Tóm lược: Hiệp hội tiêu hóa Nhật Bản đã sửa đổi hướng dẫn lâm sàng lần 3 về bệnh loét dạ dày – tá tràng năm 2020 và tạo một phiên bản tiếng Anh. Hướng dẫn được sửa đổi gồm 9 nội dung: dịch tễ học, xuất huyết dạ dày và tá tràng do loét, liệu pháp không diệt trừ, loét do thuốc, không nhiễm H. Pylori, và loét do NSAID, loét trên dạ dày còn lại, điều trị bằng phẫu thuật và điều trị bảo tồn cho thủng và hẹp. phương pháp điều trị khác nhau dựa trên biến chứng của loét. Ở bệnh nhân loét do NSAID, các thuốc NSAID được ngưng và sử dụng thuốc chống loét. Nếu NSAID không thể ngưng sử dụng, loét sẽ được điều trị bằng thuốc ức chế bơm proton. Vonoprazon và kháng sinh được khuyến cáo là lựa chọn hàng đầu cho diệt trừ HP, và PPIs hoặc Vonoprazan kết hợp kháng sinh được khuyến cáo là điều trị hàng thứ 2. Bệnh nhân không sử dụng NSAIDs và có Hp âm tính thì nghĩ đến loét dạ dày tá tràng tự phát. Chiến lược để dự phòng loét dạ dày tá tràng do NSAID và Aspirin liều thấp được trình bày trong hướng dẫn này. Cách thức điều trị khác nhau phụ thuộc vào việc đồng thời sử dụng NSAIDs hoặc Aspirin liều thấp với tiền sử loét hoặc xuất huyết tiêu hóa trước đây. Ở bệnh nhân có tiền sử loét có sử dụng NSAIDs, PPIs có hoặc không Celecoxib được khuyến cáo và sử dụng. Vonoprazon được đề nghị để dự phòng loét tái phát. Ở bệnh nhân có tiền sử loét có uống aspirin liều thấp, PPIs hoặc Vonoprazon được khuyến cáo và điều trị bằng kháng histamine H2 được đề nghị đề dự phòng loét tái phát.
Giới thiệu
Năm 2009, hiệp hội tiêu hóa Nhật Bản đã cho ra đời hướng dẫn thực hành lâm sàng dựa trên bằng chứng về bệnh loét dạ dày tá tràng. Hướng dẫn này được sửa đổi vào năm 2015 và lần nữa vào năm 2020. Trong số 90 câu hỏi trong hướng dẫn trước đó, có những câu hỏi có kết luận rõ ràng, và có những câu hỏi phải phụ thuộc vào kết quả của những nghiên cứu trong tương lai, chúng được giải đáp và sửa đổi trong hướng dẫn này. Vì thế, hướng dẫn sửa đổi này bao gồm 9 nội dung (28 câu hỏi lâm sàng và 1 câu hỏi giải đáp trong nghiên cứu gần đây), bao gồm, cũng là lần đầu tiên về dịch tễ học và ổ loét dạ dày – tá tràng còn tổn tại. Cả dịch tễ học và phương pháp điều trị bảo tồn cho thủng và hẹp trong các câu hỏi nền tảng. Dự phòng xuất huyết do loét dạ dày – tá tràng ở bệnh nhân uống thuốc kháng tiểu cầu và điều trị loét tá tràng do thiếu máu cục bộ đã được them vào câu hỏi lâm sàng và câu hỏi cần trả lời trong tương lai.
Tìm kiếm tài liệu trên thư viện Medline và Cochrane đã được thực hiện về tài liệu liên quan đến các câu hỏi lâm sàng đăng tải từ năm 1983 đến tháng 10 năm 2018, và cơ sở dữ liệu Igaku Chuo Zasshi được tìm kiếm về dữ liệu đăng tải từ 1983 đến tháng 10 năm 2018. Hướng dẫn này được phát triển sử dụng hệ thống thẩm định, phát triển và đánh giá khuyến cáo (GRADE). Chất lượng bằng chứng được chia thành các mức A (cao), B (trung bình), C (thấp) và D (rất thấp). Các thử nghiệm lâm sàng đã chứng minh, digoxin:
– Giúp giảm triệu chứng suy tim
– Cải thiện khả năng gắng sức
– Cải thiện huyết động (đặc biệt trong suy tim kèm rung nhĩ
– Giảm nhập viện do suy tim mất bù
– Nhưng không cải thiện sống còn.
Thu c: Digoxin, viên 0,25 mg, ng 0,5 mg, li u:ố ố ề
0,125-0,375 mg/ngày.
50. Tăng thải, giảm ứ trệ muối nước.
Cải thiện triệu chứng (phù, khó thở...), cải thiện khả
năng gắng sức.
Các nhóm thuốc lợi tiểu: lợi tiểu quai (Furosemide), lợi
tiểu thiazid (hypothiazid), lợi tiểu kháng aldosterone
(Spironolactone).
Cần phải bù Kali khi dùng các lợi tiểu thải Kali (lợi
tiểu quai, thiazide) hoặc phối hợp lợi tiểu thải Kali với
lợi tiểu giữ Kali (VD: Spiromide).

