This 15-minute lesson aims to enhance athlete performance through imagery exercises recalling past successes. The rationale is that imagery improves self-confidence, motivation, attention and anxiety management, and is used more by successful athletes. Participants will individually visualize a past best performance in detail using senses to relive positive emotions and techniques. They will also visualize a poor performance to gain self-awareness. The goal is to help athletes duplicate past successes through imagery by enhancing abilities and reducing stress. Studies show imagery benefits professional athletes and is important for success.

Le occlusioni venose retiniche sono caratterizzate dall’ostruzione della circolazione di una vena retinica con formazione di emorragie retiniche e possibile formazione di aree ischemiche e/o di edema maculare. La diagnosi è basata sulla clinica sulla fluorangiografia e sull’OCT. Si deve sempre effettuare un inquadramento sistemico del paziente. La terapia oculistica si avvale dell’argon laser retinico e di farmaci intraviterali.
Le occlusioni arteriose retiniche sono caratterizzate dall’ostruzione della circolazione di una arteria retinica con ischemia retinica corrispondente all’area di mancata irrorazione sanguigna. La diagnosi è clinica, si può effettuare una fluorangiografia e un OCT. Si deve sempre effettuare un inquadramento sistemico del paziente. La terapia deve essere tempestiva.

8. SEGNI E SINTOMI:
Rimane asintomatica per lungo tempo.
La triade sintomatologia è:
1)Sincope;
2)Angina;
3)Dispnea da sforzo e sintomi da scompenso cardiaco.
La sincope improvvisa vasodilatazione periferica cui consegue ipotensione e riduzione della
perfusione cerebrale. Può inoltre essere determinata da un'aritmia, in particolare tachicardia
ventricolare o fibrillazione ventricolare non sostenuta.
Se si verifica sotto sforzo è indice di stenosi aortica grave e sottende ad una GC fissa
Nei casi gravi, la PA è ridotta e la pressione differenziale è bassa (polso piccolo e tardo).
L'angina da sforzo colpisce circa 2/3 dei pazienti (la metà dei quali presenta una patologia
coronaria associata). L'angina, nei pazienti che non hanno una patologia coronaria, è dovuta ad
uno squilibrio tra l'apporto di ossigeno ed il fabbisogno del miocardio ventricolare determinato
dall'ipertrofia del ventricolo, dalla compressione delle porzioni intramiocardiche delle arterie
coronarie durante la sistole prolungata o dalla riduzione del tempo di perfusione diastolica in
caso di tachicardia.
Lo scompenso cardiaco si manifesta inizialmente come riduzione della tolleranza all'esercizio
fisico e col tempo progredisce alla dispnea franca.

10. Il soffio della SA è sistolico rude, irradiato al collo,
sempre palpabile come fremito sul focolaio aortico
e rinforzato nella mesosistole.
Il soffio eiettivo della SA presenta sempre
frequenze miste e diventa particolarmente aspro
quando il grado è 4/6.
Più tardivo è l'acme del crescendo e più
lungo il soffio, più grave è la stenosi
Nell'anziano, il soffio della SA calcifica assume
spesso carattere musicale ed è udibile soprattutto o
solamente all'apice, forse perché può non esserci
fusione delle commissure e le cuspidi possono
vibrare e produrre frequenze pure.

12. Esami da richiedere per fare diagnosi:
Ecocardiogramma
Ecocolordoppler cardiaco

14. Può rimanere asintomatica, fino alla sua evoluzione in
scompenso cardiaco, evoluzione che però si protrae per
decenni e non sempre avviene.
I sintomi tipici sono:
Cardiopalmo, dovuto a fibrillazione atriale o ad
extrasistoli ventricolari
Dispnea, nel caso si arrivi allo scompenso ventricolare
Astenia e affaticamento
15. L’esame obiettivo e con l’auscultazione permette al
medico di individuare una serie di segni:
Soffio sistolico, che è il principale segno clinico della
malattia. Questo non è in diretto rapporto con la
gravità della valvulopatia. Infatti se il soffio è intenso
può indicare un'insufficienza lieve, mentre un soffio di
debole intensità rivela una forma più grave in quanto
la valvola non riesce a fare la minima resistenza al
flusso retrogrado.
Primo tono ridotto, a causa della mancata coaptazione
dei lembi della valvola durante la sistole.
Secondo tono normale o leggermente aumentato.
Comparsa del terzo tono

18. L’ IM acuta da rottura di corda tendinea spesso
richiedono un intervento cardiochirurgico di urgenza.
L'esordio è abbastanza drammatico per l'improvviso
aumento del volume telediastolico del VS con
innalzamento della pressione di incuneamento
capillare polmonare che causa congestione polmonare.
Sono comunque descritti rari casi in cui la terapia
farmacologica che riduce il post carico aiuta a
massimizzare la gittata anterograda e ridurre la
frazione di rigurgito e differire l'intervento.
19. La forma acuta è causata da un'endocardite
acuta o dalla rottura di un muscolo papillare e,
la conseguente insufficienza , non concede
all'atrio sinistro il tempo di dilatarsi e di
ipertrofizzarsi per compensare l'aumento di
volume e di pressione.
Si crea così un rapido aumento di pressione
all'interno dell'atrio e nelle vene polmonari,
quindi nei capillari polmonari con conseguente
edema polmonare.

21. La terapia medica data dall‘ ospedale alla dimissione è
realistica?
Che difficoltà si trova nella gestione del paziente affetto da
valvulopatia o che ha eseguito una procedura
interventistica?
La rete ed i contatti tra i medici di base ed i
pazienti che hanno eseguito procedure interventistiche
esiste, si può migliorare?
