and submitted in accordance with SAPL (MISY)’s monthly reporting requirements. It also have objective of:  Meeting Contractual agreements.  To determine broad parameters of EHS management at site.  Establish & define line of command for resolution of all hazard prevention issues.  To ensure compliance with relevant applicable legislation.  Continual EHS performance improvement by directing focus on the key areas for improvement in consistent manner.

LEADERSHIP & COMMITMENT SAPL (MiSy) recognises that JSW will hold the SAPL (MiSy) Project Manager accountable for the actions of all SAPL (MiSy) employees, contractors and visitors. SAPL (MiSy) accepts there will be a share accountability and responsibility role for providing: • Safe places of work; • Safe systems of work; • Safe plant and equipment; • Safe & Environmentally compliant work practices and procedures; • Adequate supervision; and • Suitable training for SAPL (MiSy) personnel. Accordingly, The SAPL (MiSy) Project Manager will be responsible for ensuring all SAPL (MiSy) employees, contractors and visitors are familiar and fully comply with the requirements of the EHSP and JSW site health, safety and environmental practises and procedures. Individual EHS roles and responsibilities, along with task and target shall be distributed to the individuals for action, as described below. 6.1 Project Manager NO. GENERAL EHS GUIDELINES, RULES & REGULATIONS This section shall detail the general EHS guidelines, rules & regulations to be adopted in the site for the following areas. PPE (Personal Protective Equipment’s) Head Protection:- All personals engaged for this job has to wear Safety Helmet along with Chin Strap. Hand Protection:- Appropriate Hand Gloves will be used for Materials handling, Concreting, Welding, Grinding, Gas cutting, for Chemical Handling & Electrical work. Eye Protection:- Suitable goggles must be ensured for the personnel deployed for Welding (Face Shield) , Grinding , Gas cutting , Concreting , Chipping etc. Work at site. Ear Protection:- The personnel engaged in the noise zone such as Compressor Operator, Pneumatic Vibrators & Breakers, DG Operator etc. will be equipped with suitable ear protection. Protection from Dust & other poisonous gases:- Suitable nose mask, face shield are to be used by the individuals to protect them from dust, fumes & poisonous and toxic gases. Work at Height & Fall Protection:- All personnel working over 2.0 meter height must have the safety belt tied in his waste at the ground level. Further he will have to anchor the safety belt with a fixed structure before starting the work at height. Some special type of Safety Belt like Fall Arrest Devices also will be in used according to the nature & area of work. Safety net and life line system will be adopted to protect the fall of man and material. Access, Egress & Workstation Safety:- Proper access to the work place will be provided by suitable means before engaging workmen inside the work area. The accesses must be suitable, stronger & firm
  • 17. EHS Plan, JSW Jaigarh Page 17 of 36 enough as per the standard norm. The stacking of materials on the access points should be avoided so as to facilitate the workmen for free movement. The housekeeping & illumination on the accesses will be also ensured. The work area will be made clear from unwanted materials. Suitable platforms are to be provided before allowing any workmen to work at height. The housekeeping & illumination of the work area will also be ensured in the work area. The elevated platforms are to be provided with adequate supports so as to ensure the stability of the working platforms. The openings & cut outs are to be strictly avoided in the work area or working platforms to avoid fall of person & materials from Height. Electrical Safety:- Electrical distribution system plan & sketch of DB’s & cable laying is to be prepared. The cables for power supply must be routed through underground with cable indicators so as to locate & maintain the same will be convenient. If the cable is to be routed above ground, make sure it is not lying on ground instead wooden/bamboo poles shall be used for cable laying. Distribution Boards are to be inspected regularly by the concern person (Electrician) at least once in a week. The megger values of all cables are to be checked in a regular interval to avoid electrical hazards. All the power supply to site for portable tools must be routed through ELCB / RCCB. All the installations, DB’s, SDB‘s at site are to be earthed as per specification. Log out tag out (LOTO) system to be followed at site. All power driven hand tools are to be inspected jointly by safety personal and electrician at regular intervals. Hot work (Welding & Gas cutting) and Work Permit System:- While hot work ensures the following points:  All welders and cutters have adequate training.  All welders and cutters are trained in correct use of PPE.  Adequate warning signs displayed.

