2010). Its prevalence is rising due to an increase in its risk factors, most notably an increased
population life expectancy (Patient.co.uk, 2014). In the scenario Mr Williams is an obese 65 year old
man, exhibiting numerous symptoms indicative of congestive heart failure. In this PBL write up, I aim
to explore the issues raised, whilst relating them to the pathophysiology of heart failure.

Whilst discussing the scenario, our group came up with the following objectives:

We identified pulsus alternans as an unknown term from the scenario.
Pulsus Alternans is characterised by alternate strong and weak beats during a regular sinus rhythm
(Weber, 2003). The systolic pressure can vary up to 50mmHg between beats (Kumar and Clark, 2009).

Heart failure is a result of any structural or functional cardiac disorder, leading to an impaired ability

tube for confirmation of proper insertion. Another major complication when using NG tubal feeding is
aspiration within the lungs due to gastric substances enter the trachea and into the bronchial spaces of
the lungs. Keeping the head of the bed elevated greater than 30 degrees will reduce aspiration to least
possibility. Applying a gastrostomy or jejunostomy tube for enteral feedings when NG tube has
interference with therapy or ensues intolerance. The gastrostomy tube, PEG, tip situated in the
stomach and exits the body through the left upper quadrant of the abdomen where a bumper holds it

The setting of this performance was just in the classroom. It was very informal, which was nice. The
audience was mostly students from our class as well as being students from a theory class. The first
piece String Quartet No. 5, Op. 18, by Beethoven, was very upbeat. It felt like I was in at formal dance
or almost like a ballroom at a fancy party. It was very interesting to listen to as it had a very brisk

presence of a song that was influenced by Asian music. This song, unlike the last one, was more

the song sounded as if it was played in a minor key. It was quite harsh and gave me the impression
that something bad was happening. The final song was Four, tango by Astor Piazolla gave a sense of a

develop, when identifying the psychology behind the human performer, it is imperative to understand
the different philosophies and known topics that are associated to this module, these ideas and

In recent months there has been a discussion about building a wall on the United States Mexico
border. Unfortunately, it would cost over ten and a half billion

would have to be patrolled constantly in order to keep illegal immigrants from just going over it.
These costs, along with additional cost such as maintenance, makes the wall a fiscally irresponsible
plan. It is also important to consider that illegal border crossing are just one way to enter the Unite
States, according to Pew Research every year almost half a million immigrants come in with legal
visas and then simply stay passed the expiration date (http://www.pewresearch.org/fact
