This document provides instructions for employees to rate their effectiveness at innovative behaviors on a 7-point scale. It lists 14 innovative behaviors, such as operating new technologies, generating creative ideas, and finding solutions to difficult problems. Employees are asked to circle the number between 1 and 7 that best describes their effectiveness for each behavior, with 1 being very ineffective and 7 being extremely effective. A note at the bottom indicates this employee innovation scale was adapted from an existing innovative behavior scale.
