Definition: Patient-Centered Care
Definition Patient-centered care (patient centred care): “Is a model in which providers partner with families to identify and satisfy the full range of patient needs and preferences.”
To expand this definition, patient-centered care is dependent on the involvement of the staff and care team as well.
“To succeed, a patient-centered approach must also address the staff experience as staff’s ability and inclination to effectively care for patients is unquestionably compromised if they do not feel care for themselves" (Picker Institute).
Researchers from Harvard Medical School, on behalf of Picker Institute and The Commonwealth Fund, defined seven primary dimensions of patient-centered care model.
These factors are identified as:
Respect for patients’ values, preferences and expressed needs
Coordination and integration of care
Information, communication and education
Physical comfort
Emotional support and alleviation of fear and anxiety
Involvement of family and friends
Transition and continuity

Patient safety is the absence of preventable harm to a patient during the process of health care and reduction of risk of unnecessary harm associated with health.
A presentation on Malaysian Cancer Care Initiative 2017 hosted by Ramsay Sime Darby Asia. With an increased focus on involving patients to improve safety and quality as well as implementing sustainable cost-effective improvements, person-centred care is key.
Person centered care models with reference to dementia care, has demonstrated positive outcomes for behavioral disturbance. This presentation will increase awareness and understanding about person-centered care for people with dementia. Discussion includes complex needs of people with dementia, leading to compromised behavioral symptoms; including non-pharmacological approaches, sleep-wake-cycle disturbance, verbal outbursts and aggression. Further discussion encompasses evidence based outcomes with the use of person centered care that focuses on preserving the "personhood" of the individual.
The presentation describes in brief the patients need, expectations and how to develop the patient care and feedback system to obtain maximum patient satisfaction.

micro teaching on communication m.sc nursing.pdfAnurag Sharma
Microteaching is a unique model of practice teaching. It is a viable instrument for the. desired change in the teaching behavior or the behavior potential which, in specified types of real. classroom situations, tends to facilitate the achievement of specified types of objectives.
The prostate is an exocrine gland of the male mammalian reproductive system
It is a walnut-sized gland that forms part of the male reproductive system and is located in front of the rectum and just below the urinary bladder
Function is to store and secrete a clear, slightly alkaline fluid that constitutes 10-30% of the volume of the seminal fluid that along with the spermatozoa, constitutes semen
A healthy human prostate measures (4cm-vertical, by 3cm-horizontal, 2cm ant-post ).
It surrounds the urethra just below the urinary bladder. It has anterior, median, posterior and two lateral lobes
It’s work is regulated by androgens which are responsible for male sex characteristics
Generalised disease of the prostate due to hormonal derangement which leads to non malignant enlargement of the gland (increase in the number of epithelial cells and stromal tissue)to cause compression of the urethra leading to symptoms (LUTS
Ethanol (CH3CH2OH), or beverage alcohol, is a two-carbon alcohol
that is rapidly distributed in the body and brain. Ethanol alters many
neurochemical systems and has rewarding and addictive properties. It
is the oldest recreational drug and likely contributes to more morbidity,
mortality, and public health costs than all illicit drugs combined. The
5th edition of the Diagnostic and Statistical Manual of Mental Disorders
(DSM-5) integrates alcohol abuse and alcohol dependence into a single
disorder called alcohol use disorder (AUD), with mild, moderate,
and severe subclassifications (American Psychiatric Association, 2013).
In the DSM-5, all types of substance abuse and dependence have been
combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of
the 11 DSM-5 behaviors be present within a 12-month period (mild
AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria).
The four main behavioral effects of AUD are impaired control over
drinking, negative social consequences, risky use, and altered physiological
effects (tolerance, withdrawal). This chapter presents an overview
of the prevalence and harmful consequences of AUD in the U.S.,
the systemic nature of the disease, neurocircuitry and stages of AUD,
comorbidities, fetal alcohol spectrum disorders, genetic risk factors, and
pharmacotherapies for AUD.

Anti ulcer drugs and their Advance pharmacology ||
Anti-ulcer drugs are medications used to prevent and treat ulcers in the stomach and upper part of the small intestine (duodenal ulcers). These ulcers are often caused by an imbalance between stomach acid and the mucosal lining, which protects the stomach lining.
||Scope: Overview of various classes of anti-ulcer drugs, their mechanisms of action, indications, side effects, and clinical considerations.
1. Patient-centered care is the practice of caring for
patients (and their families) in ways that are
meaningful and valuable to the individual patient.
The IOM (Institute of Medicine) defines patient-
centered care as: “Providing care that is respectful of,
and responsive to, individual patient preferences,
needs and values, and ensuring that patient values
guide all clinical decisions.”
Patient-centered care
3. 1. Respect for patients’
values, preferences and
expressed needs
Involve patients in decision-
making, recognizing they
are individuals with their
own unique values and
preferences. Treat patients
with dignity, respect and
sensitivity to his/her cultural
values and autonomy.
4. 2. Coordination and integration of care
During focus groups, patients expressed
feeling vulnerable and powerless in the face
of illness. Proper coordination of care can
alleviate those feelings. Patients identified
three areas in which care coordination can
reduce feelings of vulnerability:
•Coordination of clinical care
•Coordination of ancillary and support
services
•Coordination of front-line patient care
5. 3. Information and education
In interviews, patients expressed their
worries that they were not being completely
informed about their condition or
prognosis. To counter this fear, hospitals can
focus on three kinds of communication:
•Information on clinical status, progress and
prognosis
•Information on processes of care
•Information to facilitate autonomy, self-care
and health promotion
6. 4. Physical comfort
The level of physical comfort patients
report has a significant impact on their
experience. Three areas were reported
as particularly important to patients:
•Pain management
•Assistance with activities and daily living
needs
•Hospital surroundings and environment
7. 5. Emotional support and alleviation of
fear and anxiety
Fear and anxiety associated with illness can
be as debilitating as the physical effects.
Caregivers should pay particular attention
to:
•Anxiety over physical status, treatment and
prognosis
•Anxiety over the impact of the illness on
themselves and family
•Anxiety over the financial impact of illness
8. 7. Continuity and transition
Patients expressed concern about their ability to
care for themselves after discharge. Meeting
patient needs in this area requires the following:
•Understandable, detailed information regarding
medications, physical limitations, dietary needs,
etc.
•Coordinate and plan ongoing treatment and
services after discharge
•Provide information regarding access to clinical,
social, physical and financial support on a
continuing basis.
9. 8. Access to care
Patients need to know they can access care when it is
needed. Focusing mainly on ambulatory care, the following
areas were of importance to the patient:
•Access to the location of hospitals, clinics and physician
offices
•Availability of transportation
•Ease of scheduling appointments
•Availability of appointments when needed
•Accessibility to specialists or specialty services when a
referral is made
•Clear instructions provided on when and how to get
referrals.