EHS Plan, JSW Jaigarh Page 18 of 36  All welding machines are inspected, properly earthed and ELCB/RCCB installed.  Welding and gas cutting DO’s and DON’ts are to be instructed through daily tool box / Pep talk. The following work permit systems for the site are to be followed:  Hot work & Height Work. Use, Maintenance and Inspection of Plant, Machinery & Vehicle:- All the tools, tackles & portable machinery are to be thoroughly inspected & checked before issuing them for site use. Further the maintenance of all such machinery are to be conducted regularly in a proper interval (once in a week) to ensure better utilization of the same at site. The maintenance of plant & machinery like Cranes, Hydra etc. are to be conducted properly (once in a week). However the inspection for the same equipment will also carried out jointly by EHS & Operator once in a month to ensure the safe use of the same in construction activities. The tools & tackles are to be thoroughly checked by the concern supervisor for its capability & suitability before deployment of the same at site. Maximum speed limit at site should not be more than15 KM/Hour while driving. Scaffold & Work Platforms:- All steel scaffolds must satisfy the specified standards. These should be checked before erection of the same at site. During erection of the same the standard norms for scaffoldings are to be followed at site. Scaffold should not be tampered by any physical agents. These are to be maintained properly in regular intervals to ensure safety of the Personnel working over it & for its longer life. These are to be handled properly at site. Throwing them from height is to be restricted as there are chances of developing cracks & getting damage. The Shifting & placement of the same at site are to be carried out by the supervision of an Engineer / Supervisor so as to ensure the safe use of the same at site. Adequate supports for scaffold are to be given at site
  • 19. EHS Plan, JSW Jaigarh Page 19 of 36 for their stability. Site EHS Officer will be authorized to certify the safe/unsafe condition of scaffold prior to use. Upon inspection green/red tag shall be installed to identify the condition of scaffold. Erection & Rigging:-  There must be a fixed system of signaling, clearly understood by crane operator  All cranes, lifting tools and tackles are to be tested and certified by a competent person.  Do not leave loads suspended.  Do not drop freely, snatch, lift or shock load.  Do not swing loads into position, they must be straight lifted.  Temporary ladders to cabins must be lashed.  Test the load before making the full lift, never exceed the safe working load of the crane and avoid over winding.  Make sure that it is safe to commence lifting and that all personnel are clear of the load. Mobile Crane Operation:-  Before starting, make sure that all controls are in neutral.  Ensure that the safe load indicator is working satisfactorily.  Check all round visibility and note any particular hazards such as overhead power lines.  Make sure that your emergency escape system is in working order.  Ensure that youth hook is clear of obstructions before you move it.  Hoist, lower and traverse smoothly to prevent the load swinging.  Make sure that the fire extinguisher is in proper place and that the operator knows how to use it.

Lifting Tools/Tackles Safety (Chain Slings, Wire Rope Slings etc.):-  Never overload any lifting tackle.  Use the lifting tackles properly on the load.  Do not use damaged tackles.  Do not use any tackle without safe working load marked on it. Critical lift plan, Handling & Storage of material:-  Before commencing any critical erection, a lift plan is to be prepared by concern engineer.  Proper storage of all materials is to be ensured and safe material handling procedure is to be instructed by concern engineer / site EHS Officer. Housekeeping:- Housekeeping on site is daily/continual job and is to be based on the following steps:  Designate / demarcate place for each type of material (scrap, Hazardous waste, Non Hazardous Waste etc)  Plan access in such a way that each place is easily accessible.  Clean up work place upon completion of work. Firefighting & prevention:- Portable fire extinguishers shall be provided for firefighting. Quantity and type shall be determined as per site requirement. At least 10 percent of total workforce should be covered in basic firefighting training.

