This document summarizes a training for navigators at the Philadelphia Department of Public Health AIDS Activities Coordinating Office CLUB 1509 Program. The training covered the importance of self-care and supervision to prevent compassion fatigue. It discussed how lack of self-care and proper supervision can lead to negative consequences like negligence, malpractice, and boundary violations. The training provided strategies for developing a self-care plan, creating a support system, and maintaining work-life balance.
In our roles as Victim Advocates/Service Providers, we put other people's needs above our own in our working life, often times resulting in neglecting ourselves. Helping professionals may find themselves at risk of becoming overwhelmed with other's pain-possibly resulting in burn out or compassion fatigue.
This workshop helped participants identify signs of burn out and compassion fatigue. Participants were provided tools to help transform stress and become relaxed and rejuvenated, resulting in better care of others as well as ourselves. The workshop included information on breathing, diet, self awareness and meditation, through movements associated with the ancient practice of Qigong.

Interview with Deb Melanson
Deb has worked as a case worker for 7 years. She currently works in a woman’s shelter in the Portsmouth area. She is 44 years old and loves doing her job although she admitted it can be very tiring with long days and nights. “I take some of my work home with me, even after I have already left the office” she stated. There may be emergency situations and I may need to go visit a client or go to a court session. Some days will be simple with routine visits and checkups with families I have been working with. Others take all my energy in cases where my client has disappeared, injured themselves or family, or just given up. These are the situations that are very emotional because I have worked with these individuals for a few weeks, months, and then it turns to the worse” (Melanson,2016).
When asked if Deb has ever experienced burnout she admits that she has been overwhelmed in the past with trying so hard to help her clients out and then they relapse or give up counseling. The stages of burnout that she experienced were stage 3 frustration. Deb has had several cases that have not ended very well and it got the best of her emotionally. She felt that she could have done more to help and that she let her clients down. She was thinking of ways it could have been done differently and doubting her ability. To Snap out of this phase and to make sure she does not return to it Deb gave me several strategies that she uses. 1. She spends time with her family and enjoys leisure time. Deb has a husband and a 24-year-old daughter. 2. She does not discuss anything work related or bring home any stresses, or emotions home to her family. 3. Deb finishes as much work load, visits, and case studies as she can while at work and limits others to Monday-Friday so that the weekends can be spent being refreshed. 4. Deb will attend support groups, trainings, and express herself to co-workers, management when needed. She said she does not let it get bottled up.
Burnout is defined as an internal psychological experience involving feelings, attitudes, motives, and expectations. Burned out means the energy of a person has been consumed by helping someone else. The energy crisis occurs because the psychic demand exceeds the supply. Burnout is experienced as a state of physical, mental, and emotional exhaustion caused by long term involvement in emotionally demanding situations. This feeling is accompanied by physical depletion, feelings of helplessness and hopelessness, disillusionment, negative self-concept, and negative attitudes towards work, people, and life. It represents a breaking point where it is very hard to cope with the environment around. Compassion fatigue is caused by work related stress and it is a gradual loss of compassion over time. It can share some of the same symptoms as burnout but not as severe.

HIV/AIDS has been serious public health issue facing the city of Philadelphia for the last two decades. According the AIDS Activities Coordinating Office, there are approximately 30,000 individuals living with HIV/AIDS in Philadelphia (AACO, 2012). This average is slightly higher than other major cities in the United States. AACO states the most vulnerable population is young men who have sex with men of color (MSM) between the ages of 15 and 25. This young population makes up for 56% of all new diagnosis of in the city (AACO, 2012). It is imperative for Philadelphia Fight to stay innovative with their approach to tackling sexual health issues that face this city. One major way Philadelphia Fight is addressing is epidemic is through opening the Youth Health Empowerment Project (Y-HEP).
This support manual describes the features of the CLUB 1509 HIV Navigation Program and provides sample demonstrations of the assessment tools, and program standards. It was written for the navigation teams who would like a point of reference for all CLUB 1509 service tools, client flow chart, and program standards.
This support manual is organized by task. It begins with the philosophy of care for all CLUB 1509 clients and progresses through more complex tasks such as client home visits, and biopsychosocial assessments. This supportive manual is not intended to replace your formal social work experience or your agency’s administrative protocol. This manual aims to introduce you to and support your journey in the CLUB 1509 program.

