PTSD
Post-traumatic stress disorder (PTSD) is a
real illness. You can get PTSD after living through or seeing a traumatic
event, such as war, a hurricane, rape, physical abuse or
a bad accident. PTSD makes you feel stressed and afraid after the danger is
over. It affects your life and the people around you.
PTSD can cause problems like:

-- Feeling worried, guilty or sad
PTSD starts at different times for
different people. Signs of PTSD may start soon after a frightening event and

Medicines can help you feel less afraid and
tense. It might take a few weeks for them to work. Talking to a specially
trained doctor or counselor also helps many people with PTSD. This is called

Acute Stress Disorder
Acute stress disorder (ASD) is a mental disorder that can occur in the first month following a trauma. The symptoms that define ASD overlap with those for PTSD. One difference, though, is that a PTSD diagnosis cannot be given until symptoms have lasted for one month. Also, compared to PTSD, ASD is more likely to involve feelings such as not knowing where you are, or feeling as if you are outside of your body.
How common is ASD?
Studies of ASD vary in terms of the tools used and the rates of ASD found. Overall, within one month of a trauma, survivors show rates of ASD ranging from 6% to 33%. Rates differ for different types of trauma. For example, survivors of accidents or disasters such as typhoons show lower rates of ASD. Survivors of violence such as robbery, assaults, and mass shootings show rates at the higher end of that range.

What is Post-Traumatic Stress Disorder?
Post-traumatic stress disorder (PTSD) is an anxiety
disorder that some people develop after seeing or
living through an event that caused or threatened
serious harm or death. Symptoms include
flashbacks or bad dreams, emotional numbness,
intense guilt or worry, angry outbursts, feeling
“on edge,” or avoiding thoughts and situations
that remind them of the trauma. In PTSD, these
symptoms last at least
one month.
To aid those who suffer with PTSD, the National
Institute of Mental Health (NIMH) is supporting
PTSD-focused research, and related studies on
anxiety and fear, to find better ways of helping
people cope with trauma, as well as better ways
to treat and ultimately prevent the disorder.
This research fact sheet will highlight several
important areas that NIMH researchers have
recently learned about:
possible risk factors,
treating the disorder, and
next steps for PTSD research.

Research on Possible Risk Factors for PTSD
Currently, many scientists are focusing on genes
that play a role in creating fear memories.
Understanding how fear memories are created
may help to refine or find new interventions for
reducing the symptoms of PTSD. For example,
PTSD researchers have pinpointed genes
that make:
Stathmin, a protein needed to form fear
memories. In one study, mice that did not
make stathmin were less likely than normal
mice to “freeze,” a natural, protective
response to danger, after being exposed to
a fearful experience. They also showed less
innate fear by exploring open spaces more
willingly than normal mice.1
GRP (gastrin-releasing peptide), a signaling
chemical in the brain released during
emotional events. In mice, GRP seems to
help control the fear response, and lack of
GRP may lead to the creation of greater and
more lasting memories of fear.2
Researchers have also found a version of the 5-
HTTLPR gene, which controls levels of serotonin—
a brain chemical related to mood—that appears
to fuel the fear response.3 Like other mental
disorders, it is likely that many genes with small
effects are at work in PTSD.
Studying parts of the brain involved in dealing
with fear and stress also helps researchers to
better understand possible causes of PTSD. One
such brain structure is the amygdala, known for
its role in emotion, learning, and memory. The
amygdala appears to be active in fear acquisition,
or learning to fear an event (such as touching a
hot stove), as well as in the early stages of fear
extinction, or learning not to fear.4

The PTSD scale was established in 1989 for the U.S. Department of Veteran Affairs National Center for PTSD. The definition and analytic criteria of PTSD, that CAPS has been modified to the DSM-5 criteria,4 and has proved excellent psychometric properties when linked to its previous form. In other words, scholars have been improving a great deal of knowledge regarding individual who suffers from PTSD. Research found that post-traumatic stress disorder (PTSD) have been challenging in academic and clinical study. PTSD play a major part that relate to individual that suffer with this illness for example, death, traumatic event, witness a death, and sexual event. Ehlersand Clark’s cognitive model of PTSD 3,4 advanced the negative interpretations of the traumatic memory outcomes in heightened level of stress. In other words when a person rumination about certain tragedies their level of stress becoming more stressful.