Environment Protection/Waste Management:- The SAPL (MiSy) Site Manager shall ensure all environmental aspects and impacts that affect SAPL (MiSy) operations on site are identified and included within the SAPL (MiSy) and site risk register. This will highlight the risk and potential significance of the particular aspect and list the control measures to minimize the potential risk. All work activities shall be carried out in such a way to both manage and reduce waste, and to prevent or minimize contamination to land, soil and ground water. Site Managers shall ensure all waste is managed according to the Waste Management procedures. The materials shall be disposed of appropriately and corrective action shall be undertaken. Engineering & Design Studies:- The SAPL (MiSy) Site Manager shall ensure engineering and design studies are conducted, when required to identify hazards to people, property and the environment that may arise during construction, commissioning, and operation and maintenance of plant. These studies shall be translated into design, construction methods that eliminate, reduce and control, far as practicable, hazardous tasks. The initial actions of an individual can play an important part in coping with the incident to minimize injury and damage. To this end, it is very important that the individual knows what to do and how to do it. What to do in case of fire: 1. Sound the alarm or otherwise inform others by shouting ‘fire, fire‘. Who may be in danger, call ___________________ for help. 2. With the minimum personal risk, attack the fire with the fire extinguishers provided nearby, using them as instructed. Or 1. There are people working with you who are fully trained on ‘first aid’ and ‘fire fighting’. Call on __________________ for assistance. 2. When transmitting a message clearly identify yourself by giving:  Your name & your exact location  Nature of emergency  Service required and repeat the message  Arrange to remove all the inflammable materials away from the fire area if possible.  In the event that the fire cannot be extinguished or controlled: a. Move away from the area of danger to the safe area (assembly point). b. Report to Site EHS Officer/Supervisor at once on your safe arrival to assembly point for head count. c. Do not run, avoid rumours. Be calm and quiet until you get further instructions. What to do in case of medical emergency: 1. Do not try to move an injured person if you are not fully trained on first aid. 2. Call on ___________________ to get necessary medical help. 3. The scene of an incident must be left undisturbed. 4. In some case the incident site must be rendered safe so as not to contribute to further incidents. Hence cordon off the area and do not allow anyone to enter. 5. Retain the eyewitnesses of the incident until the investigation team has arrived to the spot. Note: One dedicated emergency vehicle shall be available at site during work hours. TRAINING, COMMUNICATION & REPORTING EHS Induction:- SAPL (MiSy) shall ensure that all visitors, Staff & workmen on site undertake a SAPL (MiSy) Site Specific EHS Induction before entering into the site/commencing work. Daily Tool Box Talk:- SAPL (MiSy) shall ensure that all workmen on site is attending the tool box talk given by supervisors in the morning prior to start the work. Task-Specific EHS Training:- SAPL (MiSy) shall ensure task-specific EHS training is provided to all SAPL (MiSy) workers and Sub-Contractors as required. Training may include topics such as Risk Assessment Team (RATS), Incident Investigation, First Aid, Fire Fighting, Confined Space Entry, Working at Heights, Defensive Driving, Electrical Safety, Welding/Cutting Safety, etc. SAPL (MiSy) shall ensure that appropriate training material is developed for the level and scope of the work and activities undertaken by SAPL (MISY). A Site Training Matrix is developed and forwarded to the Project Manager/EHS Manager for future reference. Ensure all training undertaken on site is undertaken and recorded in accordance with SAPL (MiSy) Policy. Cascading any EHS messages down the line is vital for the success of any EHS Management System and to ensure that all personnel are aware of EHS issues the following technique shall be adopted.

EHS MANAGEMENT OF CONTRACTORS Assessment:- SAPL (MiSy) Contractors shall be evaluated to identify those who can demonstrate safety performance that will complement our “Zero Harm” philosophy. The SAPL (MiSy) Site Manager shall formally evaluate and assess the performance of each contractor in accordance with SAPL (MiSy) Policy Standard: Contractor Management. The evaluation shall consist of past performance and interviews with key personnel and site inspections as necessary. Kick-Off Meetings:- SAPL (MiSy) shall conduct a contractor “Kick-Off” meeting with all SAPL (MiSy) contractors. All safety matters pertaining to the particular contractor’s scope of work will be reconfirmed at this meeting. Risk assessments shall be conducted with contractor as part of the kick-off process in order to identify generic hazards and the risks associated with their scope of work. Documentation relating to these “Kick-Off” meetings shall be retained on the SAPL (MiSy) site specific EHS directory. EHS Plan for contractor:- A SAPL (MiSy) contractor is liable to follow this EHS Plan. Contractor’s EHS performance shall be reviewed by project manager/site manager/ehs manager regularly. Weekly Contractor Co-ordination Meetings:- The SAPL (MiSy) Site Manager and Site EHS Officer shall attend weekly Contractor Coordination Meeting to plan activities and co-ordinate interfaces with other contractors.