A Supervisor offers your support and explore with
the supervisee how the act of doing great navigation
work effects and influences the supervisee.
Use your supervisor as a point of reference whenever you

15. Preventing Compassion Fatigue:
The Role of Self Care in Navigation Services
Vicarious Trauma- is the emotional residue of exposure
from constant rehearing of their trauma stories and
witnessing pain, fear, and terror expressed by trauma
survivors.
Compassion fatigue- as a result of the emotional
demands of the work, CF develops when the
consequences of frequent exposure to trauma stories,
organizational stress, limited work resources culminate
and create physiological, emotional and spiritual fatigue
impacting empathetic responses and the inability to find
pleasure in working.
16. What is Self-Care
Self-care is defined as self-
nurturing methods that
affirm and protect social
workers as they undertake
the care of others (Sakvitne
& Pearlman, 1996).
Self-care is called, “an
essential underpinning to
the best practice in the
profession of social work” by
the NASW (Page 270, 2011).
17. Meet Susan
Susan is a CLUB 1509 Navigator who has been working in a
HIV/AIDS prevention and counseling for approximately three
years since obtaining her degree.
While Susan initially felt fulfilled and inspired by her work, for the
past year she has dreaded going to work each day and has even
called in sick a number of times due to feeling completely fatigued
and worn out.
Susan reported that she has been having more and more difficulty
“leaving work at work” and found herself thinking of the clients
she works with at night and on weekends.
Susan reported feeling overwhelmed by her caseload, which is
comprised entirely of men and trans people who have experienced
trauma.
She reported feeling unsafe most of the time in her personal life
and has not dated for close to a year. She indicated that she was
having so much difficulty sleeping.
Susan was waking up late for work and even missed a few
appointments due to her lack of sleep.
18. What Happened Next?
Things come to a head for Susan when she showed up late
for work one day to find her supervisor meeting with one of
her clients who was endorsing significant suicidal ideation.
Susan’s supervisor becomes furious as she begins to review
the client’s file which is missing session notes and has no
previous documentation regarding assessment for safety.
As Susan’s supervisor begins questioning Susan, she notes
that Susan is not being attentive to them.
Upon further review of Susan’s files, the supervisor finds
numerous errors and omissions including no
documentation of informed consent.
Later that morning, Susan’s supervisor receives a call from
Susan telling her that another one of her clients had called
to find talk about there HIV test results, and Susan missed
that appointment as well.

What might Susan have done differently to have a
better outcome?
What could have been implemented to help prevent
Susan from violating her ethics code and experiencing
burnout?
20. Strategies
As Susan’s mood, functioning, and work
satisfaction began to decline she could have
implemented strategies at the
organizational, professional, and personal self-
care levels.
Talking to her supervisor about diversifying her caseload and work
responsibilities
Receiving Supervision consultation
Communicating with others about her symptoms
Balancing her life to include other relaxing or fun activities

Periodic assessment of her levels of burnout

22. How Does Ethics Play into Self Care?
If someone from the navigation team is experiencing burnout, or
compassion fatigue, they may be at higher risk of using poor
judgment regarding their own abilities and competence to provide
services and/or supervise others providing services.
This puts providers at risk for negligence and even
malpractice.
23. Negligence and Malpractice Occur when:
Intervention used are not within the realm of accepted professional
practice
Procedures or techniques are used without proper training or
established understanding
Providers fail to use treatment(s) that would have been more
effective.
Providers fail to take action due to being overwhelmed/unhealthy
Adverse events such as suicide or homicide occur due to negligence
on provider’s part.

24. Boundary Violations (Reminder From Previous Training)
Blurring boundaries is another ethical pitfall for the unhealthy
navigation team.
The inability to maintain a professional stance due to the negative
effects of burnout or compassion fatigue would have terrible
repercussions for client and navigator alike.
Being vigilant about boundaries takes energy and clear thinking and
every attempt should be made to act purposefully to avoid going down
the slippery slope of boundary violations.
The unhealthy navigator may not readily identify entering into dual
relationships, which almost always result in problems for all involved.
In addition, while a healthy navigation team can easily identify their
own professional weak spots, those impacted by burnout or compassion
fatigue may have a compromised ability to do so.

Other ethical issues that may arise for
unhealthy navigation teams
include lapses in judgment regarding:
Mandatory reporting
Breach of confidentiality
Exercising privilege
Duty to warn