Welcome to INTRODUCTION TO RADIOFREQUENCY ABLATION THERAPY. This module contains an introduction to radiofrequency (RF) ablation therapy, reviews the current indications for RF ablation therapy, and discusses developing indications for RF ablation therapy.
This chart shows that males have a higher incidence of developing HF compared to females until age 65. At that time, females essentially equal males in the incidence of developing HF.
Stroke volume is affected by preload, afterload, and contractility.
Preload is the amount myocardial stretch at the end of diastole.
Afterload is the resistance that needs to be overcome for the heart to eject the blood. There is an inverse relationship between afterload and ventricular function. As the resistance to contraction increases, the force of contraction decreases which results in a decreased stroke volume. Also, as an increase in resistance occurs, there is an increase in myocardial oxygen demand.
Contractility is the inotropic state of the heart independent of the preload and the afterload.
Synergistic LV contraction, wall integrity, and the competence of the valves also affect cardiac output.
Digoxin
Digoxin has been used in the management of heart failure for more than 200 years, yet it wasn’t formally approved by the FDA for this indication until 1997.
Digoxin enhances inotropy (contractility) of cardiac muscle and, at the same time, reduces activation of the SNS and RAAS. These neurohormonal effects are sustained during prolonged treatment with digoxin.
Randomized, double-blind, placebo-controlled trials such as PROVED (Prospective Randomized Study of Ventricular Failure and the Efficacy of Digoxin) and RADIANCE (Randomized Assessment of Digoxin and Inhibitors of Angiotensin-Converting Enzyme) have shown that long-term therapy with digoxin reduces symptoms and increases exercise tolerance1. These two trials demonstrated that “patients with mild to moderate chronic heart failure due to left ventricular systolic dysfunction, who are clinically stable on either maintenance therapy of Digoxin and diuretics (PROVED), or with additional background therapy with ACE Inhibitors (RADIANCE), are at considerable risk for clinical deterioration if Digoxin is withdrawn.”2 Unfortunately, the Digoxin Investigation Group (DIG) Trial demonstrated that digoxin had no effect on mortality; however, digoxin did reduce the hospitalization rate for decompensated heart failure3 .
The ACC/AHA Guidelines support the use of digoxin in conjunction with diuretics, an ACE inhibitor, and a beta-blocker in patients with LV systolic dysfunction who remain symptomatic despite treatment with an ACE inhibitor and a beta-blocker, and in those in whom heart failure is accompanied by rapid atrial fibrillation.The usual digoxin dose is 0.125-0.25 mg per day, and should be adjusted for age, renal function, and body mass. The Guidelines note that although the adverse effects of digoxin, such as cardiac arrhythmias and gastrointestinal and neurologic complaints, occur primarily at high doses, these higher doses are usually not necessary to achieve clinical benefits in patients with heart failure.

Most patients with heart failure require a diuretic to relieve fluid retention. In addition to rapidly decreasing symptoms such as pulmonary congestion and peripheral edema, diuretics improve exercise tolerance and facilitate the use of other drugs indicated for heart failure.
Treatment with a diuretic is generally started at a low dose and then gradually tapered upward until a threshold dose is established. Some patients with heart failure can be taught to adjust their diuretic dose themselves based on changes in body weight, which should be monitored daily. After fluid retention has resolved, diuretic therapy is continued to prevent its recurrence.
Electrolyte depletion is a frequent complication of long-term diuretic therapy; therefore, electrolyte levels need to be monitored frequently during initial stages of therapy and after increases in diuretic dose.
Diuretics are usually used along with ACE inhibitors and beta-blockers in heart failure, and should never be used alone. Increased doses of diuretics have been associated with increased mortality.
Angiotensin Converting Enzyme (ACE) inhibitors are recommended for all heart failure patients, whether they are symptomatic or not. Use of ACE inhibitors relieves symptoms and improves exercise tolerance in patients with chronic heart failure. Data from placebo-controlled trials show that ACE inhibitors can also reduce the risk of death and disease progression in heart failure patients.
The benefits of ACE inhibitor therapy may not become apparent for 1-2 months after initiation of treatment. But even in the absence of symptomatic improvement, continued long-term ACE inhibitor therapy is recommended to reduce the risk of death or hospitalization.
Most patients with heart failure tolerate long-term ACE inhibitor therapy. Potential side effects include a decrease in blood pressure, transient worsening of kidney function, hyperkalemia, and chronic cough. Angioedema, a disorder characterized by the development of large, edematous areas of the skin, mucous membranes, and organs, is an infrequent, but life-threatening complication of ACE inhibition, and obviously, ACE inhibitors should not be used in patients with a history of this condition.
Enalapril (Vasotec) and Captopril (Capoten), have been shown to decrease mortality in large heart failure clinical trials. For this reason, these two are typically the drugs of choice.

Beta-Blockers
Beta-blockers exert their cardioprotective effects through blockade of excessive sympathetic stimulation of the myocardium, peripheral vasculature, and kidneys. While a short-term fall in myocardial contractility is to be expected, it is usually followed by a rise in ejection fraction over the next 1-3 months of use.
In the past, beta-blockers were believed to be contraindicated in patients with heart failure because of the LV depression that occurs with short-term use. More recently, the favorable long-term effects of beta-blockade on the heart have been recognized, and the ACC/AHA guidelines support the use of beta-blockers for patients with stable NYHA Class I, II or III heart failure related to LV systolic dysfunction.
Beta-blockers are generally well-tolerated. Hypotension associated with dizziness, light-headedness, or blurred vision may occur within the first few days of treatment, but tends to subside with continued drug administration. Decreases in heart rate and alterations in cardiac conduction produced by beta-blockers may also lead to to bradycardia or heart block. These changes can be severe, causing symptomatic hypotension, especially when high doses are used. In these cases, the dose must be reduced or discontinued if the condition persists.
Carvedilol (COPERNICUS Trial), bisoprolol (CIBIS-II Trial), and metoprolol CR/XL (MERIT-HF Trial) have all shown to decrease mortality in patients with mild to severe HF1. Currently, carvedilol and metoprolol-CR/XL are the only FDA approved beta-blockers for HF patients.
