1. Acute rheumatic fever is an autoimmune condition that can occur as a consequence of a streptococcal throat infection, causing inflammation in joints, brain, skin, and especially the heart.
2. It is diagnosed using the modified Jones criteria which looks for evidence of prior streptococcal infection and manifestations involving major criteria like carditis or chorea and minor criteria like arthritis or fever.
3. Treatment involves antibiotics to eradicate the streptococcal infection, anti-inflammatory drugs, and long-term antibiotic prophylaxis to prevent recurrent episodes from damaging the heart valves and leading to rheumatic heart disease.
Contains 17 clinical situations of prolonged fever and discussion of various differential diagnosis based on them. Also gives the key points in the diagnosis of a prototype diagnosis and the usefulness of a relevant investigation modality in identifying these conditions. The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

(DSM-5) integrates alcohol abuse and alcohol dependence into a single

In the DSM-5, all types of substance abuse and dependence have been
combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of

Valvular stenosis: appears several years or even

rarely, if ever, leads to permanent neurologic

27. Erythema Marginatum
A rare (<3% of patients with acute rheumatic fever)
but characteristic rash of acute rheumatic fever
It consists of erythematous,

Acute rheumatic fever typically develops 2-4 wk
after an acute episode of GABHS pharyngitis at a
time when clinical findings of pharyngitis are no
longer present & only 10-20% of the throat culture
or rapid streptococcal antigen test results are
positive
Therefore, evidence of an antecedent GABHS
infection is usually based on elevated or increasing
serum antistreptococcal antibody titers

DNase B, antihyaluronidase) measured: 95-100% have
an elevation
Therefore in suspectedARF clinically: perform
multiple antibody tests
Diagnosis of ARF should not be made in patients with
elevated or increasing streptococcal antibody titers
who do not fulfill the Jones criteria
True for younger, school-aged children having
GABHS pyoderma or GABHS pharyngitis
40. DIFFERENTIAL DIAGNOSIS
Patients with infective endocarditis: present with
both joint and cardiac manifestations
These patients can usually be distinguished from
patients with acute rheumatic fever by blood
cultures & the presence of associated findings
(hematuria, splenomegaly, splinter hemorrhages)
41. TREATMENT
Bed rest
Antibiotic Therapy:
10 days of orally administered penicillin or
erythromycin or a single intramuscular injection of
benzathine penicillin to eradicate GABHS from the
upper respiratory tract
Afterwards, the patient should be started on long-
term antibiotic prophylaxis
42. TREATMENT
Anti-inflammatory Therapy:
Anti-inflammatory agents (salicylates,
corticosteroids) should be withheld if arthralgia or
atypical arthritis is the only clinical manifestation of
presumed acute rheumatic fever
Acetaminophen can be used
Patients with typical migratory polyarthritis & with
carditis without cardiomegaly or congestive heart
failure:
treatment with oral salicylates, 100 mg/kg/day in 4
divided doses PO for 3-5 days, followed by
75 mg/kg/day in 4 divided doses PO for 4-8 wk
43. TREATMENT
Patients with carditis & cardiomegaly or congestive
heart failure:
treatment with corticosteroids
Prednisone 2 mg/kg/day in 4 divided doses for 2-6 wk
followed by a tapering of the dose that reduces the
dose by 5 mg/24 hr every 2-3 days. At the beginning
of the tapering of the prednisone dose, aspirin should
be started at 75 mg/kg/day in 4 divided doses to
complete 12 wk of therapy
44. TREATMENT
Supportive therapies for patients with moderate to
severe carditis include digoxin, fluid & salt
restriction, diuretics & oxygen
The cardiac toxicity of digoxin is enhanced with
myocarditis
45. TREATMENT
Sydenham Chorea
Occurs after the resolution of the acute phase of
the disease
Anti-inflammatory agents are usually not indicated

Tricuspid valvuloplasty may be required in rare cases
86. PULMONARY VALVE DISEASE
Pulmonary insufficiency usually occurs on a functional
basis secondary to pulmonary hypertension & is a late
finding with severe mitral stenosis
The murmur (Graham Steell murmur) is similar to
that of aortic insufficiency, but peripheral arterial
signs (bounding pulses) are absent
The correct diagnosis is confirmed by two-
dimensional echocardiography and Doppler studies

According to the WHO, 15.6 million people worldwide are living with RHD. Of the 500 000 who develop ARF each year, 300 000 go on to develop RHD and 233 000 deaths are attributable each year to ARF/RHD. The are conservative estimates and the true burden of disease is thought to be even greater. This mortality rates are higher than those of rotaviruses, meningitis and hepatitis B and half of those with malaria.

The Jones criteria were introduced in 1944 as a set of clinical guidelines for the diagnosis of RHF. They have subsequently undergone significant modifications ,the final ones published in 2002.
These revised WHO criteria speak to the diagnosis of :
a primary episode of RF
recurrent attacks of RF in patients without RHD
recurrent attacks of RF with RHD
rheumatic chorea
insidious onset rheumatic carditis
chronic RHD
It is important to note that in the context of a preceding streptococcal infection, 2 major criteria, or a combination of one major and 2 minor, provide reasonable evidence for a diagnosis of RF.
Major criteria: carditis, arthritis, chorea, subcutaneous nodules, erythema marginatum.
Minor criteria:
Clinical: fever, polyarthralgia
Lab: elevated acute phase reactants ESR/CRP

Erythema marginatum and subcutaneous nodules are the dermatological manifestations of ARF.
The only manifestation of ARF with potentially life-threatening and permanent sequelae is the carditis- as evident either as valvulitis( the precursor to rheumatic heart disease) pericarditis and myocarditis.ie a pancarditis.
Histologically Aschoff nodules is the hallmark pathognomonic feature and on special investigations the minor criteria such as high ESR or CRP is noted. Evidence of previous infection with GAS either via ASOT/anti-DNAse B titres or with precious evidence of streptococcus being cultured from the throat is an important adjunct to this diagnosis.
Currently carditis as diagnosed by echo alone is not included in the major criteria despite repeated calls for its inclusion. It is currently recommended that all patients with the clinical diagnosis of ARF even those without clinical evidence of carditis be referred for an echocardiogram( if available)
This long axis parasternal images demonstrates the progression of valvular disease in the period between ARF and RHD. It is important to remember that timeous diagnosis, treatment and secondary prevention may in cases prevent this progression.
