Stress that although the pathology may be in the circulation, its effects on conscious level may depress Airway and Breathing so they must be supported as necessary.
Although most cases of shock will require prompt fluid resuscitation, always check for heart failure by looking for marked hepatomegaly and look at the ECG to identify a treatable paroxysmal tachycardia (do a rhythm strip in an infant with a rate over 200bpm and a child with a rate over 150).
Give antibiotics to those in whom a diagnosis of septicaemia is made obvious by a purpuric rash, taking a blood culture, FBC. electrolytes and glucose first.

An artificial but helpful approximation
Fluid should be crystalloid.
Most cases of gastroenteritis should be improved significantly after 2 boluses. If they are not, then a “surgical abdomen” should be considered. Even with a good response to fluid a “surgical abdomen” should be considered in any child with bile-stained vomiting, blood/mucus in stools (intussusception), abdominal guarding and if the diarrhoea seems trivial for the degree of apparent fluid loss.
An abdominal film and an ultrasound will be helpful.
Consider antibiotics if more than two boluses of fluid are required

A high index of suspicion should exist for septicaemia. As well as for the child with obvious septicaemia, children with no other obvious cause for their shock should receive parenteral antibiotics after appropriate blood tests
The cardinal sign of meningococcal septicaemia is a purpuric rash in an ill child. At the onset, however, the rash is not florid and a careful search should be made for purpura in any unwell child. In about 13% of patients with meningococcal septicaemia, an initial blanching erythematous rash precedes or replaces a purpuric one, and in 7% of cases no rash occurs. In the much rarer toxic shock syndrome the initial clinical picture includes a high fever, headache, confusion, conjunctival and mucosal hyperaemia, scarlatiniform rash with secondary desquamation, subcutaneous oedema, vomiting and watery diarrhoea. Early administration of antibiotics, concurrent with initial resuscitation is vital.

In septicaemia it remains usual practice to give fluid as 4.5% human albumen.
Children in septic shock often require several boluses of fluid to achieve relative stability. Once the third bolus of fluid has been commenced, the patient should be intubated by rapid sequence induction of anaesthesia and ventilated. This is done both to support a seriously ill patient by maximising oxygenation and to anticipate the development of pulmonary oedema caused by fluid leak in the lungs. All intubated children must have continuous SaO2 and CO2 monitoring
At the same time it is likely that inotropes will be needed. Mention dobutamine, dopamine, epinephrine
In septic shock, myocardial depression is a co-existent feature. Therefore, at the same time as the third bolus of fluid is commenced an infusion of dobutamine should be started at an initial rate of 10 microgm/kg/minThis can be given through a peripheral vein as it is unlikely that central venous access will yet have been obtained. The rate of infusion should be adjusted to the patient’s response
In the child with meningococcal septicaemia and raised intracranial pressure fluid management is a real problem. The child requires sufficient fluid to maintain a good perfusion but too much fluid will add to the cerebral oedema. Respiratory support is required early. The child must be monitored and assessed frequently and early discussion with a PICU should occur.
Oxygen acts to close the duct but is needed to treat shock. Try different concentrations, monitoring with the pulse oximeter for the best for each infant. Once the duct has opened with the prostaglandin this should not be a problem
