The name of the person beingobservedshouldnot be includedexceptforpositiverecognition.
• Upon completionof observation,discussanyat riskbehaviors/practices observedandoffer
safe alternatives.
• Inadditiontolookingfor‘at risk’behaviors,lookforsafe behaviors/practices.
• Alwaysendthe critique with the person/groupbeing observedwithpositiveremarksof safe

Basic Steps of an Observation
Step-1Observe workasit istakingplace.Quite oftenyouwill be involvedinthe workyourself.
• Observe people’sactivity,their actionsandsurrounding environment.
• Be careful notto startle or interrupta workerat an inappropriate time.
• Lookfor unsafe actsas well as good safetyperformance.
• Lookfor detail andan overall impression.
• Keepanopenmind.
Step-2
If yousee an act or conditionthatisunsafe oryou thinkmaybe unsafe, stopthe joband discuss
the situationwiththe individual(s) involved.
• Stopunsafe acts immediately,unlessstopping the workerwill create agreaterhazard.If the
actionis notimmediatelydangerousto life andhealth,use yourjudgmenttodecide if itis better
to waitfor a few moments.
• Be considerate and understanding,treataworkerthe wayyou wouldexpectedto be treated.
Askthe personor personsinvolvedinthe jobtoexplainwhattheyare tryingtoaccomplishand
whatthe proceduresare.
Step-3 Ask:
• Can youtell me aboutthe job?
• What are the hazards and risks?
• Have youcompletedaJSA/RAT?
• Do youhave a written procedure?
• Whydo you thinkIstopped you?
Askthem,whatare some thingsthatcouldgo wrong and how are some waystheycouldget
hurt duringthe job.
Step-4Ask:
• What couldgo wrong withthe job?
• Howcouldyou or others be hurt?
• Whoelse wouldbe affectedif you
Step-5 Ask,what are some waystheycouldcomplete the job safer.(change inprocedures,
differenttools,PPE)
• Allowthe workertoexplainhow he believesthe jobcanbe done more safely.
• Your jobisto listen,aworker that isallowedtofindhisown solutiontoasituationismore
likelytodothe jobright inthe future.
• If necessary,actas a coach to helpchange unsafe behavior.
• Provide praise whenajobis beingdone correctly.
Step6 Make an agreementtocorrectthe situationandcomplete the jobsafely.Everyone
involvedwill benefit!
Complete the appropriatebehaviorobservationcard.Be sure to mark all the requiredboxesand
provide abrief summaryof the unsafe act and corrective action.
Step-7
• Be specificandcheckthe appropriate categoryandbox(s) thatapply
• Give brief descriptionof observationin commentsection
• Listany corrective actionsanddates theyare to be completed
• Provide name of person(s) completing the observation
• Submitcardfor reviewandprocessing.
• Data can be trackedand trendedfor managementtoimprove.
Finally,share yourbehaviorobservationcardwiththe entire crew sothat everyone canbenefit
and worksafer. Practice BBS, Reduce Riskand Preventthisfromhappeningto youor one of your
co-workers!

  • 1. Safety training Observation Programme (STOP) STOP CARD: The card can also usedforNearMiss, Hazard ID, andH&S or Environmental events. Data can easilybe gatheredandtrended. All employeeshave the obligationtostopworkanytime theyfeelthat theirsafetyorthe safety of otheremployeesisatrisk. • Observationsdonotcontainnames,exceptforrecognition. • Alwaysendyourobservationbycomplimentinggoodbehaviors/practicesobserved. • Gatherand turn inobservationstoyoursupervisorfortrendingpurposes. Use the data to understandtypesof at risk behaviors,develop performance targets andto addressbehaviorstargetedforimprovement. • Use trendeddatato developactionplans andupgrade / customize training. • Share observations, trendeddataand performance targets withemployeesand other locations. The name of the person beingobservedshouldnot be includedexceptforpositiverecognition. • Upon completionof observation,discussanyat riskbehaviors/practices observedandoffer safe alternatives. • Inadditiontolookingfor‘at risk’behaviors,lookforsafe behaviors/practices. • Alwaysendthe critique with the person/groupbeing observedwithpositiveremarksof safe behaviors/practices observed. • Incentivesworkwell forbuildinganindepthsafety program.Examplesinclude best observation,most qualityobservations,etc. Benefitsof STOP for the Company • Enhancedreporting • IncreasedHazardRecognition • Share RecognizedHazards • Data gatheredisusedtodeveloptrends • Trendeddatacan be usedtoimprove overall safetyfor employees • Employeeshave greatersense of ownershipof the HSE program • Incentivesmaybe tiedtobestqualityobservations
  • 2. • IncreasedEmployee Retention Basic Steps of an Observation Step-1Observe workasit istakingplace.Quite oftenyouwill be involvedinthe workyourself. • Observe people’sactivity,their actionsandsurrounding environment. • Be careful notto startle or interrupta workerat an inappropriate time. • Lookfor unsafe actsas well as good safetyperformance. • Lookfor detail andan overall impression. • Keepanopenmind. Step-2 If yousee an act or conditionthatisunsafe oryou thinkmaybe unsafe, stopthe joband discuss the situationwiththe individual(s) involved. • Stopunsafe acts immediately,unlessstopping the workerwill create agreaterhazard.If the actionis notimmediatelydangerousto life andhealth,use yourjudgmenttodecide if itis better to waitfor a few moments. • Be considerate and understanding,treataworkerthe wayyou wouldexpectedto be treated. Askthe personor personsinvolvedinthe jobtoexplainwhattheyare tryingtoaccomplishand whatthe proceduresare. Step-3 Ask: • Can youtell me aboutthe job? • What are the hazards and risks? • Have youcompletedaJSA/RAT? • Do youhave a written procedure? • Whydo you thinkIstopped you? Askthem,whatare some thingsthatcouldgo wrong and how are some waystheycouldget hurt duringthe job. Step-4Ask: • What couldgo wrong withthe job? • Howcouldyou or others be hurt? • Whoelse wouldbe affectedif you

Step-5 Ask,what are some waystheycouldcomplete the job safer.(change inprocedures, differenttools,PPE) • Allowthe workertoexplainhow he believesthe jobcanbe done more safely. • Your jobisto listen,aworker that isallowedtofindhisown solutiontoasituationismore likelytodothe jobright inthe future. • If necessary,actas a coach to helpchange unsafe behavior. • Provide praise whenajobis beingdone correctly. Step6 Make an agreementtocorrectthe situationandcomplete the jobsafely.Everyone involvedwill benefit! Complete the appropriatebehaviorobservationcard.Be sure to mark all the requiredboxesand provide abrief summaryof the unsafe act and corrective action. Step-7 • Be specificandcheckthe appropriate categoryandbox(s) thatapply • Give brief descriptionof observationin commentsection • Listany corrective actionsanddates theyare to be completed • Provide name of person(s) completing the observation • Submitcardfor reviewandprocessing. • Data can be trackedand trendedfor managementtoimprove. Finally,share yourbehaviorobservationcardwiththe entire crew sothat everyone canbenefit and worksafer. Practice BBS, Reduce Riskand Preventthisfromhappeningto youor one of your co-workers! Behaviour modification programmes. The four programmesare  Time Out ForSafety(TOFS),  AdvancedSafetyAuditing(ASA),  SafetytrainingObservationProgramme (STOP) and  Care Plus.