Action plans will be agreed and documented to correct areas of underperformance. These hazard logs and action plans shall be retained on the SAPL (MiSy) site specific EHS directory. Contractor EHS Officer/Steward:- SAPL (MiSy) shall ensure a full-time EHS Officer/Steward whenever twenty (20) or more personnel are engaged in, or conducting manual work per each Contractors contract scope. Resumes of proposed Contractor EHS Officer/Steward shall be submitted to the Project Manager/EHS Manager for approval and, where necessary, interviews conducted prior to mobilization. The SAPL (MiSy) Project Manager shall ensure the SAPL (MiSy) Site EHS Officer and Contractor EHS Officer/Steward attends the Weekly meeting to coordinate construction and operations activities and develop and implement incident prevention and safety improvement strategies. Minutes and actions arising from this meeting shall be retained on the SAPL (MiSy) site specific EHS directory.

EHS Plan, JSW Jaigarh Page 31 of 36 Annexure 1:- EHS CODE OF PRACTICE FOR CONTRACTORS Name of Project:____________________________ Date:___________  No worker below 18 years and above 60 years of age shall be engaged for a job.  All workers shall be screened (Basic EHS Induction Training) before engaging them on the job. Physical fitness of certain workers, assessed by medical practitioner shall be submitted. The final decision rests with the project management to reject any person on the ground of physical fitness.  Daily tool Box Talk is the mandatory requirement of Sandvik, it must be practiced religiously.  Usage of mobile phones is strictly prohibited during height work. Supervisor shall collect all mobile phones at the time of height work.  contractors shall ensure adequate supervision at workplace. They shall ensure that all persons working under them shall not create any hazard to self or co – workers.  Nobody is allowed to work without wearing Safety helmet with chin strap and Safety Shoes. Other PPE’s as appropriate shall be used by all workers while at work.  Workers should wear full body harness with double lanyard for works above 2mts. height. Full body harness should be anchored to a rigid structure wherever possible. Where there is no anchorage point, contractor shall arrange proper life line (8mm wire rope or 20/25mm PP rope) for fastening the lanyard. The employee should hook his full body harness once he reaches the work spot.  Adequate illumination at workplace shall be ensured before starting the job at late hours/night (if applicable).  All the dangerous moving parts of the portable / fixed machinery being used shall be adequately guarded.  Ladders being used at site shall be rigid in condition and shall be adequately secured at bottom and top. Ladder shall not be used as work platforms. Wooden Ladders & Make-shift ladders are not allowed. Only aluminum or MS ladder is allowed.  Horseplay is strictly prohibited and also running at workplace except in case of emergency.  Material shall not be thrown from the height & it should only be lowered through proper arrangements. If required the area shall be barricaded and one person shall be posted outside the barricaded area for preventing the trespassers from entering the area.  Other than the electricians, no one is allowed to carry out electrical connections, repairs on electrical equipment or other jobs related thereto.  Power supply to any equipment shall be routed through Residual Current Circuit Breakers (RCCB/ ELCB) of 30mA sensitivity.  Only sheathed flexible (Double Insulated) cable to be used for area lighting and all the electrical equipment & power generator to be given two distinct earthlings.

