Felicia currently works, a professor or student will submit an electronic Purchase Order sheet
which indicates quantities, prices, descriptions and any other pertinent information such as a

Created to support this document is a self assessment tool for each Emergency Model of Care.
The purpose of the tool is to allow Local Health Districts to assess if a MOC is appropriate for
their ED or if an implemented MOC is functioning to its utmost potential. The tool is intended to
