Expo is the most significant annual health and social care event in the calendar, uniting more NHS and care leaders, commissioners, clinicians, voluntary sector partners, innovators and media than any other health and care event.
Expo 15 returned to Manchester and was hosted once again by NHS England. Around 5000 people a day from health and care, the voluntary sector, local government, and industry joined together at Manchester Central Convention Centre for two packed days of speakers, workshops, exhibitions and professional development.
This year, Expo was more relevant and engaging than ever before, happening within the first 100 days of the new Government, and almost 12 months after the publication of the NHS Five Year Forward View. It was also a great opportunity to check on and learn from the progress of Greater Manchester as the area prepares to take over a £6 billion devolved health and social care budget, pledging to integrate hospital, community, primary and social care and vastly improve health and well-being.

HIV/AIDS has been serious public health issue facing the city of Philadelphia for the last two decades. According the AIDS Activities Coordinating Office, there are approximately 30,000 individuals living with HIV/AIDS in Philadelphia (AACO, 2012). This average is slightly higher than other major cities in the United States. AACO states the most vulnerable population is young men who have sex with men of color (MSM) between the ages of 15 and 25. This young population makes up for 56% of all new diagnosis of in the city (AACO, 2012). It is imperative for Philadelphia Fight to stay innovative with their approach to tackling sexual health issues that face this city. One major way Philadelphia Fight is addressing is epidemic is through opening the Youth Health Empowerment Project (Y-HEP).

This support manual describes the features of the CLUB 1509 HIV Navigation Program and provides sample demonstrations of the assessment tools, and program standards. It was written for the navigation teams who would like a point of reference for all CLUB 1509 service tools, client flow chart, and program standards.
This support manual is organized by task. It begins with the philosophy of care for all CLUB 1509 clients and progresses through more complex tasks such as client home visits, and biopsychosocial assessments. This supportive manual is not intended to replace your formal social work experience or your agency’s administrative protocol. This manual aims to introduce you to and support your journey in the CLUB 1509 program.

Linkages & Progress Notes
2. LINKAGES
• Linkages will include direct contact between the navigation team and
the service providers to which the client is being referred.
• All linkages need to be documented
• A linkage is defined as one service visit.
• Follow-up with the client and agency on linkage.
• As often as possible, a member of the navigation team accompanies
the client to the first contact between services providers and the client.
• The client is linked to all services identified in the CLUB 1509 Action
Plan, including Ryan White Case management, social services and
PrEP services, as soon as possible.
3. COORDINATION OF SERVICES
• Process of connecting client to needed services and programs.
• Services should be timely, appropriate and of high quality.
• Process involves:
– Identifying resources
– Providing referrals
– Aiding linkage
– Giving information
– Coaching
– Managing the overall process
4. PROGRESS NOTES
• Produce a progress note for every contact.
• Always ask the client if the contact information is
still the same.
• Has the client had contact with the assistant
navigator? When?
• Did the client follow-up on a referral? What were
the results of the referral?
• What is the clients mood?
• Is the client still taking PrEP?
• Is the client still taking their HIV medication?
5. PROVIDING ONGOING THERAPEUTIC

• Using your Motivational Interviewing and Integrative Next Step
Counseling (iNSC) skills
• When a client no longer interested in program
• Client may not be taking their PrEP or ARV medication
• Client may not have stable housing.

James and John have been in a long term
relationship for 5 years. James expresses to you he
would like to be on PrEP but is nervous about having
the discussion with John.
• What can you do as the navigator support his
choice to be on PrEP?

with his paramour about PrEP?
• How can you best support James in the referral to
PrEP?
• What aspects of Jame’s Bio-Psycho-Social
Assessment would be most pertinent to consider?
• How should this progress note be written out?
7. EXAMPLE CASE
After testing negative Julio expressed an interest
in the CLUB 1509 program. When speaking to your
assistant navigator you find out he cannot read
English well. As a result, you have to read
everything to him.
• How can you support Julio once he decides to be
in the CLUB 1509 program?
• Does a referral need to be made?
• How can you address the issue of literacy in
future documentation?
• How could his low English proficiency impact
linkage into services?