EHS Plan, JSW Jaigarh Page 32 of 36 Inserting of bare wire for tapping the power from electrical socket is completely prohibited & Metal Clad Plug Tops must be used.  Ensure all scaffoldings / work platforms shall be strong enough to take the expected load. Use of Bamboo materials for scaffolding is strictly prohibited at our site. The width of the working platform and fall protection arrangements shall be maintained as per the site engineer’s recommendation. Approach ladder (MS or Aluminum) is to be provided for the entire working platform.  All tools and tackles shall be inspected before every use. Defective tools are to be removed from service immediately. No lifting equipment / lifting tackle shall be used unless it is Tested / inspected and approved by Sandvik. Lifting equipment /tools and tackles should be supported by valid certificates issued by the Competent Person and a copy of certificate shall be submitted to Site Manager/Site EHS Officer.  Good housekeeping must be maintained at area of work in such a way that Passages shall not be blocked with materials. Materials shall not be stacked on the landing area of the staircases or by blocking the passage to firefighting equipment’s/ fire hydrants.  Welding Cable and electrode holders must be fully insulated and return lead shall be connected to the job/ work piece. Power cables must be safely routed either below ground or above ground without affecting the safe passage. Cable markers shall be provided for underground cables.  Debris, scrap and other materials must be cleared time to time from the workplace and at the end of work day all the debris and waste must be disposed at designated places.  All the unsafe conditions, unsafe acts and Near Miss Incidents identified by contractors shall be reported to site Engineers and / or Site EHS Officer & it must be corrected on priority basis.  Contractor should follow Sandvik specified EHS Standards & specification for the procurement of any EHS material. Repeated noncompliance report raised by Site EHS Officer may result in a violation memo (Penalty).  Workers deployed for height work job shall be specially screened (Induction) and only selected persons who are medically fit, experienced and EHS inducted shall be deployed at site. Workers who have acrophobia are proscribed from going to height works.  Proper access and working platform shall be provided to reach the work spot.  Workers shall carry tools in a bag or in a box within the site premises & no tools shall be taken out of the site unless otherwise required for repair with prior permission.  Welder should wear welding shield for face and eye protection with safety helmet while doing welding work. Welding electrode bit rods shall be collected in a separate box and disposed through an authorized vendor.  Welding machines must be provided with power cutoff switches with over load tripping device (RCCB).  Welding cable joints must be securely made without any loose joints. Welding cable connectors or lugs shall be used to ensure the secured connections. Joints made of lugs shall be properly insulated.
  • 33. EHS Plan, JSW Jaigarh Page 33 of 36  Electrode holder shall be of good quality with proper insulation .The welder should never be allowed to weld when he is wet. Usage of proper personal protective equipment’s shall also be ensured.  During rainy season, welding / grinding work shall not be carried out in an open atmosphere.  Combustible and inflammable materials shall be removed from the place where welding is to be done. Oil or Paint Drums (empty or with contents) shall never be kept in the vicinity of the welding area. Gunny Bags are not allowed to use in the hot work area.  Welding cables shall not be laid along with other electrical cables as; they may damage insulation of other cables.  Work permit system shall be followed as per the requirement of the job.  Allow the workmen to attend safety meetings, EHS trainings and awareness programs as and when required.  Incidents which may cause environmental damage must be reported to Site EHS Officer.  Drip trays must be used to avoid the oil spillage on the ground.  Clean drinking water & toilet facilities shall be provided to all workmen.  Dust suppression measures shall be taken adequately as per the site Safe Work Procedure.  MSDS shall be maintained for all the chemicals entering into the site and Site EHS Officer must be informed about the storage of such chemicals. The hazards and environmental impact of such chemical shall be communicated to all workmen.  Ensure workers responding to mock drills conducted at site. Under taking: I have understood all the above mentioned EHS code of practices and abide to follow the same without fail. There is often a time lag between action taken by an organization to improve performance and any change in actual performance. Lagging indicators may hide potential risks. For example, having a low incidence of LTI does not necessarily mean that adequate safety systems and controls are in place. Examples of Lagging Indicators currently used by Sandvik: · LTIFR · TRIFR · Workers Compensation costs · Number of days lost Leading indicators: are measures of proactive safety activities that are intended to reduce the risk of injury, damage or loss. When used effectively, leading indicators can be used as a gauge of good practice by measuring how well arrangements for workplace health and safety are performing.
