3. TEAM APPROACH TO SERVICE
COORDINATION
• CLUB 1509 is the first initiative at AACO that embraces the importance to a team
approach to providing intervention services
• Navigators and Navigator Assistants work in a collaborative fashion with each
individual client
• Both the Navigator and Navigator Assistant have differing backgrounds and
perspectives with which to do the work
• It is vital that Navigator and Navigator Assistant understand each other’s role and
consider the most effective methods of communication

• Clinical social service
professionals who utilizes
evidence base tools to
provide services.
Additionally, provides
supervision to the Assistant
Navigator.

• The navigator assistants
can use their lived
experience to combat their
clients social barriers. Also,
to do outreach to clients
who may need social
services.

days (more frequently as assessed)
• Therapeutic discharge up to 6 months
(a year if needs persist)
