Editor's Notes
Any problem in an ABC area should be dealt with before moving on to the next area
e.g. a closed airway should be opened before breathing is assessed
Antibiotics may already have been given but if not, meningitis is a default diagnosis, to be considered for treatment in the absence of a clear alternative diagnosis.
A lumbar puncture should be done unless there are contraindications (see later slide). In fact, if the child presents with a decreased conscious level, there are almost certainly contraindications to lumbar puncture
Many causes of decreased conscious level are impossible to identify in the first hour. Indeed, many may not be identified in the first 24hrs
The APLS approach is to ensure the child’s ABCD stability and to “treat the treatable”- more common conditions which, if correctly treated in the first hour will bring about a good outcome

If hypoglycaemia is present, it is most useful to take 10mls or so of blood before giving the dextrose treatment so that levels of diagnostic metabolites can be assayed later
Send it to the lab for separation and appropriate storage.
Use carbon dioxide capnometry to keep ventilation controlled
Follow in text if difficult to see on screen.
This is a re-cap of the approach to the child with a decreased conscious level. First ABC assessment and intervention, then identify and treat the treatable plus manage RICP if present
This severe injury had major bleeding which was controlled with pressure

10 µg/kg is used but some children need doses as high as 100 µg/kg up to a maximum of 2 mg. Naloxone has a short half‑life, relapse often occurring after 20 minutes. Further boluses, or an infusion of 10‑20 µg/kg/min may be required.
It is important to normalise CO2 before the naloxone is given as adverse events such as ventricular arrhythmias, acute pulmonary oedema, asystole or seizures may otherwise occur. This is because the opioid system and adrenergic system are interrelated. Opioid antagonists and hypercapnia stimulate sympathetic nervous system activity. Therefore if ventilation is not provided to normalize carbon dioxide prior to naloxone administration, the sudden rise in epinephrine concentration can cause arrhythmias.

In the text there is an algorithm which shows the systematic approach to the child with a decreased conscious level. In a similar way this can be referred to at the conclusion of the lecture
