A Question Well Put 51歳女性 左大腿部痛 N Engl Med 2009;360:1446-51
これまで • 慢性下痢、腹痛:ガストリノーマ • 難治性気管支喘息:好酸球性多発血管炎性肉芽腫症 • 高Ca血症、貧血、腎障害、骨病変:サルコイドーシス • 疲労感、遂行障害：PRES • 不明熱：血管内リンパ腫 • 頭痛、発熱後の発疹：発疹チフス • 肺胞出血、糸球体腎炎：Goodpasture’s症候群 • MVP、頭痛：IE • 慢性下痢：VIPoma • 出血傾向：多発性骨髄腫による後天性von Willebrand 病 • 腹部膨満感：好酸球性胃腸症 • 妊婦の突然発症の両側頭部頭痛と息切れ：褐色細胞腫 • 難治性VT：心サルコイドーシス • 突然の霞目、複視：好酸球増多症候群に伴う心内膜炎 • ４週間続く倦怠感：強皮症腎クリーぜ • 全身浮腫：心アミロイドーシス • 心不全、CK上昇：脳挫傷後汎下垂体機能低下症 • 口腔内乾燥、頻尿：乳癌脳転移による尿崩症
51歳女性 左大腿部痛 既往：高血圧、うつ病 過去数カ月で徐々に増悪する左大腿部痛があり、過去１週間で歩 行困難となった 背下部、上腕、胸部の虚弱と痛みもある 体重減少、食思不振、外傷、発熱なし 6ヶ月前に左大腿の“疲労骨折”に対する手術を受けた 左大腿部の労作時痛、上腕といくつかの肋骨、下部脊椎の圧痛が ある 筋肉の可動域やMMTは正常
51歳女性 左大腿部痛 4年前に子宮筋腫に対する子宮摘出術（卵巣温存）を受けた 6ヶ月前に左大腿骨の非外傷性骨折に対して髄内釘を挿入された 術中の病理所見は良性 術中に骨減少症がわかり、Tスコアは大腿骨頸部で−2.1、脊椎で −2.1 血算、血清・尿の電気泳動、Ca、PTH、Cr、甲状腺機能は正常 活性化vitDは16 術後vitD製剤を1ヶ月投与され、リハビリも受けた 術後回復は良好で歩行も可能となったが、痛みは持続し徐々に増悪し てきた
Tスコア 若者の骨密度を0として 正常 Tスコアが－1SD以上 骨減少症 Tスコアが－1 ～ －2.5SD 骨粗鬆症 Tスコアが－2.5 以下
Ca、Pと周辺ホルモン PTH：Ca↑ P↓ 骨からCaを遊離させる 尿細管でCa再吸収、P排泄 腎臓でvitDを活性化 活性型vitD3：Ca↑ P↑ 小腸からのCa、P吸収促進 骨代謝を高める 尿細管のPTH作用増強（Ca↑） カルシトニン：Ca↓ P↓ 骨吸収を抑制しCa↓ 病気がみえるより引用
ビタミンDの代謝
51歳女性 左大腿部痛 下痢、腹痛、黄疸なし レントゲンでは骨減少症と複数の肋骨骨折、左大腿骨と恥骨に横 断性の溶骨変化 放射性核種を用いた骨のスキャンではこれらすべての領域で取り 込みが増加していた CBC、尿、血液・尿の免疫電気泳動、電解質、Ca、PTH、 PTHrPは正常。 25（OH）vitDは54、ALP193、肝機能正常、γGT正常
51歳女性 左大腿部痛 胸腹部骨盤部CTと両側のマンモグラフィーが撮影されたが、腫 瘍は認められなかった 過去の骨生検を見直しても病的な所見はなかったが、テトラサイ クリンのラベリングによる組織形態計測は行われなかった（骨代 謝を測るものらしい）
51歳女性 左大腿部痛 血中P1.1mg、1,25-(OH)2vitDは15 患者には13人の兄弟がいるが骨の病気や病的骨折の家族歴なし 6ヶ月前の血中P1.0 蓄尿では尿中P972mg（＞100であり腎のP排泄多い）（測定時の血中 P1.3）、Ca39mg、Cr1260mg、血中Crは0.4 FEP=（尿中P x 血中Cr）/（血中P x 尿中Cr）＝0.23 （＞0.05であり腎のP排泄多い） UA、HCO3正常 尿糖陰性 鑑別は？検査は？
51歳女性 左大腿部痛 vitD欠乏なく、副甲状腺機能亢進もなく、FEP＝23% →原発性腎尿細管障害によるリン代謝異常 多いのはFanconi症候群 多くは遺伝性で、後天性では骨髄腫やシェーグレン、重金属中毒 によって起こる
Fanconi症候群 近位尿細管機能の広範な障害により多彩な症状を呈する症候群 ブドウ糖再吸収↓：腎性糖尿 アミノ酸再吸収↓：汎アミノ酸尿 P再吸収↓：高P尿、低P血症 Ca再吸収↓：低Ca血症 K再吸収↓：低K血症 HCO3再吸収↓：代謝性アシドーシス 尿酸再吸収↓：低尿酸血症 イヤーノートより
51歳女性 左大腿部痛 FGF-23がオーダーされた 過去のCTでは肋骨、骨盤、脊椎の多数の骨折があり、仮骨形成 あり 明らかな腫瘍はなし
FGF23（fibroblast growth factor） 腎尿細管でのリン再吸収の抑制作用 活性型ビタミンD産生低下による腸管からのリン吸収抑制作用 により血中Pを低下させる イヤーノートより
51歳女性 左大腿部痛 FGF-23は418と上昇していた オクトレオチドスキャンでは胸壁の右後部で取り込みが上昇して いた CTでは右第７肋骨の後部に12mmの円形の高吸収域あり、過去 には骨折の仮骨形成が見られていた部位であった
51歳女性 左大腿部痛 肋骨の腫瘍が切除され、血管外皮腫のような腫瘍が認められた。 多分良性 FGF-23染色はされなかったが、術後に血中FGF-23は下がった 他の治療はされなかったが、血中P、尿中Pは正常化した その後特に骨折なく経過し、元気に歩けている
診断：腫瘍性骨軟化症 腫瘍が大量のFGF23を産生し、低P血症による骨軟化症 をきたす。原因となる腫瘍は間葉系の良性腫瘍で小さい ことが多く、発見が難しい。 腫瘍を外科的に摘出すれば完治する イヤーノートより 51歳女性 左大腿部痛
病的骨折鑑別 腫瘍性： 固形癌の転移 血液癌（多発性骨髄腫） 代謝性骨疾患： 骨粗鬆症 骨軟化症 パジェット病 副甲状腺機能亢進症による嚢胞性線維性骨炎
くる病・骨軟化症 種々の原因により骨の石灰化不全がおこり、骨の強度が低下した 状態。骨端線閉鎖前の小児期に発症するものをくる病という。 ビタミンDの作用不足 or 低P血症 ビタミンD作用不足：欠乏、活性化障害、受容体障害 低P血症：尿細管障害、FGF23作用過剰
A Question Well Put 上手な問題設定 腫瘍性骨軟化症を知らなくても、 「成人発症の、ビタミンD抵抗性の、腎性P喪失、PTHとビタミ ンDが正常のものは？」 と問題設定する苦労を惜しまないようにしよう Schon “name the things to which we will attend and frame the context in which we will attend to them” 物事に名前をつけ、状況に枠組みを当てはめて問題を設定しよう
