This document contains a self-evaluation form for a subordinate to rate their own task performance on a scale of 1 to 5. The subordinate is asked to rate their effectiveness in fulfilling roles and responsibilities, how well their performance matches their supervisor's expectations, and their competence as an employee. They are also asked to consider if their supervisor would like to change how they perform their job. The evaluation is intended to provide insight into how well the subordinate perceives they are meeting performance standards.