Although originally considered an anxiety disorder, the DSM-5 removed PTSD from the classification of anxiety disorders and included it in a new chapter, Trauma- and Stressor-Related Disorders. PTSD was once considered a psychological condition of combat veterans who were “shell shocked” by and unable to face their experience on the battlefield. Individuals with PTSD were labeled as weak, faced rejection from their military peers and society in general, and were removed from combat zones or discharged from the military. Today we know that PTSD is a psychobiological mental disorder associated with changes in brain function and structure and can affect survivors of combat experience but also survivors of terrorist attacks, natural disasters, serious accidents, assault or abuse, and even sudden and major emotional losses (National Institute of Mental Health, 2014a). Scientists are focusing on genes that play a role in creating fear memories as well as studying parts of the brain that deal with fear and stress (Clapp, 2016). The DSM-5 criterion for PTSD has been expanded to include both direct and indirect exposure to potentially traumatic experiences (Uher et al., 2014).
Prevalence
Most of the research on PTSD has been conducted with male veterans of military combat. In the cohort of Vietnam veterans (now in the “baby boomer” cohort), 3 out of 10 experience PTSD. Among Afghanistan and Iraq veterans, 11% to 20% experience PTSD (United States Department of Veterans Affairs, 2014). Only recently realized is the fact that many World War II veterans have lived most of their lives under the shadow of PTSD without realization of their disorder. PTSD occurs increasingly in women, although research is scarce. Rape, child abuse, and domestic violence are the most likely traumas that will result in PTSD in women. With more women serving in the military, combat-induced PTSD among women is expected to increase (Kaiser et al., 2014a). Prevalence rates of PTSD among older adults have not been adequately studied, but estimates are that between 3% and 5% of individuals older than age 60 experience PTSD. Many older individuals may not meet the full criteria for a PTSD diagnosis but may still exhibit symptoms (partial or subsyndromal PTSD) (Chopra et al., 2014). The percentage of older individuals with subclinical levels of PTSD symptoms ranges from 7% to 15% (Kaiser et al., 2014a). Current estimates may underrepresent the prevalence of PTSD in older adults (Clapp, 2016). In addition to military combat, seniors in our care now have also experienced the Great Depression, the Holocaust, and racism events that also may precipitate PTSD. Although they may have managed to keep symptoms under control, a person who becomes cognitively impaired may no longer be able to control thoughts, flashbacks, or images. Discussion: Posttraumatic Stress Disorder (PTSD)
Posttraumatic Stress Disorder (PTSD) is known as a mental health condition or a psychiatric disorder that occurs in people who witnessed or experienced a traumatic event in which physical harm occurred or was threatened (Bisson, Cosgrove, Lewis, & Roberts, 2015). According to Lancaster et al. (2016), PTSD creates long-lasting consequences of traumatic ordeals that creates intense fear, feeling of guilt, helplessness, persistent sadness due to sudden death of loved one, natural disaster, major accident, war or combat, physical assault, rape, terrorist attacks. According to Pai et al. (2017), DSM-5 diagnostic criteria related to PSTD includes flashbacks of the trauma, nightmares, intense distress, panicking, lack of sleep or self-disturbance, self-destructive behavior, aggressive behavior, reduce interest, avoidance or avoiding distress memories, emotionally numb and increase the use of alcohol and drugs. In the case study of Thomson Family, William Thompson is a 38 years old African American who is the younger brother of henry. William is a military person and was involved in an Iraq war veteran who recently starts living with her brother in Pasadena, California (Laureate Education, 2012a). Both his brother and wife argue that William is suffering from PTSD but William does not accept this thing and show the symptoms of avoidance when someone tries to talk to him about that. William starts living with his brother when he was unable to pay his mortgage and now working on jeopardy due to his PTSD and alcoholic concerns. The behavior of William that aligns with the DSM-5 diagnostic criteria of PTSD includes avoidance, war veteran, alcoholic concerns, concentration issues on working due to which he was unable to pay the mortgage due to joblessness, working in jeopardy due to PTSD concerns and his brother also admits that William is suffering from PTSD.
Psychotropic Medications
Selective serotonin reuptake inhibitors (SSRIs) are the only FDA-approved drugs that are used in the treatment of PTSD (Ipser & Stein, 2012). In PTSD treatment sertraline antidepressants such as Zoloft, Pfizer and Paroxetine antidepressant HCl such as Paxil are recommended in the medication process. SSRIs work by helping to create a balance between certain chemicals such as neurotransmitter serotonin levels in the brain (Feduccia, et al., 2019). This chemical balance in the brain helps in regulating mood, improving sleep disturbance, improving appetite and decreasing other symptoms. According to Alexander (2012), the first-line treatment method for PTSD includes the use of Fluoxetine (Prozac) which helps in improving the energy level, restoring daily interest, decrease fear, unwanted thought, improve concentration and reduce panic attacks.

The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

Hematology/oncology fellows, attending faculty, and other health care professionals involved in the treatment of patients with mantle cell lymphoma (MCL).

Recomendações da OMS sobre cuidados maternos e neonatais para uma experiência pós-natal positiva.
Em consonância com os ODS – Objetivos do Desenvolvimento Sustentável e a Estratégia Global para a Saúde das Mulheres, Crianças e Adolescentes, e aplicando uma abordagem baseada nos direitos humanos, os esforços de cuidados pós-natais devem expandir-se para além da cobertura e da simples sobrevivência, de modo a incluir cuidados de qualidade.
Estas diretrizes visam melhorar a qualidade dos cuidados pós-natais essenciais e de rotina prestados às mulheres e aos recém-nascidos, com o objetivo final de melhorar a saúde e o bem-estar materno e neonatal.
Uma “experiência pós-natal positiva” é um resultado importante para todas as mulheres que dão à luz e para os seus recém-nascidos, estabelecendo as bases para a melhoria da saúde e do bem-estar a curto e longo prazo. Uma experiência pós-natal positiva é definida como aquela em que as mulheres, pessoas que gestam, os recém-nascidos, os casais, os pais, os cuidadores e as famílias recebem informação consistente, garantia e apoio de profissionais de saúde motivados; e onde um sistema de saúde flexível e com recursos reconheça as necessidades das mulheres e dos bebês e respeite o seu contexto cultural.
Estas diretrizes consolidadas apresentam algumas recomendações novas e já bem fundamentadas sobre cuidados pós-natais de rotina para mulheres e neonatos que recebem cuidados no pós-parto em unidades de saúde ou na comunidade, independentemente dos recursos disponíveis.
É fornecido um conjunto abrangente de recomendações para cuidados durante o período puerperal, com ênfase nos cuidados essenciais que todas as mulheres e recém-nascidos devem receber, e com a devida atenção à qualidade dos cuidados; isto é, a entrega e a experiência do cuidado recebido. Estas diretrizes atualizam e ampliam as recomendações da OMS de 2014 sobre cuidados pós-natais da mãe e do recém-nascido e complementam as atuais diretrizes da OMS sobre a gestão de complicações pós-natais.
O estabelecimento da amamentação e o manejo das principais intercorrências é contemplada.
