Ongoing assessment is a key component of social work services. Social workers should assess clients at every opportunity to meet or communicate, focusing on progress towards goals, adherence to prevention plans, and new medical, social, or mental health needs. Reassessment helps monitor effectiveness and determine if goals or plans need updating. It is an ongoing process, not a single event, and includes revisiting initial needs and priorities identified. Domains of ongoing assessment include patterns of service use, living situation, communication preferences, literacy, culture, strengths, family relationships, functioning, independence, needed resources and supports, ability to access services, health conditions, and safety.

Evaluation for week 1, 2 and 3 mong nursing care after kidney transplantation (immediate, mediated, late), the material studied has as main findings to control hemodynamic status, blood pressure, respiratory function and capillary glucose levels; monitor the hydration situation; perform volume replacement, diuresis control every hour; fasting weigh Promote sodium and fluid restriction as indicated.
Restriction of salt and fluid becomes crucial in the management of oliguric kidney failure, wherein the kidneys do not adequately excrete either toxins or fluids (Workeneh & Batuman, 2022). What is the most important assessment for a nurse to make when caring for a client with AKI who has an elevated potassium level?
Nursing assessment for hyperkalemia patients focuses on monitoring for signs and symptoms of life-threatening cardiac dysrhythmias, as well as identifying and addressing the underlying cause of hyperkalemia. 1. Monitor heart rate and rhythm. Be aware that cardiac arrest can occur Furosemide can be used to correct volume overload when the kidneys are still responsive; this often requires high intravenous (IV) doses. Furosemide plays no role in converting an oliguric AKI to a nonoliguric AKI or in increasing urine output when a patient is not hypervolemic. Usually the presenting symptom is grossly bloody urine; the caregiver may describe the urine as smoky or bloody. Periorbital edema. Periorbital edema and/or pedal edema may accompany or precede hematuria. Fever. Glomerulonephritis signs and symptoms may include: Pink or cola-colored urine from red blood cells in your urine (hematuria) Foamy or bubbly urine due to excess protein in the urine (proteinuria) High blood pressure (hypertension) The hallmark of myasthenia gravis is muscle weakness that worsens after periods of activity and improves after periods of rest. Certain muscles are often (but not always) involved in the disorder such as those that control: Eye and eyelid movement General signs and symptoms include numbness or weakness of face, arm, or leg (especially on one side of the body); confusion or change in mental status; trouble speaking or understanding speech; visual disturbances; loss of balance, dizziness, difficulty walking; or sudden severe headache. 5 Ways to Build Employee Trust for Less Turnover and Fewer IncidentsCase IQ
Over the past few months, we’ve seen employees quit in record numbers. While there are many reasons for “The Great Resignation”, a standout is employee trust. A study from Gallup suggests that only one in three employees strongly agree that they trust the leadership of their organization.
Employees want to trust the companies they work at and the people they work with. They want to feel comfortable coming to HR with their complaints and concerns. But building trust takes time, and effort. As organizations are planning their post-pandemic strategies, now is a perfect time to place an intentional emphasis on building trust.

• Ongoing assessment is a key component of social work
services
• Every opportunity to meet or otherwise communicate with the
client is an opportunity for assessment
• The assessment is revolving around client progress towards
items on their action plan, client adherence to HIV prevention

social, or mental health needs
• Assessment can also come through third party observation
4. NASW STANDARDS ON ONGOING
ASSESSMENT

• During the reassessment process, the social work case manager and
client (and, if appropriate, other members of the client system) revisit the
needs, assets, and priorities identified in the initial assessment and
discuss the client’s emerging concerns.
• Reassessment serves both monitoring and evaluative functions,
enabling the social worker and the client to determine whether services
have been effective in helping achieve the client’s goals.
• On the basis of such reassessment, the case manager and the client
may determine that case management goals or service plans need to be
5. SOME DOMAINS OF ONGOING
ASSESSMENT
• patterns of service over- and underutilization
• motivating factors in seeking services

points of resilience
• family composition, structure, roles, and
communication
• physical, cognitive, and psychosocial
functioning
• desire and capacity for independence
• need for economic or other psychosocial
resources, supports, and services
• ability to navigate relevant service systems
